Racial and ethnic differences in statin prescription and clinical outcomes among hospitalized patients with coronary heart disease.

We aimed to evaluate the association among race and ethnicity, statin prescription, and clinical outcomes among hospitalized patients with coronary heart disease (CHD), adjusted for confounders. Racial and ethnic disparities in CHD outcomes may be related to differential uptake of preventive medications, but data from real-world settings are limited. This was a 1-year prospective study of patients with preexisting CHD without a documented contraindication to statin (n = 3,067, 35% black or Hispanic, 65% white or Asian, 35% women) who participated in an National Heart, Lung and Blood Institute

Physical activity of Estonian family doctors and their counselling for a healthy lifestyle: a cross-sectional study.

BACKGROUND: Physical activity offers major health benefits and counselling for it should be integrated into the medical consultation. Based on the literature, the personal health behaviour of the physician (including physical activity) is associated with his/her approach to counselling patients. Our hypothesis is that family doctors (FD) in Estonia are physically active and their recommendation to counsel patients with chronic diseases to use physical activity is high. The study was also interested in how FDs value physical activity among other important determinants of a healthy lifestyle, e.

Implementing remote patient monitoring to improve hypertension control in a primary care network: Rationale and design of the monitor-BP Cluster Randomized Trial.

BACKGROUND: Home blood pressure monitoring with clinical or technological support (Supported HBPM) is an evidence-based intervention recommended by national guidelines for improving hypertension (HTN) control. However, few healthcare systems have implemented Supported HBPM because it remains unclear how best to promote uptake among patients and clinicians, and the real-world effectiveness of Supported HBPM is unknown. The Monitor-BP Trial aims to determine the impact of implementing a flexible Supported HBPM program on practice-level HTN control and to evaluate the reach, adoption, maintenance

National survey of pediatricians' opinions about and practices for acute otitis media and xylitol use.

BACKGROUND: Acute otitis media (AOM) is the main reason for physician visits and antibiotic prescriptions in children. Pediatricians (Peds) are gatekeepers for services and sources of information for families. The 2004 American Academy of Pediatrics/American Academy of Family Physicians (AAP/AAFP) Clinical Practice Guideline: Diagnosis and Management of Acute Otitis Media recommended preventative and management measures for Peds' practice. Treatments for AOM (antimicrobial therapy and surgery) sometimes have questionable effectiveness, risks, and high costs. Thus, Peds should consider using pr

Impact on psychosocial outcomes of a nationally available weight management program tailored for individuals with type 2 diabetes: Results of a randomized controlled trial.

AIMS: Type 2 diabetes mellitus (T2DM) can substantially decrease quality of life (QOL). This study examined the effects on QOL-relevant psychosocial measures of a widely available commercial weight loss program enhanced for individuals with T2DM. METHODS: A year-long multi-site randomized clinical trial compared the Weight Watchers (WW) approach, supplemented with phone and email counseling with a certified diabetes educator (CDE), to brief standard diabetes nutrition counseling and education (Standard Care; SC). Participants were 400 women and 163 men (N=279 WW; 284 SC) with T2DM [mean (±SD)

The use of the internet in data assimilation in rare diseases.

The incidence and prevalence of achalasia is 0.6/100,000 people per year and 1/10,000, respectively. It is difficult then for one center to accumulate a large cohort of patients. One study (Gut 33:1011,1992) described the presenting symptoms in only 38 patients. To approach this problem differently, we used the internet to access a larger patient population. Using search engines at Google and Yahoo, Inc., we identified achalasia support groups. We examined the most populated support group (YSG) and assembled the exchanged messages into 4 categories: support, symptoms, treatment, and diagnosis.

ACTIV-6: Operationalizing a decentralized, outpatient randomized platform trial to evaluate efficacy of repurposed medicines for COVID-19.

Despite the availability of vaccinations, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) continues to cause Coronavirus Disease 2019 (COVID-19) infection with a spectrum of disease in the acute setting. Transmission, infection, and severe disease remain common. There is a critical need to establish treatment regimens in the ambulatory setting that can reduce symptom burden and potentially prevent progression to severe disease and death. Many existing medicines previously approved for other uses may have benefit but remain unproven in informative clinical trials. Accelerating COVI

Nutritional management practices and perspectives of Radiation Oncologists: outcomes of an International Atomic Energy Agency survey.

BACKGROUND: Radiation therapy (RT) commonly leads to nutrition‑related adverse effects. Poor nutritional status is associated with worse outcomes, yet nutrition support remains underprioritized. This study explores Radiation Oncologists (ROs) perspectives and practices in nutrition management in RT settings across diverse global contexts. METHODS: An electronic survey developed by the IAEA was distributed globally through a mailing list and social media. The questionnaire comprised 38 items on hospital practices and professional perspectives. Data were analysed using descriptive statistics, C

Older racial and ethnic minority patients' perceptions of Medicare Annual Wellness Visits: a qualitative study.

PURPOSE: Although Medicare Annual Wellness Visits (AWVs) offer many potential benefits to older adults, racial and ethnic minorities have lower rates of AWV completion. Our objective was to understand older minority patients' attitudes and preferences related to preventive care and AWVs. METHODS: Between June and October 2024, we conducted four focus groups at two urban primary care settings-one academic health system, and one federally qualified health center-among Medicare enrollees aged ≥66 with ≥1 primary care encounter at a participating organization in the prior year. We recruited patie

What outcomes are important to people with foot and ankle disorders in rheumatic and musculoskeletal diseases? An OMERACT qualitative interview study across four continents.

BACKGROUND: The foot and ankle are frequently affected in rheumatic and musculoskeletal diseases (RMDs), yet there is a lack of high-quality evidence to determine the effectiveness of treatments. Outcomes in research are often inconsistently measured, impeding evidence synthesis. Additionally, clinical decisions are based on research outcomes, but these are not always regarded as important by people with RMDs. This study aimed to determine domains of importance to people with RMDs who have experienced foot and ankle disorders, and aid in developing a standardised core outcome set (COS) to addr

Long-Term Clinical Outcomes of Severe Combined Immunodeficiency Patients Given Nonablative Marrow Transplants.

BACKGROUND: Severe combined immunodeficiency (SCID) is a syndrome characterized by profound T-cell deficiency that is universally fatal in infancy unless immune reconstitution is achieved by hematopoietic stem cell transplantation, gene therapy, or enzyme replacement. Published long-term clinical follow-up is limited for transplanted patients with SCID. OBJECTIVE: To characterize the long-term outcomes of patients with SCID treated at a single center. METHODS: We examined the clinical outcomes of 177 successive SCID infants given allogeneic bone marrow over 38 years without pretransplant che

Action at a Distance: Geriatric Research during a Pandemic.

BACKGROUND: "Action at a distance" may be the new norm for clinical researchers in the context of the COVID-19 pandemic that may require social distancing for the next 18 months. We must minimize face-to-face contact with vulnerable populations. But we must also persist, adapt, and help our older patients and study participants during the pandemic. METHODS: Clinical researchers have an obligation to help, and we can. Recommendations for clinical researchers working with older adults during the COVID-19 pandemic are discussed. RESULTS: Implement technology now: Minimize face-to-face contact w
