Measurement of renal anatomy for prediction of lower-pole caliceal stone clearance: reproducibility of different parameters.

BACKGROUND AND PURPOSE: The architecture of the lower renal pole seems to have a considerable influence on caliceal stone clearance after different therapeutic modalities. The published data are partially inconsistent, and publications on reproducibility are completely lacking. The aim of this study was to evaluate the intraobserver and interobserver reproducibility of different measures of lower-calix anatomy. MATERIALS AND METHODS: We studied the intraobserver and interobserver reproducibility of parameters describing the lower-pole anatomy that are significant for treatment outcome. Forty

Prenatally diagnosed urinary tract abnormalities: long-term outcome.

The long-term outcomes of prenatally detected uropathies are poorly documented. Limited data on fetal intervention show a possible reduction in early mortality from pulmonary hypoplasia, but no beneficial effect on long-term prognosis for renal function. Prenatally detected vesicoureteric reflux (VUR) is characterised by males with high-grade primary reflux, who are at long-term risk of renal impairment. Prenatal diagnosis and surgical intervention have contributed to a reduction in long-term morbidity in children with pelviureteric junction (PUJ) obstruction. By the same token, many children

Renal cryoablation and radio frequency ablation: an evaluation of worst case scenarios in a porcine model.

PURPOSE: Although ablative technologies, including radio frequency (RF) ablation (RFA) and cryoablation (CA), are being used to treat renal masses, complications associated with injury to vital renal structures are not well understood. We investigated these worst case scenarios by deliberately targeting vital renal structures with CA or RFA in a porcine model. MATERIALS AND METHODS: Following surgical exposure of the right kidney in female pigs a cryoneedle or an RF probe was deliberately placed under visual and ultrasound guidance in the renal pelvis (CA in 5 pigs and RFA in 7), major calix

Flexible ureteroscopically assisted percutaneous renal access.

OBJECTIVE: To combine retrograde flexible ureteroscopic techniques with a simultaneous percutaneous puncture to gain precise antegrade renal access in selected patients. METHODS: Patients with minimally dilated collecting systems and complex stone burdens (including caliceal diverticular calculi) underwent adjunctive flexible ureteroscopy in an attempt to expedite percutaneous renal access. This combined retrograde and antegrade approach was also used in treating obese patients and those in whom prior attempts at percutaneous renal access had failed. The prone split-leg position and flexible

[Gigantic pyohydronephrosis and contralateral ureteral bud. Problems of differential diagnosis].

Both giant hydronephrosis and blind-ending ureteral bud are rare conditions. We report on a patient with an impacted ureteral stone leading to giant pyohydronephrosis (6.51) on the right side, which in turn caused displacement of the ureter containing the stone to the left. An asymptomatic blind-ending ureteral bud initially complicated the diagnosis. After nephrectomy, a unsuspected transitional cell carcinoma of the right renal pelvis and the ureter was found.

[Clinical experiences with extracorporeal shockwave lithotripsy].

Clinical experience with 2738 patients treated by extracorporeal shock wave lithotripsy between March 1985 and December 1988 is reported. All treatments were performed with the Dornier HM-3 lithotriptor. 34% of the patients needed auxiliary measures, consisting primarily of urological manipulation to improve urinary drainage or for better localization and/or focussing of the stones. Severe complications were rare; urosepticemia occurred in 0.3%, 2 patients had to undergo nephrectomy because of abscessing pyelonephritis, and there was one death due to recurrent pulmonary embolism in a patient w

Spontaneous rupture of the renal calyx secondary to a vesicoureteral junction calculus.

BACKGROUND: Spontaneous urinary collecting system rupture is caused by increased ureteral intraluminal pressure secondary to an obstruction. Rarely, a small stone exerts high intraureteral pressure especially if it is located distally. Many management modalities with good outcomes have been implicated. PURPOSE: Herein, we present a case of nontraumatic rupture of the renal calyx due to a 4 mm obstructing stone at the vesicoureteral junction. BASIC PROCEDURES: CT scan of abdomen and pelvis without contrast, CT scan of the abdomen and pelvis with intravenous contrast, Cystoscopy, Double-J uret

Supracostal access tubeless percutaneous nephrolithotomy: minimizing complications.

INTRODUCTION AND OBJECTIVE: Supracostal access in percutaneous nephrolithotomy (PCNL) may be avoided due to concern for thoracic complications. The objective of the study is to report the safety and efficacy of supracostal access utilizing a tubeless (stent only) PCNL technique. PATIENTS AND METHODS: Retrospective review of perioperative outcomes of 70 patients (76 renal units) who underwent a supracostal tubeless PCNL. No nephrostomy tubes were left. All patients had a 7F ureteral stent and Foley catheter placed. The nephrostomy sheath was removed with the patient held in end-expiration, and

Case report: percutaneous balloon dilatation and ureteral stenting for tuberculous renal infundibular and ureteral strictures.

A 23-year-old woman developed progressive obstructive uropathy due to multiple renal infundibular and ureteral strictures while on drug treatment for renal tuberculosis. Communication between isolated upper and mid-pole calyces, and the ureter, was established by percutaneous guidewire manipulation. The strictures were successfully managed using percutaneous balloon catheter dilatation and ureteral stenting without the need for open surgical exploration. Kidney function was preserved at one year follow-up.

Urologic manifestations of sickle hemoglobinopathies.

We analyzed the renal radiographic changes in the sickle hemoglobinopathies in 92 patients, in 70 of whom the specific hemoglobin type was documented by electrophoresis. The following conclusions have been drawn: (1) Approximately 50% of patients with SS hemoglobin have renal enlargement (both on roentgenograms and at autopsy), while only a small proportion of patients with the other hemoglobinopathies show renal enlargement; (2) slightly more than one half of patients with each type of hemoglobinopathy have calyceal changes; and (3) these calyceal changes do not fall into a specific pattern w

Predictors of Stone-Related Events in Asymptomatic Untreated Intrarenal Calculi.

Purpose: There is a lack of data on the natural history of asymptomatic intrarenal calculi. In this study, we investigate stone-related events (SREs) in patients with untreated intrarenal calculi. We also investigate predictive factors for SREs. Methods: All patients found with an asymptomatic intrarenal calculus on CT kidney, ureter, bladder managed conservatively with interval imaging for ≥6 months were included. Patients were evaluated for any SRE. The rate of event according to calculus size, location, and number of calculi was also analyzed. Multivariate logistic regression analysis was p

Percutaneous nephrolithotomy for isolated calyceal stones: How important is the stone location?

OBJECTIVE: To evaluate the effect of percutaneous access site on the success and complication rates of isolated calyceal stones. MATERIAL AND METHODS: We retrospectively evaluated 2700 patients who underwent percutaneous nephrolithotomy (PNL) in our clinic between October 2002 and August 2014. We selected only the patients with isolated lower, middle or upper calyceal stones and we grouped the patients according to the location of their stones. Successful operation was defined as complete stone clearence or retention of stone fragments smaller than 4 mm which do not lead to infection, obstruc

Retrograde vs. antegrade fl exible nephroscopy for detection of residual fragments following PNL: A prospective study with computerized tomography control.

INTRODUCTION: The main aim of stone surgery is to establish stone free status. Performing fl exible nephroscopy is an effective tool in this manner. The aim of this study was to evaluate the role of retrograde fl exible nephroscopy for detection of residual fragments following percutaneous nephrolithotomy (PNL) in comparison with antegrade approach. MATERIALS AND METHODS: Data of 137 patients underwent ECIRS was collected prospectively. In all cases following stone clearance, collecting system was checked for residual fragments. First antegrade than retrograde fl exible nephroscopy was perfor

Ureterocalycostomy - final resort in the management of secondary pelvi-ureteric junction obstruction: our experience.

Ureterocalycostomy can be performed in patients in whom desired methods of treating secondary PUJ (Pelvi-Ureteric Junction) obstructions either failed or could not be used. In our study, one child and two adults in whom one redo-ureterocalycostomy and two ureterocalycostomies were performed for severely scarred PUJ. The causes for secondary PUJ obstruction were post-pyelolithotomy in one case, post-pyeloplasty and ureterocalycostomy for PUJ obstruction in the second patient and the third patient had long upper ureteric stricture post-ureteropyeloplasty due to tuberculosis. In all these cases u

Inversion, hydration and diuresis during extracorporeal shock wave lithotripsy: does it improve the stone-free rate for lower pole stone clearance?

OBJECTIVE: It was the aim of this study to assess the efficacy and safety of combined forced hydration and diuresis with limited inversion during shock wave lithotripsy (SWL) by comparing this treatment modality with conventional SWL for lower calyceal nephrolithiasis. PATIENTS AND METHODS: In this prospective, non-randomized study, we included 100 patients with lower calyceal calculi <or=2 cm. Fifty of them received conventional SWL and the other 50 underwent SWL combined with oral hydration, diuresis and 12 degrees inversion position during SWL. Intravenous urography was performed for all p

Multidetector computed tomography urography (MDCTU) for diagnosing urothelial malignancy.

Multidetector computed tomography (MDCT) is well established for the detection of stones and renal masses, but more recently MDCT urography (MDCTU) is becoming widely used for examination of the entire urinary tract aimed specifically for diagnosing urothelial lesions. Evidence is rapidly accumulating to support the use of MDCTU in this manner. Familiarity with the MDCTU signs of urothelial malignancy is a prerequisite for optimum radiological practice. This article provides a review of the appearances of transitional cell cancer in the upper urinary tract and bladder.

[Anterograde transpelvic endopyelotomy].

The Authors describe their own technique of antegrade endopyelotomy for cases of primary ureteropelvic junction obstruction. After percutaneous access has been gained via the lower calix, the technique involves making a wide opening of the renal pelvis and exploring the peripelvic space in front of a 3 to 4 cm long section of the ureter. The aim is to carry out the operation with an unrestricted view and without the need for a large calibre stent, in order to perform endopyelotomy in pediatric patients or in the presence of anomalous vessels. The follow-up demonstrates good results in 80% of 4

[Prestenotic narrowing in the upper urinary tract (author's tract)].

While prestenotic dilatation in the upper urinary tract is considered as a characteristic radiological feature, earlier narrowing due to compensating hypertrophy of the musculature is either ignored or misinterpreted. Several examples of prestenotic narrowing are demonstrated and their significance in radiological diagnosis is stressed. This raises the question whether the narrowing of the ureters, seen in cases of bladder neck obstruction, particularly with prostatic hypertrophy, can be explained by a similar physiopathological response. This question can only be answered by performing pressu

Total replacement of the ureter by a segment of the sigmoid colon in the treatment of giant hydronephrosis: calyco-sigmoido-cystostomy.

Two cases of giant hydronephrosis of a solitary kidney are reported. In both of them correctly performed pyelo-ureteroplasties had not assured satisfactory emptying of the pyelocalyceal system. Its dilatation and infection had not disappeared. In order to liquidate the nephrostomy, the ureters were replaced by a sigmoidal loop (calyco-sigmoido-cystostomy) with favorable results assessed 24 and 19 months after operation. The use of an isolated sigmoid segment for replacement of the ureter is discussed.
