Donor finger morbidity in cross-finger flaps.

We reviewed 16 patients who had cross-finger flap procedures carried out between 1991 and 1996 at the West Midlands Regional centre for Plastic and Reconstructive Surgery. We looked specifically at the donor finger morbidity of each patient with a median follow up interval of 43 months. At follow up there were 10 patients with cold intolerance, eight patients with subjective joint stiffness and a documented reduced range of finger joint movement. Skin graft reconstruction of the secondary defect was associated with poor colour match in eight cases (seven hyperpigmented, one hypopigmented) and v

No difference between two types of exercise after proximal phalangeal fracture fixation: a randomised trial.

QUESTION: Are 6 weeks of synergistic wrist and finger exercises with the metacarpophalangeal joint constrained in an orthosis (constrained exercises) more effective than traditional finger exercises with the metacarpophalangeal joint unconstrained (unconstrained exercises) after open reduction and internal fixation of a proximal phalangeal fracture in terms of impairment, activity limitation and participation restriction at 6 and 12 weeks? DESIGN: Randomised, parallel-group trial with concealed allocation, intention-to-treat analysis and blinded outcome assessors. PARTICIPANTS: Sixty-six par

Rupture of finger distal interphalangeal collateral ligament treated with a mini soft anchor.

BACKGROUND: Ruptured finger distal interphalangeal (DIP) collateral ligament is a rare injury, with controversial treatment until the present. Our case series aimed to demonstrate feasible surgical intervention using a mini anchor. METHODS: The present study includes four patients with ruptured finger DIP collateral ligaments who received primary repair in a single institute. They have suffered from joint instability due to ligament loss caused by infection, motorcycle accidents, and work-related accidents. All patients were operated on similarly using a 1.0 mm mini anchor for ligament reatta

Radiographic joint space width in individuals with hand osteoarthritis: Are their "Работает" joints really healthy?

OBJECTIVES: We aimed to investigate the systemic nature of hand osteoarthritis (OA). We hypothesized that people who suffer from hand OA would display narrower radiographic joint space width (JSW) - not only in joints with apparent radiographic OA but also in their unaffected "Работает" joints. METHOD: We examined 3394 participants from the Osteoarthritis Initiative with available dominant hand radiographs at baseline. Cases were defined as having interphalangeal OA (IPOA) based on a Kellgren and Lawrence (KL) score of ≥2 in two or more finger joints, whereas controls did not have IPOA. We use

Radioulnar interphalangeal joint angles in children and adolescents aged 0 to 19 years.

The purpose of this study is to determine the normal ranges of radioulnar (i.e. medial-lateral) finger deviations during growth. We retrospectively measured radioulnar interphalangeal joint angles in 6236 properly aligned thumbs and fingers in trauma radiographs of 4720 patients aged 0 to 19 years. The mean interphalangeal joint angle of the thumb was 0.2° (standard deviation 1.5°). The average proximal interphalangeal joint angles were ulnar deviation of 2.5° (1.7°) for the index, ulnar deviation 1.7° (1.5°) for the middle, radial deviation 1.3° (1.8°) for the ring, radial deviation 2.0° (2.8

Floating distal interphalangeal joint injury: case report.

A 37-year-old man sustained a floating distal interphalangeal (DIP) joint injury to the left index finger. The diagnosis was not confirmed until 3 months after the injury because of minimal deformity of the injured finger. Open reduction and internal fixation of the dorsally displaced floating DIP joint was carried out. Bone union was achieved but radiographs revealed osteoarthritis of the DIP joint.

Pin Insertion to the Interosseous Hood Minimize the Finger Motion Restriction for the Proximal Phalangeal Percutaneous Fixation: A Cadaveric Study.

Background: The purpose of this study was to identify the optimal pin insertion point to minimize finger motion restriction for proximal phalangeal fixation in cadaver models. Methods: We used 16 fingers from three fresh-frozen cadavers (age, 82-86 years). Each finger was dissected at the level of the carpometacarpal joint and fixated to a custom-built range of motion (ROM)-measuring apparatus after skin removal. The pin was inserted into the bone through four gliding soft tissues: the interosseous hood, dorsal capsule, lateral band, and sagittal band. Then, each tendon was pulled by a prescri

Biomechanical analysis of the human finger extensor mechanism during isometric pressing.

This study investigated the effects of the finger extensor mechanism on the bone-to-bone contact forces at the interphalangeal and metacarpal joints and also on the forces in the intrinsic and extrinsic muscles during finger pressing. This was done with finger postures ranging from very flexed to fully extended. The role of the finger extensor mechanism was investigated by using two alternative finger models, one which omitted the extensor mechanism and another which included it. A six-camera three-dimensional motion analysis system was used to capture the finger posture during maximum volunta

Denervation of the Joints of the Hand and Wrist: Surgical Techniques and a Systematic Review with Meta-Analysis.

BACKGROUND: Joint denervation of the wrist, basal joint of the thumb, and the finger is an option for patients with chronic pain. Compared with other surgical treatment options, function is preserved and the rehabilitation time is limited. A systematic review and meta-analysis were performed for each joint to determine whether the choice of technique and choice of denervation of specific articular sensory branches lead to a different outcome. METHODS: Embase, MEDLINE (OvidSP), Web of Science, Scopus, PubMed publisher, Cochrane, and Google Scholar database searches yielded 17 studies with repo

Fast spin echo-T2-weighted sequences with fat saturation in dactylitis of spondylarthritis. No evidence of entheseal involvement of the flexor digitorum tendons.

OBJECTIVE: To establish by means of fast spin echo (FSE)-T2-weighted sequences with fat saturation if enthesitis of the flexor digitorum superficialis and profundus tendons is the primary lesion in spondylarthritis (SpA) finger dactylitis. METHODS: Eleven dactylitic fingers and their corresponding normal, contralateral fingers, belonging to 6 patients who met the Amor criteria for SpA, were studied by FSE-T2-weighted sequences with fat saturation. RESULTS: All dactylitic fingers showed moderate or severe fluid collection in the flexor tendon synovial sheaths. Involvement of the joint cavity

[Autoplastic foot-to-hand joint transplantations].

Our surgical experience in the field of total and partial foot-to-hand joint transplants has evolved over the past 9 years (1973). In the case of serious traumatic injury of finger joints of the hand, we don't suggest any surgical solution other than that of arthrodesis or arthroplasty of the damaged joint. In our experience joint transplantation is a reconstructive operation of the whole functionality of the injured finger. This operation partially restores the active function of the finger involved and gives both the patient and the surgeon the possibility of an alternative to arthrodesis, a

Quantifying catch-and-release: the extensor tendon force needed to overcome the catching flexors in trigger fingers.

The extensor tendon forces required to overcome the catching flexors in trigger fingers are unknown. A biomechanical model with moment equilibrium equations and method of least squares was developed for estimating the tendon force at triggering in trigger fingers. Trigger fingers that exhibited significant catching and sudden release during finger extension were tested. A customized "pulling tester" was used to pull the finger from flexion to extension and provide synchronic measurement of the pulling force. The displacement of the tested finger was measured by a motion capture system. This pr

Examination of the Dorsal Finger Vein Anatomy Using a Vein Visualization Device.

Background: The vein anatomy of the dorsal finger is often difficult to identify suitable veins for anastomosis when treating digital amputations, but it has not been well studied to date. The aim of our study was to determine the vein anatomy of the dorsal finger using a vein visualization device. Methods: The study sample consisted of 20 volunteers (11 men and 9 women; 148 fingers and 37 thumbs). The number and location of veins, the distance from the finger midline to the most central vein, and the distance from the central vein to the adjacent vein were examined using a vein visualization

Perifascial Areolar Tissue Transplantation for Covering Exposed Proximal Interphalangeal Joint After Electric Burn.

Soft tissue damage in electric burn injuries is often deep and may expose tendons, bones, or joints in the hand. Here, we present the case of a 76-year-old man treated using perifascial areolar tissue (PAT) transplantation for covering the proximal interphalangeal joint of the middle finger, which had been exposed because of an electric burn. Following ointment therapy, surgery was performed on day 34 postinjury when a deep ulcer with opening of the proximal interphalangeal joint was observed on the dorsum of the right middle finger. After resecting the cartilage of the proximal interphalangea

Alternative hand flaps for amputations and digital defects.

Traumatic or thermal injury to the hand may result in a composite tissue loss. Exposed tendon, bone, or joint surface is best treated by flap coverage. This paper will discuss four types of alternative hand flaps that can be used to treat difficult areas and/or minimize complications. (1) A thumb flap elevated from the metacarpophalangeal joint flexion crease covers a fingertip injury and the donor site is closed primarily. (2) A proximally based side-finger flap, preferably from the ulnar side of the donor finger, can be used to cover a fingertip defect in a young person or a thumb tip in any

Irreducible Volar Subluxation of the Distal Interphalangeal Joint: An Unrecognised Cause.

This report describes a rare case of irreducible closed volar subluxation of the distal interphalangeal (DIP) joint of the small finger following a sports injury. Closed reduction failed, leading to open surgery, which revealed ulnar condyle entrapment between a partially torn extensor tendon and volarly displaced oblique retinacular ligament (ORL). Flexing the proximal interphalangeal (PIP) joint to 90° while extending the DIP joint relaxed the ORL, enabling dorsal repositioning and successful reduction. Awareness of this mechanism may help hand surgeons achieve effective reduction. Level of

Vascularized Composite Tissue Spare Part Transfer for Central Hand Defect Reconstruction: Case Report.

Injuries to the hand with loss of joints, tendons, nerves, and soft tissue may require complex, innovative reconstructive techniques to achieve a favorable functional and aesthetic outcome. We present a case of a manual laborer who sustained a multifaceted injury from a metal press machine with loss of composite structures including the middle and ring finger metacarpophalangeal joints, flexor and extensor tendons, digital nerves, and dorsal/volar soft tissues. Reconstruction included using the spare parts technique for transferring his ring finger proximal interphalangeal joint as a pedicle t

Arthroplasty of the metacarpophalangeal joints with use of silicone-rubber implants in patients who have rheumatoid arthritis. Long-term results.

The long-term results of 144 arthroplasties of the metacarpophalangeal joints, with use of silicone-rubber implants, were reviewed for twenty-seven patients (thirty-six hands) who had rheumatoid arthritis. The operations were all performed or supervised by the same surgeon. All patients had an arthroplasty of the metacarpophalangeal joint of all four fingers, and all had subjective and objective clinical evaluation and roentgenographic assessment. Relief of pain, the cosmetic appearance of the hand, the range of motion of the metacarpophalangeal and proximal interphalangeal joints, and the pre

Ulnar neuropathies in rheumatoid arthritis.

A loss of functional motor axons in the median and ulnar nerves occurred in half of thirty-three patients with rheumatoid arthritis. Weakness of small hand muscles may predispose to the development of ulnar deviation of the fingers in patients with joint disease at the radio-ulnar and metacarpophalangeal joints. There is no evidence that spasm of small hand muscles is a significant cause of ulnar deviation of the fingers in rheumatoid arthritis. Ulnar deviation of the fingers in rheumatoid arthritis is not due to selective impairment of the ulnar nerve or the deep palmar branch of the ulnar ne
