Management of bulimia nervosa.

Bulimia nervosa is a disorder of complex etiology that tends to occur in young women. These individuals binge eat and purge by vomiting or other means, and often have depression, anxiety, substance abuse and extremes of impulse control. It is thought that binge eating and purging behaviors are, at least in part, a means of coping with dysphoric mood states and interpersonal stress. Bulimic symptoms are not likely to abate without development of new coping skills and behaviors. In the past 25 years, considerable progress has been made in developing specific psychotherapies and medication for th

[Distinction, limits and complementarity between efficacy and effectiveness studies: new perspectives for psychotherapy research].

Perron et al. (2004) criticize the collective expertise conducted by the INSERM on the efficacy of different psychotherapy methods for different mental disorders. They say the work is biased in favour of Cognitive and Behavioral Therapies (CBT), while there is a negative bias regarding other methods, in particular psychodynamic therapies. Philippe Cialdella, a specialist of methodology and quantitative analysis in psychiatry, exposes an counter-argument in 10 points and brings welcome clarifications on the methodology and statistics. There is neither statistical bias in the INSERM report (whic

Identifying the most suitable treatment for depression based on patients' attachment: study protocol for a randomized controlled trial of supportive-expressive vs. supportive treatments.

BACKGROUND: In the absence of one intervention that can cure all patients with major depressive disorder (MDD), the leading cause of disability worldwide, increased attention has been focused on selecting the best treatment based on patient characteristics. Theory-driven hypotheses for selecting the best treatments have not yet been adequately investigated. The present study tested the a priory hypothesis that attachment orientations may determine whether patients benefit more from a treatment where alliance provides a facilitative environment for the treatment to work, as in the case of suppo

Nonsomatic treatment of depression.

There is solid evidence that active and goal-oriented cognitive-behavioral or relationship-focused therapies are generally superior to more generic therapies or to no treatment for clinically diagnosed and for undiagnosed but symptomatic youths. Between 50% to 87% of diagnosed youths who received a targeted treatment had recovered from their depressive episodes, in comparison to 21% to 75% of those who received some other generic therapy and 5% to 48% of wait-listed youths. The cognitive behavioral and relationship-oriented interventions that were tested tended to be even more successful in re

Developing facilitative interpersonal skills: A randomized controlled study of an apprenticeship training for first-year clinical psychology students.

OBJECTIVE: This study aimed to evaluate whether apprenticeship training could enhance facilitative interpersonal skills (FIS) in first-year students enrolled in a six-year integrated clinical psychology study program, compared to training-as-usual. METHOD: A total of 108 students were recruited and randomly assigned to either training-as-usual or training-as-usual plus apprenticeship. Students in the apprenticeship group participated in one weekly therapy session with different therapists over 10 weeks. FIS were assessed using a performance-based observer-rated test before and after the inter

Wise interventions: Psychological remedies for social and personal problems.

Long-standing social problems such as poor achievement, personal and intergroup conflict, bad health, and unhappiness can seem like permanent features of the social landscape. We describe an approach to such problems rooted in basic theory and research in social psychology. This approach emphasizes subjective meaning-making-working hypotheses people draw about themselves, other people, and social situations; how deleterious meanings can arise from social and cultural contexts; how interventions to change meanings can help people flourish; and how initial change can become embedded to alter the

Beyond the roadblocks: transitioning to adulthood with Asperger's disorder.

Growing up with Asperger's disorder is complex and fraught with difficulty. Although the literature includes some research related to the transition of youth with Asperger's disorder to school and employment, none pertains to the transition to adulthood and independent living. Although a marginal number of young adults with Asperger's disorder eventually achieve independence, many of them continue to depend on families for supportive services. Currently, health care organizations and social services lack coherent, integrated systems to assist youth with Asperger's disorder and their families w

Prodromes, coping strategies and psychological interventions in bipolar disorders.

Bipolar disorder is known to be highly recurrent and people with bipolar illnesses often experience high degrees of interpersonal and social impairment. The emergence of prodromal symptoms not only causes distress but may also predispose patients to greater risk of a full relapse. Studies have found that patients can report prodromes reliably. Common mania prodromes include decreased need for sleep, increased activities, being more sociable and racing thoughts while common depression prodromes are loss of interest, not being able to put worries aside and interrupted sleep. Furthermore, patient

The impact of brief dynamic interpersonal therapy (DIT) on veteran depression and anxiety.

A significant proportion of veterans continue to experience depression or anxiety following treatment with the empirically supported psychotherapies offered by the Veterans Health Administration. Continued development and testing of new treatments may be useful. Brief dynamic interpersonal therapy (DIT) is a short-term psychodynamic therapy developed for depression in the United Kingdom and is being disseminated as an alternative to cognitive behavioral therapy within the National Health Service. This study represents the first evaluation of DIT in the United States, implemented at a veterans

Psychotherapy of Dependent Personality Disorder: The Relationship of Patient-Therapist Interactions to Outcome.

So far, only a few studies have focused on psychotherapy for Dependent Personality Disorder (DPD). DPD is marked by a repetitive pattern of efforts aiming at maintaining close relationships, which may present as a lack of assertiveness and as a difficulty in making routine decisions. The present study aims at exploring processes of change taking place during the working phase of a clarification-oriented psychotherapy (COP) by focusing on the in-session patient-therapist interaction, as it changes during treatment and their links with treatment outcome. Methods: N = 74 patients with DPD were re

[Diagnostics and multimodal treatment of depression in old age : New developments].

Depressive disorders in elderly persons are frequent and can either first be manifested at a younger age and show a recurrent pattern during later life (ICD 10, F33.*) or have an onset beyond 60 years, which is termed late onset depression (LOD). This LOD has a higher tendency to chronification and implies an increased risk of progression, especially to vascular dementia. The multifactorial genesis of depression in old age includes psychosocial, vascular and metabolic factors and requires multimodal therapy modules at the biological and psychosocial level, which is now increasingly being empir

Update on the treatment of anorexia nervosa: review of clinical trials, practice guidelines and emerging interventions.

BACKGROUND: Anorexia nervosa is a potentially deadly psychiatric illness that develops predominantly in females around puberty but is increasingly being recognized as also affecting boys and men and women across the lifespan. The aim of this environmental scan is to provide an overview of best practices in anorexia nervosa treatment across the age spectrum. METHOD: A triangulation approach was used. First, a detailed review of randomized controlled trials (RCTs) for anorexia nervosa published between 1980 and 2011 was conducted; second, clinical practice guidelines were consulted and reviewed

Patients with cluster a personality disorders in psychotherapy: an effectiveness study.

BACKGROUND: While psychopharmacological studies are common in patients with cluster A personality disorders, the effects of psychotherapy have received little attention. The aim of this study is to explore whether psychotherapeutic treatment yields health gains for these patients. METHODS: The study was conducted between March 2003 and June 2008 in 6 mental health care centres in the Netherlands, with a sample of 57 patients with a DSM-IV-TR axis II cluster A diagnosis. Patients were assigned to 3 settings of psychotherapeutic treatment (outpatient, day hospital, inpatient), and effectiveness

Predicting response to cognitive therapy and interpersonal therapy, with or without antidepressant medication, for major depression: a pragmatic trial in routine practice.

BACKGROUND: Identifying patient characteristics that predict response within treatments (prognostic) or between treatments (prescriptive) can inform clinical decision-making. In this study, we sought to identify predictors of response to evidence-based treatments in a sample of depressed patients seeking help in routine practice. METHODS: Data come from a pragmatic trial of 174 patients with major depression who received an evidence-based treatment of their own choice: cognitive therapy (CT), interpersonal therapy (IPT), antidepressant medication (ADM) alone or in combination with either of t

Impact and clinical management of depression in patients with coronary artery disease.

The rates of major adverse coronary events, including recurrent ischemic events and death, in patients with coronary artery disease (CAD) have been shown to be significantly increased in patients with depression. In addition, health care costs are higher and health-related quality of life is lower in depressed patients with CAD. Several pathophysiologic mechanisms have been proposed for the association of increased events seen in this population. Studies have focused on antidepressants (specifically, selective serotonin reuptake inhibitors and mirtazapine), psychotherapy (cognitive behavioral

Evaluating Change in Transference, Interpersonal Functioning, and Trust Processes in the Treatment of Borderline Personality Disorder: A Single-Case Study Using Ecological Momentary Assessment.

Transference-focused psychotherapy (TFP) is an empirically supported treatment for borderline personality disorder (BPD) that improves functioning via targeting representations of self affectively relating to others, particularly as evoked in the therapeutic relationship. If change in TFP operates as theorized, then shifts in patterns of "self affectively relating to others" should be observed in the transference prior to shifts in daily relationships. Using ecological momentary assessment (EMA), a patient with BPD rated daily interpersonal events for 2-week periods during 18 months of TFP; at

Corrective Experiences to Enhance Trust: Clinical Wisdom From Good (Enough) Psychiatric Management.

Unstable trust within social interchange underlies the symptom constellation of borderline personality disorder (BPD), resulting in preoccupation with intense dyadic relationships, limited capacity for social collaboration, and constricted social networks. Good Psychiatric Management (GPM) provides a distilled formulation of how interpersonal hypersensitivities drive the engine of BPD's symptomatic oscillations in both affect and attachment. The authors summarize clinically relevant conclusions from the empirical literature on trust in BPD, synthesize it with selected ideas from other empirica

Complicated grief therapy as a new treatment approach.

Complicated grief therapy (CGT) is a relatively new psychotherapy model designed to address symptoms of complicated grief. Drawn from attachment theory and with roots in both interpersonal therapy (IPT) and cognitive-behavioral therapy, CGT includes techniques similar to prolonged exposure (repeatedly telling the story of the death and in vivo exposure activities). The treatment also involves focusing on personal goals and relationships. CGT has been demonstrated to be effective in a trial in which participants with complicated grief were randomly assigned to CGT or IPT; individuals receiving

Why Is It Still So Difficult to Acknowledge the Reality of Borderline Personality Disorder?

Borderline personality disorder (BPD) has emerged as a major clinical and societal challenge since its mid-twentieth-century conceptualization. This paper addresses four persistent obstacles to effective care: diagnostic complexity, interpersonal dysfunction and risk behaviors, high societal burden, and pervasive stigma. We argue that despite recent critiques, many of which propose replacing the diagnosis with trauma-related formulations, BPD remains a valid, coherent, and clinically indispensable construct. Misdiagnosis contributes to iatrogenic harm, while accurate diagnosis enables targeted
