Treatment of nonunions of the distal phalanx with olecranon bone graft.

Distal phalangeal fractures are the most common fractures of the hand but nonunions are unusual in the distal phalanx. Eleven patients were operated on for nonunions of the distal phalanx. The diagnosis of nonunion was made by the presence of the clinical (pain, deformity, instability) and radiological signs of nonunion more than 4 months after the initial injury. Three patients had developed infection and four of them had bone resorption after their initial treatments, which probably caused nonunion. Olecranon bone grafting combined with Kirschner wire fixation was done in all patients. The m

Long-term results of olecranon fractures treated using the XS nail® system.

PURPOSE: Olecranon fractures are particularly vulnerable to distraction and subsequent fracture dislocation due to the high tensile forces. Surgical treatment aims at reducing the fracture and restoring the anatomical joint surface condition, as well as neutralizing the strain inhibiting fracture healing. The XS nail® (Intercus GmbH, Bad Blankenberg, Germany), an intramedullary implant exerting compression across the entire fracture surface, unlike plates, leaves a minimal extra-cortical profile, and can be secured with threaded locking wires, thereby retaining the anatomical reduction without

Application of a novel shape-memory alloy concentrator in displaced olecranon fractures: a report of the technique and mid-term clinical results.

PURPOSE: Olecranon fracture is a common upper limb fracture, and several surgical approaches have been advocated for its fixation. To overcome the complications associated with common techniques, we present a novel shape-memory alloy concentrator, an alternative for tension band compression, to fix olecranon fracture. METHODS: Fifty-seven patients (26 men and 31 women) with olecranon fracture, with a mean age of 45 years, were included in this study. Each patient had undergone open reduction and internal fixation using the Nitinol (Ni-Ti) arched shape-memory connector (ASC). The clinical asse

Biomechanical comparison of tension band wiring and plate fixation with locking screws in transverse olecranon fractures.

BACKGROUND: Tension band wiring (TBW) is the standard method for treating transverse olecranon fractures, but high rates of complications and reoperations have been reported. Plate fixation (PF) with locking screws has been introduced as an alternative method that may retain the fracture reduction better with a higher load to failure. METHODS: Twenty paired cadaveric elbows were used. All soft tissues except for the triceps tendon were removed. A standardized transverse fracture was created, and each pair was allocated randomly to TBW or PF with locking screws. The triceps tendon was mounted

Functional outcomes of distal triceps tendon repair comparing transosseous bone tunnels with suture anchor constructs.

BACKGROUND: Distal triceps tendon ruptures are relatively rare. Few studies have investigated functional outcomes after repair. There is no consensus on fixation methods for this injury. The purpose of this study was to compare the functional outcomes and the reoperation rates after distal triceps tendon repairs using transosseous tunnels and suture anchors. METHODS: A multicenter, retrospective review of all primary triceps repairs done between 2006 and 2015 was performed. Patients were included if they had a minimum of 2 years of follow-up. Intraoperative data recorded included repair metho

[Fractures of the olecranon].

OBJECTIVE: Fractures of the olecranon are the most common fractures of the elbow in adults. Due to the dislocating force of the triceps muscle, internal fixation is the treatment of choice. INDICATIONS: All fractures of the olecranon without contraindications. CONTRAINDICATIONS: Infection and severe soft tissue damage. SURGICAL TECHNIQUE: Dorsal approach to the olecranon with the patient in a prone position. Open reduction and internal fixation with tension band wiring or plate fixation according to fracture pattern. POSTOPERATIVE MANAGEMENT: Treatment goal is early functional mobilization

Fracture line distribution of olecranon fractures.

PURPOSE: The association between specific olecranon fracture characteristics (e.g., displacement, fragmentation, subluxation) and fracture line distribution might help surgeons predict intra-articular fracture location based on fracture characteristics that can be determined on radiographs. We hypothesized that fracture mapping techniques would reveal different fracture patterns for minimally displaced fractures, displaced fractures, and fracture-dislocations of the olecranon. METHODS: A consecutive series of 78 patients with olecranon fractures were evaluated using initial radiographs and co

Factors influencing K-wire migration in tension-band wiring of olecranon fractures.

BACKGROUND: Tension-band wiring is a popular method of internal fixation for simple olecranon fractures. Although fracture union rates and clinical outcomes are good, up to 80% of patients require removal because of prominent/symptomatic metalwork. The current literature remains unclear as to the best orientation of the longitudinal wires to minimize hardware failure. The aim of this study was to determine the surgically modifiable factors related to spontaneous wire pullout. METHODS: A retrospective review of hospital theater records over a period of 6 years was performed to identify all ole

Fixing simple olecranon fractures with the Olecranon Osteotomy Nail (OleON).

OBJECTIVE: Stable fixation of simple olecranon fractures or olecranon osteotomies in order to allow early functional treatment. INDICATIONS: Simple (non-comminuted) olecranon fractures and (Chevron) osteotomies of the olecranon. CONTRAINDICATIONS: Comminuted fractures and fractures more than 40 mm distal than the tip of the olecranon are contraindications. SURGICAL TECHNIQUE: Using a slightly curved posterior approach, the fracture is anatomically reduced. The fracture is temporary stabilized using K-wires. A guiding K-wire is positioned centrally in the medullary canal in the lateral proje

The effect of triceps repair techniques following olecranon excision on elbow stability and extension strength: an in vitro biomechanical study.

OBJECTIVES: To determine the effect of two types of triceps repair techniques on elbow stability and extension strength in the setting of olecranon deficiency using a cadaveric model. METHODS: Eight fresh-frozen cadaveric arms were tested in an elbow motion simulator, which produced active elbow extension by applying physiological loads to the tendons. Computed tomography-based surface models were used to determine cutting planes corresponding to sequential levels of olecranon resection. Both anterior and posterior triceps repairs were simulated for each level. RESULTS: Progressive sectionin

[Case report and literature review: elbow fracture dislocation in children].

Elbow dislocation in children is a very infrequent traumatic event which was first described by Stimson in 1900 and then by Tachdjian in 1990. Its estimated incidence ranges from 3% to 6% of all elbow injuries, peaking at 13-14 years. Elbow trauma is classified considering the direction in which the proximal radioulnar joint shifts with respect to the humerus, into posterior and anterior dislocation. The former is the most frequent and accounts for 95% of cases. Elbow fracture dislocation is an even rarer event. The incidence rate of avulsion fracture of the medial epicondyle is 25-36%, of the

Use of mid upper arm circumference for evaluation of nutritional status of OSMF patients.

BACKGROUND: Oral submucous fibrosis (OSMF) is always associated with juxtaepithelial inflammatory reaction followed by fibroblastic changes in lamina propria, with epithelial atrophy leading to stiffness of oral mucosa and causing trismus and inability to eat. Mid upper arm circumference (MUAC) is a useful tool for a fast assessment of the nutritional status. AIMS: The study was undertaken to evaluate the correlation of MUAC as a nutritional status indicator in OSMF patients. PATIENTS AND METHODS: The study group comprised 50 clinically diagnosed and histopathologically confirmed cases of OS

Free fibula flap elbow-joint hemiarthroplasty reconstruction for chronic osteomyelitis of the distal humerus.

The vascularized fibula flap has become a major tool in upper limb reconstruction. Free fibula flap reconstructions of the humeral part of the shoulder and the radial part of the wrist joints are well-documented, but reports of elbow joint reconstruction are rare. The authors report a 53-year-old patient with chronic osteomyelitis of the distal humerus that was unsuccessfully treated by many local surgical debridements and long-term systemic antibiotics. The patient underwent a wide debridement of the distal two-thirds of the humerus, and a spacer was inserted to fill the bony humeral gap. At

Correction of elbow flexion contracture by means of olecranon resection and anterior arthrolysis in obstetrical brachial plexus palsy sequelae.

The authors have developed a particular surgical technique (olecranon bone resection together with anterior elbow arthrolysis) to increase the elbow's range of motion in adolescents and young adults suffering from elbow flexion contracture in obstetrical palsy sequelae. The surgical procedure was carried out in a preliminary group of 11 patients. The original procedure included a double incision: first of all by means of a posterior approach to resect the tip of the olecranon and then another incision carried out through the anteromedial aspect of the elbow, with a view to performing the anter
