Navigating the Mitochondrial Maze: A Case Report of Emergent Cesarean Delivery After Labor Induction in a Patient With Leigh Syndrome.

Leigh syndrome is a rare mitochondrial disorder characterized by progressive psychomotor regression. Due to high childhood mortality and rare adult-onset cases, there are no guidelines for the obstetric management of this population. In these patients, physiological stress from labor may precipitate acute decompensation, manifesting with metabolic acidosis and multiorgan failure. We report the obstetric anesthetic management of a patient with adult-onset Leigh syndrome who required an emergent intrapartum cesarean delivery after labor induction. This case highlights the necessity of tailored a

From Fear to Function: How Physiotherapists Navigate Psychological Challenges in Patients Undergoing Rehabilitation After ACL Reconstruction.

BACKGROUND: Psychological readiness plays a critical role in return to sport (RTS) outcomes after anterior cruciate ligament reconstruction (ACLR), yet physiotherapists (PTs) often feel unprepared to support this aspect of recovery. Although clinical guidelines advocate for psychological strategies, practical tools and structured interventions remain scarce. HYPOTHESIS: Experienced PTs will actively use psychological strategies during rehabilitation to support patient readiness for RTS. PTs perceive themselves as playing a central role in addressing psychological barriers. STUDY DESIGN: Qual

Navigating acute myeloid leukemia towards better outcomes: Treatment pathways and challenges for patients ineligible for intensive chemotherapy.

Acute myeloid leukemia (AML) is an aggressive hematologic malignancy that affects primarily older individuals. Patients ineligible to receive intensive standard chemotherapy followed by consolidation with/without hematopoietic stem cell transplant have a suboptimal prognosis. In recent years, significant advances have been made in the AML field leading to the development of new anti-leukemic approaches, including lower-intensity therapies specifically developed for patients who are ineligible for intensive chemotherapy. As the available options for this hard-to-manage and historically undertre

Endobronchial Ultrasound Staging During Navigation Bronchoscopy for Peripheral Pulmonary Nodules in the Real World: Which Patients Will Benefit?

BACKGROUND/OBJECTIVES: The prevalence of lung cancer in patients with a peripheral pulmonary nodule referred for navigation bronchoscopy (NB) is high. Combining NB with a systematic EBUS for staging is common practice. We investigated the added value of performing EBUS in the population referred for NB in relation to the available pre-procedural [18F]FDG-PET and CT imaging information. METHODS: This single-center study evaluated all consecutive patients who underwent an NB in an academic referral center. [18F]FDG-PET and CT scoring of lymphadenopathy was based on routine [18F]FDG-PET and/or c

An App for Navigating Patient Transportation and Acute Stroke Care in Northwestern Ontario Using Machine Learning: Retrospective Study.

BACKGROUND: A coordinated care system helps provide timely access to treatment for suspected acute stroke. In Northwestern Ontario (NWO), Canada, communities are widespread with several hospitals offering various diagnostic equipment and services. Thus, resources are limited, and health care providers must often transfer patients with stroke to different hospital locations to ensure the most appropriate care access within recommended time frames. However, health care providers frequently situated temporarily (locum) in NWO or providing care remotely from other areas of Ontario may lack suffici

Utilizing robotic-assisted navigation for pelvic instrumentation in pediatric patients with neuromuscular scoliosis: a technical note and case series.

Pelvic fixation is commonly used in correcting pelvic obliquity in pediatric patients with neuromuscular scoliosis and in preserving stability in adult patients with lumbosacral spondylolisthesis or instances of traumatic or osteoporotic fracture. S2-alar-iliac screws are commonly used in this role and have been proposed to reduce implant prominence when compared to traditional pelvic fusion utilizing iliac screws. The aim of this technical note is to describe a technique for robotically navigated placement of S2-alar-iliac screws in pediatric patients with neuromuscular scoliosis, which (a) m

The Role of Navigated Transcranial Magnetic Stimulation Motor Mapping in Adjuvant Radiotherapy Planning in Patients With Supratentorial Brain Metastases.

Purpose: In radiotherapy (RT) of brain tumors, the primary motor cortex is not regularly considered in target volume delineation, although decline in motor function is possible due to radiation. Non-invasive identification of motor-eloquent brain areas is currently mostly restricted to functional magnetic resonance imaging (fMRI), which has shown to lack precision for this purpose. Navigated transcranial magnetic stimulation (nTMS) is a novel tool to identify motor-eloquent brain areas. This study aims to integrate nTMS motor maps in RT planning and evaluates the influence on dosage modulation

Navigating Electroconvulsive Therapy in Patients on GLP-1 Receptor Agonists: Balancing Safety, Compliance, and Treatment Consistency: A Case Report.

Electroconvulsive therapy (ECT) is typically performed under general anesthesia without a secured airway, placing patients with retained gastric contents at increased risk of aspiration. As more patients present for ECT while taking GLP-1 receptor agonists, standard fasting requirements pose unique safety and patient-experience challenges. A 24-hour clear-liquid diet every other day is burdensome, while frequent rapid sequence intubations may prolong treatment and recovery. To address this, ECT and anesthesia leadership developed a novel pre-ECT fasting protocol for patients on GLP-1 RAs. We r

Navigated percutaneous column screw fixation combined with total hip arthroplasty for non-reconstructable acetabular fractures in geriatric patients.

BACKGROUND: Periprosthetic acetabular fractures and acetabular fractures are becoming increasingly common in the elderly population. The ultimate treatment goal in these patients should include immediate unrestricted mobilisation. Additionally, the burden of the operative procedure should be minimized. We therefore developed a technique of single stage navigated percutaneous screw fixation of the anterior and posterior columns combined with total hip arthroplasty for the treatment of non-reconstructable acetabular fractures in geriatric patients. The aim of this study is to retrospectively eva
