Anaesthetic arthrography of the shoulder joint in dogs.

OBJECTIVE: To evaluate the use of intra-articular anaesthesia combined with positive contrast arthrography (collectively called 'anaesthetic arthrography' [AA]) in the shoulder in order to identify and confirm the source of pain in lame dogs. METHODS: Anaesthetic arthrography was performed in 30 dogs with shoulder joint lameness with a mixture of contrast medium (iohexol: 240 mg I/ml) and a local anaesthetic (mepivacaine 2%: 20 mg/ml). The effect of intra-articular anaesthesia was evaluated by an objectified visual scoring system and the arthrograms were evaluated for their diagnostic value.

Movements of the forelimbs of the cat during stepping on a treadmill.

In normal cats stepping on a treadmill an analysis has been made of movements of the scapula, shoulder, elbow, wrist and digits. The scapula is capable of making large and complicated movements over the rib cage. In locomotion they may be resolved into a cyclical, similunar movement of the glenoid cavity in the parasagittal plane of the cat's body. The movements of the scapula are rather constant over a wide range of velocities. They most resemble those of the hip which also shows a delay in the onset of extension and no yield phase. It is suggested that movements of the scapula have a high pr

Can osteophytes be used as age at death estimators? Testing correlations in skeletonized human remains with known age-at-death.

Age-at-death estimation is one of the major challenges when establishing an adult skeleton biological profile. The presence of degenerative joint changes - e.g. osteophytes - has been regarded as a good postmaturity age indicator. This study assessed if a clear relationship between age and osteophytes exists. To accomplish this goal, a total of 16 joint surfaces, from the shoulder, elbow, wrist, hip, knee, and ankle, were examined in 604 adult individuals, of both sexes from two Portuguese Identified collections. Individuals had between 20 and 98 year old at death. Statistically significant re

[Standardized sonographic examination of the hip joint].

AIM: We demonstrate a technique for examining the hip joint sonographically in a standardised way using sectional planes corresponding to the guidelines provided by the work group "Sonography of the Musculo-Skeletal System" of the German Society of Ultrasound (DEGUM). MRT-scans were used as a comparative standard. METHOD: The ultrasound examination was performed in the following standardised way: a transversal and longitudinal scan were done in the ventral region and additionally a longitudinal scan in the lateral region. In cases without pathological findings two standardised planes should b

Clinical results of bony increased-offset reverse shoulder arthroplasty (BIO-RSA) associated with an onlay 145° curved stem in patients with cuff tear arthropathy: a comparative study.

BACKGROUND: The main limits of the Grammont design reverse shoulder arthroplasty (RSA) are loss of external rotation and scapular notching. They can be addressed with glenoid or humeral lateralization. The aim of the study was to compare outcomes of lateralized bony increased-offset RSA (BIO-RSA) vs. standard RSA in patients with an onlay 145° curved stem. METHODS: A comparative cohort study of 29 standard RSAs and 30 BIO-RSAs was performed. At 2 years postoperatively, Constant score, American Shoulder and Elbow Surgeons score, visual analog scale score, range of motion, and radiographs were

Effect of glenoid concavity loss on shoulder stability- a case report in a professional wrestler.

BACKGROUND: Current glenoid defect measurement techniques only quantify bone loss in terms of defect diameter or surface. However, the glenoid depth plays an important role in shoulder stabilization by means of concavity compression. CASE PRESENTATION: We present a case of a professional wrestler who suffered from anterior shoulder instability after sustaining a bony Bankart lesion without loss of glenoid surface area but flattening of the concavity due to medialization of the fragment. The patient's glenoid concavity was reconstructed arthroscopically by reduction and percutaneous screw fixa

How to deal with glenoid type B2 or C? How to prevent mistakes in implantation of glenoid component?

BACKGROUND: Although TSA has been shown to significantly yield better outcomes than hemiarthroplasty, glenoid prosthesis loosening remains the most common complication. Inadequate primary fixation enables the glenoid component to move. In primary glenohumeral osteoarthritis (GHOA), glenoid involvement and proper morphology vary considerably. Postero-inferior glenoid hypoplasia could be associated with some degree of osteoarthritis. According to Walch, 24 % of glenoids in GHOA are type B2 or C (excessive posterior retroversion), which increases the challenge for the glenoid component fixation.

Arthroscopic posterior bone block: surgical technique.

Posterior glenohumeral instability remains a difficult problem. There are still many controversies regarding surgical treatment, due to a lack of understanding the pathomechanical issues leading to posterior instability. This article presents a new arthroscopic technique of posterior bone block augmentation, which we found to be effective, repeatable and successful. This technique can be used for combined soft tissue and bony defects as well as for revisions after previous soft tissue reconstructions.

Analysis of operative versus nonoperative treatment of displaced scapular fractures.

BACKGROUND: Operative indications for displaced scapular fractures have been controversial and inconsistent. Surgeons have been dissuaded to operate on these uncommon fractures because of the complex anatomy, approaches, and fracture patterns. It is unclear whether return to work, pain, or complications differ in patients with scapular fractures treated nonoperatively or operatively. QUESTIONS/PURPOSES: We therefore assessed differences in rates of union, range of motion, ability to return to work, pain, and complications between operatively and nonoperatively treated scapular body and neck f

Type E2 glenoid bone loss orientation and management with augmented implants.

BACKGROUND: The purpose of this study was 2-fold: (1) to quantify type E2 bone loss orientation and its association with rotator cuff fatty infiltration and (2) to examine reverse baseplate designs used to manage type E2 glenoids. METHODS: Computed tomography scans of 40 patients with type E2 glenoids were examined for pathoanatomic features and erosion orientation. The rotator cuff fatty infiltration grade was compared with the erosion orientation angle. To compare reconstructive options in light of the pathoanatomic findings, virtual implantation of 4 glenoid baseplate designs (standard, ha

Structural glenoid grafting during primary reverse total shoulder arthroplasty using humeral head autograft.

BACKGROUND: Large glenoid bone defects in the setting of glenohumeral arthritis can present a challenge to the shoulder arthroplasty surgeon. The results of large structural autografting at the time of reverse total shoulder arthroplasty (RTSA) are relatively unknown. METHODS: This retrospective case series describes the clinical and radiographic results of large structural autografting from the humeral head to the glenoid during primary RTSA. RESULTS: Of 17 patients who met inclusion criteria, 14 (82% follow-up) were evaluated postoperatively at a mean of 2.6 years (range, 2.0-5.4 years). M

Analysis of Agreement Between Computed Tomography Measurements of Glenoid Bone Defects in Anterior Shoulder Instability With and Without Comparison With the Contralateral Shoulder.

BACKGROUND: Computed tomography (CT) is frequently used to diagnose glenoid bone defects in anterior shoulder instability. The assessment of glenoid defects on 2-dimensional (2D) and 3-dimensional (3D) CT scans has been reported with and without a comparative study of the contralateral shoulder; however, no previous studies have analyzed if these 4 methods agree. PURPOSE: To estimate agreement between CT assessments of glenoid defects by examination of the affected shoulder alone and by comparison with the contralateral side on both 2D and 3D CT scans. STUDY DESIGN: Cohort study (diagnosis);

Clinical and computed tomography-radiologic outcome after bony glenoid augmentation in recurrent anterior shoulder instability without significant glenoid bone loss.

BACKGROUND: The presence of a significant bony defect in anterior shoulder instability cases warrants glenoid reconstruction surgery typically by means of an autograft. Some surgeons use the same graft techniques even in the absence of a significant bony defect, thus augmenting the glenoid surface. The goal of this study is to investigate the clinical and computed tomography-radiologic outcome after glenoid augmentation surgery. METHODS: Between 2006 and 2011, 11 patients with recurrent anterior shoulder instability and glenoid bone loss of 5% or less were treated with an iliac crest autograf
