Isolated primary cold abscess of the sternum: a case report.

BACKGROUND: Musculoskeletal tuberculosis forms 10-25% of extrapulmonary tuberculosis which mainly involves the spine or weight-bearing joints. Tuberculous involvement of the sternum is a rare clinical entity even in countries where tuberculosis has high prevalence. Primary tuberculous sternal osteomyelitis accounts for approximately 0.3% of all types of tubercular osteomyelitis and the probable source appears to be extension from paratracheal or hilar lymph nodes. Despite tuberculosis being a common disease in endemic countries and worldwide, a thorough literature search of the PubMed database

Sonographic imaging of fetal tympanic rings.

OBJECTIVE: To examine the feasibility of ultrasonographic imaging of fetal tympanic rings. METHODS: This was an observational cohort study of 80 healthy fetuses in low-risk pregnancies, divided into four gestational-age subgroups (12, 16, 23 and 32 weeks), each comprising 20 consecutive fetuses. Tympanic ring visualization was achieved by two-dimensional and three-dimensional (3D) sonography. A standard algorithm for tympanic ring examination was constructed using 3D multiplanar reconstruction. The volume acquisition plane was directed to the inferolateral aspect of the fetal temporal bone. T

Stent biomechanics in hemodialysis-related left brachiocephalic vein stenosis: from site-specific loading to a staged device-validation framework.

Hemodialysis-related left brachiocephalic vein stenosis is usually managed as one local variant of central venous stenosis, yet its mechanical environment is distinctive. The vein follows a long, multi-axially curved retrosternal course through a narrow window between the manubrium and the aortic arch, bounded by two separate junctions, the lateral venous angle and the medial brachiocephalic-caval confluence. An implanted stent is therefore exposed to focal unidirectional compression, cyclic bending and torsion, short landing zones, and branch-coverage risk, conditions not reproduced by circum

Feasibility and findings of different radiographic methods in diagnosing canine heartworm disease.

BACKGROUND: Heartworm disease (HWD) represents the pathological consequences of heartworm (HW) (Dirofilaria immitis) infection on different organs and organ systems, primarily the lungs and heart. Radiography can objectively assess the severity of cardiopulmonary disease secondary to HW infection. However, interpreting dogs' radiographs can be quite challenging with respect to choosing an appropriate radiographic method. AIM: The aim of this article is to review the published literature on radiographic methods, both subjective and objective, emphasizing their advantages and disadvantages. In

A radiomics-driven approach for predicting response to induction therapy in multiple myeloma: Leveraging CT-based delta features.

OBJECTIVE: This work established and validated a radiomics-driven framework to quantify therapeutic response in multiple myeloma (MM). By leveraging CT-based Delta features, we aimed to delineate sub-visual temporal changes and assess their incremental predictive value beyond conventional clinical biomarkers. METHODS: We retrospectively evaluated 131 newly diagnosed MM patients (April 2019-November 2024), partitioned into training and validation cohorts (7:3 ratio). Baseline and post-induction CT images (mean interval: 132 days), reconstructed via high-resolution sharp kernels, served as the

Taxonomic and molecular characterization of a new entomopathogenic nematode species, Heterorhabditis casmirica n. sp., and whole genome sequencing of its associated bacterial symbiont.

BACKGROUND: Nematodes of the genus Heterorhabditis are important biocontrol agents as they form a lethal combination with their symbiotic Photorhabdus bacteria against agricultural insect pests. This study describes a new species of Heterorhabditis. METHODS: Six Heterorhabditis nematode populations were recovered from agricultural soils in Jammu and Kashmir, India. An initial examination using mitochondrial and nuclear genes showed that they belong to a new species. To describe this new species, a variety of analyses were conducted, including reconstructing phylogenetic relationships based on

[Vacuum assisted closure for the treatment of sternal wound infections -- rapid infection control and bridging to reconstructive surgery].

Vacuum Assisted Closure (V.A.C.) is a sound strategy to control severe post-sternotomy infection. Secondary re-wiring of the sternal bone or reconstructive surgery may be required later to achieve complete wound closure. Ten patients with severe sternal bone infection underwent initial V.A.C. therapy and delayed surgery for wound closure. Complete wound healing was achieved in all cases. As a case report we describe a patient with right-sided mastectomy and irradiation for breast cancer, who developed severe post-sternotomy infection and sternal bone necrosis following CABG. A combination of V

"Hidden" bone metastasis from thyroid carcinoma: a clinical note.

The (131)I-iodide ((131)I) whole-body scan, for thyroid carcinoma is at times difficult to interpret. In a diagnostic whole body (131)I scan of a patient with follicular carcinoma, a posterior skull lesion was partially hidden by overlapping facial structures. On lateral head view, the abnormality was clearly evident. SPECT/CT and MRI showed the lesion originated in the occipital bone and had enlarged into the posterior fossa. The mass was surgically removed and the patient received (131)I therapy for residual tissue. The study demonstrates a pitfall in the reading of two dimensional radioiodi

The surgical airway.

Traumatic airway injuries are rare in children, partly due to their unique anatomy. The larynx is well protected from direct blows behind the mandibular arch, and only a small portion of the trachea is unprotected above the manubrium due to the relatively short neck. Furthermore, the tracheobronchial tree is less prone to injuries as compared with adults due to its elasticity. A high index of suspicion is thus needed to adequately diagnose and manage pediatric airway injuries. Laryngotracheal injuries in particular may present with discreet initial symptoms that if undiagnosed may rapidly prog

Effect of lung volume reduction surgery on diaphragm length in severe chronic obstructive pulmonary disease.

Lung volume reduction surgery (LVRS) has been suggested as improving respiratory mechanics in patients with severe chronic obstructive pulmonary disease (COPD). We hypothesized that LVRS might lengthen the diaphragm, increase its area of apposition with the chest wall, and thereby improve its mechanical function. To determine the effect of bilateral LVRS on diaphragm length, we measured diaphragm length at TLC, using plain chest roentgenograms (CXRs), in 25 patients (11 males and 14 females) before LVRS and 3 to 6 mo after LVRS. A subgroup of seven patients (reference data) also had diaphragm

Investigation into the possible cause of subjective decreased sensory perception in the nipple-areola complex of women with macromastia.

Patients with macromastia often comment on a lack of sensation in their nipple-areola complex. A study was designed to investigate the cause of this decreased sensation. Two hypotheses were proposed. First, the decreased sensation could result from neuropraxia of the sensory nerve fibers secondary to traction caused by the heavy breast parenchyma. The second hypothesis proposed that tissue expansion of the nipple and areola by the voluminous breast parenchyma caused a decrease in nerve fibers per surface area and hence decreased sensory perception. Sixty-one patients were assessed in the study

[Treatment Strategies for Septic Arthritis of the Sternoclavicular Joint].

Septic arthritis of the sternoclavicular joint (SCJ) is a relatively rare disease. Due to serious complications including mediastinitis and generalised sepsis early diagnosis and rapid onset of treatment are mandatory. The disease often affects immunocompromised patients, diabetics, or patients with other infectious diseases. The therapeutic options range from administration of antibiotics to extended surgery including reconstructive procedures. Apart from rare situations where conservative treatment with antibiotics is sufficient, joint resection followed by plastic surgical procedures are re

Traumatic chylothorax following blunt thoracic trauma.

BACKGROUND: Chylothorax occurs when chyle from the thoracic duct leaks into the pleural space. While majority of cases are iatrogenic, traumatic chylothorax can occur when rib or vertebral fractures disrupt the thoracic duct. These occurrences are exceedingly rare, particularly following blunt traumatic insult. METHODS: We performed a retrospective review of a case of chylothorax following blunt trauma. Data was extracted from the electronic medical record. CASE: A 60-year-old female presented to the trauma bay after a motor vehicle crash as a restrained driver with bilateral chest pain. Of

[The internal thoracic blood vessels (internal thoracic arteries and veins) and their practical significance].

Internal thoracic blood vessels (A. et Vv. thoracicae internae) are parietal vessels of the thoracic anterior wall. Because of their position, they are often exposed to injuries during the fracture of the ribs and the sternal bone. These facts require a general knowledge about the anatomical variations of these vessels, specifically the knowledge concerning their mutual relationship, their anastomoses and their distance from the lateral margins of the sternal bone. Due to the poor and different data in the available literature, we directed our investigations towards the study and confirmation

First Report of Trichodorus variopapillatus (Nematoda: Trichodoridae) from the Czech Republic.

Members of the Trichodoridae can cause substantial crop losses directly by feeding on plant roots and indirectly as vectors of tobraviruses; both vector and virus are polyphagous. In April of 2008, soil samples from the rhizosphere of Ulmus minor Mill in a deciduous broadleaf forest at Krivoklat yielded a population of Trichodorus variopapillatus Hooper, 1972. Nematodes were identified by morphological and morphometric characters as well as by molecular analysis. For classical identification, specimens were extracted from soil by a decanting-sieving method, heat killed and fixed in triethanola

Sternum Metastases: From Case-Identifying Strategy to Multidisciplinary Management.

We aimed to overview the most recent data on sternal metastases from a multidisciplinary approach (diagnosis strategies, outcome, and histological reports). This narrative review based on a PubMed search (between January 2020 and 22 July 2023) using key words such as "sternal", "manubrium", and "metastasis" within the title and/or abstract only included original papers that specifically addressed secondary sternal spreading of cancer in adults, for a total of 48 original articles (14 studies and 34 single case reports).
