Dissection of the Trigeminal Ganglion of Nonrodent Species Used in Toxicology Studies.

The ganglion of the trigeminal (V cranial) nerve is generally sampled at necropsy in nonrodent toxicology studies only when somatic or autonomic peripheral nervous system toxicity is suspected. The ganglion is far more difficult to locate in nonrodents than in rats and mice, and suitable methods to dissect it have been described only for swine. The trigeminal nerve caudal to the ganglion passes through a canal, roofed by bone in dogs and rabbits and by a tough layer of dura mater in swine and nonhuman primates. The ganglion is partly or wholly obscured by overlying dura mater. Of the 3 intracr

Osteoplastic maxillotomy approach for infraorbital nerve schwannoma, a case report.

BACKGROUND: Extracranial schwannomas can readily occur in the head and neck region and rarely involve the trigeminal nerve. As a rule, their treatment is surgical and dictated by the location of the tumor and nerve of origin. METHODS: We describe a case of a 14-year-old boy with a mass invading right nasal fossa, maxillary sinus, orbital floor, pterygopalatine fossa, and infratemporal fossa. The diagnosis of a nerve sheath tumor was evoked after angiography showed no vascular blush. RESULTS: The tumor was removed through a Weber-Fergusson incision with subciliary extension followed by maxill

Expansion of donor-unrestricted MAIT cells with enhanced cytolytic function suitable for TCR redirection.

Objectives Intraoperative vagus nerve monitoring during skull base surgery has been reported with the use of an oral nerve monitoring endotracheal tube. However, the intraoral presence of an endotracheal tube can limit exposure by its location in the operative field during transfacial approaches and by limiting superior mobilization of the mandible during transcervical approaches. We describe a transnasal vagus nerve monitoring technique. Design and Participants Ten patients underwent open skull base surgery. Surgical approaches included transcervical (five), transfacial/maxillary swing (three

Diagnostic relevance of trigeminal evoked potentials following infraorbital nerve stimulation.

A new tool in neurophysiological exploration of the trigeminal nerve has recently been introduced. It has been demonstrated that stimulation of the infraorbital nerve trunk gives rise to very reliable scalp responses reflecting the activity of the afferent pathway between the maximally nerve and the brain stem. The authors demonstrate that alterations of such trigeminal evoked responses fit with documented pathological processes at various locations along the trigeminal pathway (maxillary sinus, parasellar region, and within the brain-stem parenchyma). They report the findings in 68 patients s

A case of zoster sine herpete of the trigeminal nerve.

Varicella-zoster virus reactivation causes zoster (shingles), a syndrome characterized by severe pain and a vesicular rash. The present report details a case of varicella-zoster virus reactivation of the maxillary and mandibular division of the right trigeminal nerve without evidence of vesicular rash (zoster sine herpete). It is difficult to identify owing to no typical clinical signs such as vesicular eruption. Zoster sine herpete of the trigeminal nerve, in particular, is rarely reported. In this case, the diagnosis was based on clinical findings and was supported by the demonstration of an

Ocular complications after inferior alveolar nerve block: a case report.

Ocular complications, transient loss of vision and diplopia, and blanching of the skin of the infraorbital region were reported in a female patient after an inferior alveolar nerve block for extraction of the permanent mandibular left third molar tooth. Injection of the anesthetic solution into the maxillary artery could result to such complications. The anatomy related to this case, with suggestions for management of such a patient is discussed.

Anesthesia and pain management.

Acute orofacial pain is usually managed by the administration of local anesthetics, systemic analgesics, or a combination of the two methods. In an emergency, intraoral maxillary nerve blockade is helpful for controlling pain in the midface, although infiltrations may be more suitable for discomfort originating from individual teeth or portions of the alveolar process. Mandibular anesthesia can be achieved by open or closed-mouth techniques for inferioral alveolar-lingual nerve blockade. Systemic pain relief is optimized by using full analgesic doses of NSAIDs, with opioids serving to increase

Sensory pulpal nerve fibres and trigeminal ganglion neurons express IL-1RI: a potential mechanism for development of inflammatory hyperalgesia.

AIM: To localize interleukin-1 receptor type I (IL-1RI) in rat dental pulp and trigeminal ganglion (TG) and to test the hypothesis that pulpal inflammation increases neuronal expression of IL-1RI. METHODOLOGY: Female Wistar rats were subjected to unilateral pulp exposures in the maxillary and mandibular first molars, whereas the contralateral jaws served as untreated controls. Seven days later the animals were transcardiacally perfused and the jaws and the TGs were removed and prepared for immunohistochemistry. Immunoreactivity for IL-1RI was examined alone (DAB) and together with calcitonin

Comparison of the feasibility and safety of infrazygomatic and suprazygomatic approaches to pterygopalatine fossa using virtual reality.

BACKGROUND: Injections of local anesthetics into pterygopalatine fossa gained popularity for treating acute and chronic facial pain and headaches. Injury of maxillary artery during pterygopalatine fossa injection can result in pseudoaneurysm formation or acute bleeding. We aimed to identify the optimal approach into pterygopalatine fossa by comparing feasibility and safety of suprazygomatic and two infrazygomatic approaches. METHODS: We analyzed 100 diagnostic CT angiographies of cerebral arteries using 3D virtual reality. Each approach was determined as a target point in pterygomaxillary fis

Adenoid Cystic Carcinoma of the Maxillary Sinus with Isolated Trigeminal Anesthesia.

Adenoid cystic carcinoma (ACC) is a rare malignant secretory gland tumor. It is characterized by slow growth, long clinical course, local recurrences, and distant metastases. In the sinonasal tract, it most commonly arises in the maxillary sinus. It often presents at an advanced stage with perineural spread (PNS). Our patient presented with left-sided facial numbness without other symptoms. The numbness was localized to the left cheek, left side of nose, and left upper lip. Magnetic resonance imaging (MRI) of the brain revealed an enhancing lesion involving the left maxillary sinus with orbita

Modified Lateral Surgical Approach to the Pterygopalatine Fossa for Safe Preservation of the Peripheral Branch of the Marginal Mandibular Nerve.

In cases where oral cancer spreads toward the maxillary tubercle, surgery may extend to the pterygopalatine fossa. There are 2 main extraoral approaches: anterior and lateral. Previously, we introduced a modified lateral approach with a mouth corner incision from the lower lip, that preserves the mental and marginal mandibular nerves. Now, we present a further modification that allows safer preservation of the peripheral branches of the marginal mandibular nerve around the mouth. The procedure has 2 key points. The first is to follow the course of the marginal mandibular nerve from the submand

[Orbital trauma inflicted by a butcher's knife without any injury of the eyeball].

We present a 25 year old man, who suffered from an apparently serious orbital injury inflicted by a butcher's knife during a fight. The knife perforated the upper lid of the right eye, and than penetrated the orbita along its medial wall into the right maxillary sinus, the ethmoid sinus, finally reaching the left maxillary sinus. The eye bulb, as well as the optic nerve and the muscles remained intact. A major hemorrhage from the nasal cavity, which occurred immediately after the extraction of the knife, was managed successfully by means of anterior and posterior nasal packing. Intranasal syne

The posterior inferior orbitotomy.

Four patients with orbital apex tumors between the optic nerve and inferior rectus underwent a posterior inferior orbitotomy through the maxillary sinus. Three tumors were removed successfully and the fourth was not located, but the visual function improved after surgery, presumably owing to decompression of the posterior orbital floor. The technique was carried out through a standard Caldwell-Luc approach through the maxillary sinus. The posterior inferior orbital wall was removed and the inferior rectus was retracted either laterally or medially to gain access to the tumor, which was removed

Significance of CT and MR findings in sphenoid sinus disease.

Disorders of the paranasal sinuses, particularly the sphenoid sinus, can be associated with significant disorders of the optic and other cranial nerves. We examined 100 consecutive routine CT scans, 100 posterior fossa CT scans, and 100 MR scans to look for evidence of sinus disease, especially of the sphenoid sinus. The sphenoid sinus was abnormal in 7% of scans by all methods. Other sinuses were more frequently abnormal, including maxillary (23%), ethmoid (34%), and frontal (16%). Although MR was more sensitive in detecting sinus inflammation in the ethmoid and maxillary sinuses, the frequen

Neuritic toothache.

Diagnosing and managing neuropathic pain with associated sensory aberrations can be aggravating and frustrating for both the dentist and the patient. The teeth and intraoral and extraoral tissues can be affected if neuritis is located in the maxillary or mandibular nerve branches. Dentists should be aware of a neuritic etiology for tooth pain because an incorrect diagnosis can lead to ineffective and unnecessary dental work.

A morphological study of the multi-posterior superior alveolar canals of maxilla in the Japanese macaque by cone-beam computed tomography.

The posterior superior alveolar canal (PSAC) composed of several canals which contains vessels and nerve in molar region of the maxilla of Japanese macaque. The PSAC of maxilla run to the maxillary sinus. However, the PSAC and accessory canal (AC) of the maxilla in the Japanese macaque (JM) is unknown in morphological features in the maxilla. The purpose of this study was to describe the PSAC of the primates and to determine whether this structure could be used as a model for the human clinical condition. In this study, we showed the course of PSAC structure of the 23 JMs (male: n = 15; female

Dose distribution of intensity-modulated proton therapy with and without a multi-leaf collimator for the treatment of maxillary sinus cancer: a comparative effectiveness study.

BACKGROUND: Severe complications, such as eye damage and dysfunciton of salivary glands, have been reported after radiotherapy among patients with head and neck cancer. Complications such as visual impairment have also been reported after proton therapy with pencil beam scanning (PBS). In the case of PBS, collimation can sharpen the penumbra towards surrounding normal tissue in the low energy region of the proton beam. In the current study, we examined how much the dose to the normal tissue was reduced by when intensity-modulated proton therapy (IMPT) was performed using a multi-leaf collimato
