Nuclear localization of type I parathyroid hormone/parathyroid hormone-related protein receptors in deer antler osteoclasts: evidence for parathyroid hormone-related protein and receptor activator of NF-kappaB-dependent effects on osteoclast formation in regenerating mammalian bone.

Parathyroid hormone-related protein (PTHrP) is not required for osteoclastogenesis during embryonic development; however, after birth it has been shown to regulate osteoclast formation during tooth eruption. Our study explores the hypothesis that PTHrP also may regulate osteoclast differentiation in the regenerating skeletal tissues of deer antlers, bones capable of complete regeneration. Osteoclast-like multinucleated cells (MNCs) formed spontaneously in micromass cultures derived from antler cartilage and these cells had the phenotypic characteristics of osteoclasts. PTHrP and receptor activ

Robotic resection of ectopic mediastinal parathyroid adenoma with intraoperative parathyroid hormone monitoring: a case report.

BACKGROUND: Primary hyperparathyroidism is a disease caused by the secretion of excess parathyroid hormone (PTH) owing to the enlargement of the parathyroid gland. Ectopic parathyroid glands exist in the mediastinum in approximately 1-2% of cases, which is relatively rare. Intraoperative monitoring of serum PTH level is important to assess whether the source of hyperparathyroidism has been eliminated. CASE PRESENTATION: A 53-year-old asymptomatic woman was diagnosed with ectopic mediastinal parathyroid adenoma. A three-port robotic partial resection of the thymus containing the tumor was atte

Effect of etelcalcetide on parathyroid hormone secretion by primary hyperparathyroidism patient-derived primary parathyroid cells.

INTRODUCTION: Etelcalcetide (Parsabiv®, AMG 416/ONO-5163) is a novel allosteric modulator for the calcium-sensing receptor approved for hemodialysis patients with secondary hyperparathyroidism of uremia. Etelcalcetide reduced parathyroid hormone levels in hemodialysis patients with secondary hyperparathyroidism of uremia in clinical studies. However, its direct effect on parathyroid hormone secretion in human parathyroid cells remains unknown. This study aimed to determine if etelcalcetide suppresses parathyroid hormone secretion by human parathyroid cells in vitro. MATERIALS AND METHODS: We

Metastatic Parathyroid Hormone-Secreting Prostate Adenocarcinoma Mimicking Ectopic Parathyroid Adenoma Demonstrated on 99mTc-MIBI Image.

Tc-MIBI has long been used to localize hyperfunctioning parathyroid tissue in patients with hyperparathyroidism. This tracer can also concentrate in various neoplastic tissues including prostate adenocarcinoma. We herein report a case with parathyroid hormone-secreting metastatic prostate cancer mimicking an ectopic parathyroid adenoma on the Tc-MIBI scan. We conclude that metastatic prostate cancer should be included as one of the differential diagnoses when interpreting Tc-MIBI scan.

Bone marrow uptake in parathyroid scintigraphy is a consequence of extremely high parathyroid hormone levels.

OBJECTIVE: We retrospectively evaluated patients with end-stage renal disease (ESRD) who were referred to our department for parathyroid scintigraphy. The aim of this study was to investigate the causes of bone marrow uptake observed on parathyroid scintigraphy. METHODS: We included 18 ESRD patients (10 F, 8 M; mean, 52 ± 13 years old; range, 45-59) in the study. The disease duration of the patients was mean 7.7 ± 4.7 years. The patients' mean plasma calcium and parathormone (PTH) levels were 9.7 ± 1.4 mg/dL and 1,553.3 ± 691.7 pg/mL, respectively. Dual-phase technetium-99m 2-methoxyisobutyl-

Parathyroid carcinoma: current understanding and new insights into gene expression and intraoperative parathyroid hormone kinetics.

Parathyroid carcinoma is an indolent but ultimately life-threatening malignancy. Due to the lack of definitive diagnostic markers and overlapping clinical features of benign primary hyperparathyroidism (PHPT), this disease is often misdiagnosed as parathyroid adenoma. Therefore, a high index of suspicion preoperatively and early intraoperative recognition with en bloc surgical resection are crucial for favorable outcome. Owing to the rarity of the disease, little is known about the molecular pathogenesis of parathyroid carcinoma. Here, we review the literature to present current understanding

Antagonizing the parathyroid calcium receptor stimulates parathyroid hormone secretion and bone formation in osteopenic rats.

Parathyroid hormone (PTH) is an effective bone anabolic agent, but it must be administered parenterally. An orally active anabolic agent would provide a valuable alternative for treating osteoporosis. NPS 2143 is a novel, selective antagonist (a "calcilytic") of the parathyroid cell Ca(2+) receptor. Daily oral administration of NPS 2143 to osteopenic ovariectomized (OVX) rats caused a sustained increase in plasma PTH levels, provoking a dramatic increase in bone turnover but no net change in bone mineral density. Concurrent oral administration of NPS 2143 and subcutaneous infusion of 17beta-es

Role of octreotide on release of intact 1-84 parathyroid hormone from human parathyroid cells.

BACKGROUND: Octreotide has been shown to lower urinary calcium in primary hyperparathyroidism although the mechanism remains unclear. This study examined the effect of octreotide on parathyroid hormone (PTH) secretion from human parathyroid cells in culture and as isolated cells. Additionally in situ hybridization was performed for somatostatin receptor messenger RNA (mRNA) and immunocytochemistry for somatostatin in eight parathyroid adenomas. METHODS: Tissue from three hyperplastic glands and three adenomas was studied as dispersed cell suspensions. Incubation was in buffers containing high

Thallium-technetium subtraction scintigraphy of enlarged parathyroid glands after calcitonin stimulation of parathyroid hormone secretion.

To improve the sensitivity of thallium-technetium subtraction scintigraphy for preoperative localization procedure of enlarged parathyroid glands in primary hyperparathyroidism, we administered calcitonin intramuscularly 4 hours before the scintigraphy in 14 consecutive patients. Injection of calcitonin reduced plasma levels of ionized calcium from 1.47 +/- 0.10 mmol/l to 1.41 +/- 0.09 mmol/l (p less than 0.01). Concomitantly, serum levels of intact parathyroid hormone increased from 6.4 +/- 2.5 pmol/l to 7.9 +/- 2.6 pmol/l (p less than 0.001). The scintigram after calcitonin injection visuali

Parathyroid hormone heterogeneity and assay-dependent interpretation in practice.

Parathyroid hormone (PTH) is a key biochemical marker in the diagnosis and management of disorders of calcium-phosphate metabolism. However, circulating PTH represents a heterogeneous mixture of full-length PTH(1-84) and multiple truncated fragments that differ in biological activity and clearance. These molecular forms are variably detected by commercially available immunoassays, resulting in assay-dependent differences in measured PTH concentrations. In endocrine surgery, intraoperative PTH monitoring (ioPTH) is widely used to assess the adequacy of parathyroidectomy based on the rapid decli

Parathyroid Hormone Contributes to Pulmonary Hypertension in Hypoxic Conditions.

BACKGROUND: Pulmonary hypertension (PH) is characterized by increased pulmonary artery pressure and can lead to right heart failure. Parathyroid hormone (PTH) is secreted by the parathyroid gland and plays a crucial role in calcium homeostasis. PTH also acts on the cardiovascular system and affects cardiovascular prognosis. We hypothesized that PTH would play a potential role in the pathogenesis of PH. METHODS: Serum PTH levels were measured in patients with PH or suspected PH who underwent evaluation using right heart catheterization. We assessed whether the regulation of PTH and the PTH1R (

Day zero parathyroid hormone levels predict cure after parathyroidectomy.

OBJECTIVE: To evaluate the predictive value of day zero post-operative parathyroid hormone (PTH) levels in determining cure for primary hyperparathyroidism (pHPT) following parathyroidectomy. METHODS: This multicentre, retrospective diagnostic accuracy study utilized data from a single surgeon. Patients who underwent parathyroidectomy for pHPT were included, with exclusions for secondary or tertiary causes and incomplete follow-up. Day zero post-operative PTH levels were the key predictor variable, while cure at 6 months, defined by normocalcaemia, was the outcome. Diagnostic parameters inclu

Management of Postthyroidectomy Hypocalcemia Using Intraoperative Parathyroid Hormone Monitoring.

IMPORTANCE: Postthyroidectomy hypoparathyroidism is typically diagnosed with low serum calcium levels, often requiring patients to remain in the hospital for appropriate treatment. Given the short half-life of parathyroid hormone (PTH), can hypoparathyroidism be diagnosed intraoperatively when re-exploration and autotransplant are still possible? OBJECTIVE: To determine whether intraoperative parathyroid hormone monitoring (ioPTH) permits the stratification of patients into appropriate tiers for postoperative supplementation. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study

An Unusual Etiology of Recurrent Parathyroid Hormone-Independent Hypercalcemia.

The diagnosis of parathyroid hormone (PTH)-independent hypercalcemia can be challenging due to its heterogeneous etiology. We report a case of a 65-year-old woman diagnosed with PTH-independent hypercalcemia 7 years prior. In view of elevated serum acetylcholine esterase and 1,25-dihydroxyvitamin D, a granulomatous etiology was suspected. The 18F-fluorodeoxyglucose (FDG) positron emission tomography-computed tomography (PET-CT) revealed diffuse uptake in the axial and appendicular muscles, suggestive of sarcoidosis of muscle. She responded to a 6-week course of glucocorticoids, with normalizat

Parathyroid hormone-PTH1R signaling in cardiovascular disease and homeostasis.

Primary hyperparathyroidism (pHPT) afflicts our aging population with an incidence approaching 50 per 100 000 patient-years at a female:male ratio of ~3:1. Decisions surrounding surgical management are currently driven by age, hypercalcemia severity, presence of osteoporosis, renal insufficiency, or hypercalciuria with or without nephrolithiasis. Cardiovascular (CV) disease (CVD) is not systematically considered. This is notable since the parathyroid hormone (PTH) 1 receptor (PTH1R) is biologically active in the vasculature, and adjusted CV mortality risk is increased almost threefold in indiv

Determination of parathyroid hormone: from radioimmunoassay to LCMS/MS.

Parathyroid hormone (PTH) determination is of paramount importance for the exploration of diseases related with calcium metabolism and for the follow-up of patients suffering from bone and mineral disorders associated with chronic kidney diseases (CKD-MBD). Unfortunately, the biologically active form of PTH, i.e. 1-84 PTH, circulates in the blood stream with many fragments and post-translationally modified forms, which decreases the specificity of immunoassays. The assays used to measure PTH, either from 2nd or 3rd generation, are not standardised, which may lead to interpretation errors and c

Parathyroid hormone-related protein in breast cancer bone metastasis.

Parathyroid hormone-related protein (PTHrP) was discovered as the tumor product causing the humoral hypercalcemia of malignancy. Its structural similarity to the hormone, PTH, with 8 of the first 13 amino acids identical, was sufficient to explain the sharing by PTHrP and PTH of a common receptor, PTH1R, although the remainder of the sequences are unique. PTHrP has important roles in development of several organs, including breast and bone, and functions as a paracrine factor postnatally in these and other tissues. In addition to its hormonal role in cancer, PTHrP is produced by two thirds of
