Necrotizing fasciitis as a complication of odontogenic infection: a review of management and case series.

BACKGROUND: The aims of the present study were to establish the incidence of head and neck necrotizing fasciitis (NF) in the Adelaide Oral and Maxillofacial Surgery Unit; review the current literature regarding the management of head and neck NF; and determine the evidence for the role of hyperbaric oxygen therapy in the management of NF. METHODS: A retrospective audit of all patients admitted to the Royal Adelaide Hospital Oral and Maxillofacial Surgery Unit 2006-2015 with severe odontogenic infections was carried out. Patient demographics were recorded and treatment details were collected a

Clinical characteristics of lung abscess by red complex bacteria infection: a case report and literature review.

BACKGROUND: Treponema denticola, Porphyromonas gingivalis and Tannerella forsythia are common oral pathogens collectively referred to as the "red complex bacteria", serve as crucial periodontopathic agents. Owing to the challenges associated with anaerobic culture, their contribution to lower respiratory tract infections, especially lung abscess, is often undervalued. Metagenomic next-generation sequencing (mNGS) has evolved as a potent instrument for the identification of fastidious organisms. CASE PRESENTATION: A 63-year-old male with chronic cough and hemoptysis was admitted to our hospita

Descending necrotizing mediastinitis caused by retro-pharyngeal Eggerthia catenaformis infection.

INTRODUCTION: Eggerthia catenaformis, a non-spore-forming anaerobic Gram-positive bacillus component of the human fecal microbiota has rarely been reported in human diseases. In almost every case described in current literature to date, dental diseases (abscesses, periodontitis, or caries), are the most common source of the infection which extends to the brain, cervical spaces, pulmonary parenchyma, the pleural cavity, the abdominal wall, and the abdominal cavity. CASE REPORT: An 82-year-old male Caucasian patient was admitted to our Emergency Department (ED) with a painless, right submandibu

Case report of atypical Lemierre's Syndrome associated with Fusobacterium nucleatum infection without internal or external jugular venous thrombophlebitis.

Fusobacterium nucleatum is an anaerobe that is commensal to the human oral cavity. It is usually a component of periodontal plaque that is emerging as a pathogen and quickly attracting attention of the medical and research communities. It has been even discovered in bronchoalveolar lavage of some patients with lung cancer. Lemierre's syndrome (LS) is characterized as septic thrombophlebitis of the internal jugular vein, which usually begins with oropharyngeal infection that worsens and leads to inflammation of the wall of the jugular vein. This is the hallmark of the disease. However, in this

Supernumerary fourth molar and dental pathologies in a Chalcolithic individual from the El Mirador Cave site (Sierra de Atapuerca, Burgos, Spain).

We present a middle-aged Chalcolithic male with a supernumerary distomolar in the mandible. The prevalence of supernumerary teeth in present-day populations is low, ranging from 0.1% to 3.4%; most supernumerary teeth are documented in the anterior and molar regions of the maxilla in present populations. However, the prevalence of supernumerary molars in past populations is still unknown. Moreover, a complete pathological study has been done of this individual. Maxilla, mandible and teeth have been analyzed searching for dental pathologies. A cone-beam computed tomography (CBCT) analysis of man

Dentipain, a Streptococcus pyogenes IdeS protease homolog, is a novel virulence factor of Treponema denticola.

Treponema denticola is a major pathogen of chronic periodontitis. Analysis of the T. denticola genome revealed a gene orthologous with a cysteine protease-encoding gene from Streptococcus pyogenes (IdeS). IdeS interferes with IgG-dependent opsonophagocytosis by specific cleavage of IgG molecules. Analysis of this gene (termed ideT) revealed it to encode a two-domain protein whose N-terminus is composed of tandem immunoglobulin-like domains followed by a C-terminal IdeS-like protease domain. In this study we show that during secretion the IdeT protein is processed into an N-terminal fragment wh

Clinical characteristics of Treponema denticola-associated lung abscess diagnosed by metagenomic next-generation sequencing: a case series analysis.

INTRODUCTION: Treponema denticola is an oral anaerobic bacterium commonly associated with periodontitis, but its role in lower respiratory tract infections (e.g., lung abscess) has long been overlooked. For bacteria that grow anaerobically, traditional culture methods exhibit low detection rates, which directly lead to the mis-diagnosis of anaerobic infection. With the ultilization of metagenomic next-generation sequencing (mNGS) in clinical practice, we studied the clinical features and treatment strategies of T. denticola-associated lung abscess. METHODS: A retrospective analysis was conduc

Tooth Autotransplantation of A Mandibular Third Molar Followed by Delayed Root Canal Treatment to Manage External Inflammatory Resorption: A Case Report.

Tooth autotransplantation (TA) is indicated in cases of structurally compromised teeth with poor prognosis when a donor tooth is available. This case report describes the TA of a mandibular right third molar (48) into the position of the mandibular right second molar (47) in a 32-year-old male patient. Clinically, extensive coronal destruction and a sinus tract were observed in tooth 47, rendering retreatment unfavorable. The tooth was diagnosed as previously treated, with a chronic apical abscess. Therefore, tooth 48 was autotransplanted into the 47 position. Four months later, apical externa

Dental Infection Causing Methicillin-Resistant Staphylococcus aureus Bacteremia and Spinal Infection: A Case Report.

Poor dentition as a source of infection causing bacteremia and spinal infections (such as paraspinal abscess, and discitis) should be considered even in the absence of recent dental surgery. The case presents a patient found to have an infection in the cervical and thoracic spine, methicillin-resistant Staphylococcus aureus bacteremia, and poor dentition. Although the patient had a history of drug use, he denied a history of intravenous drug use. He had Crohn's disease that resulted in periodontal and endodontal tooth disease. The patient was found to have poor dentition with erythematous gums

Fusobacterium necrophorum endocarditis with intracranial abscesses: A case report and review of the literature.

UNLABELLED: Fusobacterium species are obligate anaerobic Gram-negative bacilli that are non-spore forming. The two most common species tied to human disease are F. nucleatum and F. necrophorum. They are commensal organisms in the oropharynx, gastrointestinal tract, and genital tract of humans, but can cause invasive infections especially in the head and neck such as periodontal disease and Lemierre's syndrome. F. necrophorum is an uncommon cause of infective endocarditis. We present a case of a previously healthy male who developed F. necrophorum endocarditis with multiple sites of pyogenic ab

Necrotizing fasciitis of odontogenic origin in Ibadan, Nigeria.

We reviewed eight patients with necrotizing fasciitis of odontogenic origin. There were three women and five men, mean age 58 (range 46-72), and none had any associated medical condition such as diabetes. All cases had symptoms of toothache for a mean duration of 34 days (range 26-42) before they sought treatment. Infection originated in the molar teeth region, and initially presented as an odontogenic or periodontal abscess. The clinical features of necrotizing fasciitis became apparent only after the superficial fascia had been invaded. The transient unusually reddish hue for a dark skin mig

Unusual horizontal and vertical root fractures of maxillary molars: an 11-year follow-up.

Intra-alveolar root fractures of posterior teeth are rare when compared with other dental injuries. This case report describes one vertical and two horizontal root fractures of teeth 3, 14, and 15. The teeth all tested normal to cold and electric pulp tests. The patient reported no history of accidental trauma, and no signs of scarring were found. These fractures were discovered during a routine full-mouth radiographic survey. All teeth were asymptomatic and in good function. During the 11 yr that followed, there was no dental treatment, except for routine periodontal maintenance. The patient

A bacteriological and histological evaluation of 58 periapical lesions.

Periapical tissue from 58 cases requiring periapical surgery was examined histologically and cultured for the presence of microbes. Twenty-nine had a possible oral cavity communication and 29 did not. Approximately one-half of each biopsy was submitted for culture while the other portion was examined histologically. Cultures were positive for the presence of bacteria in 51 of 58 cases while bacteria were seen histologically in only 8 of 58 cases. A total of 50 different species of bacteria were isolated from the 58 cultures of periapical tissue. Of 133 isolates, 87 were strict anaerobes, 37 we

Surgical uprighting of mandibular second molars: A single-group retrospective cohort study.

INTRODUCTION: The management of impacted, unerupted, or malpositioned mandibular second molars with orthodontic therapy requires special attention in order to achieve normal anatomic positioning within the dental arch. We present a surgical approach to managing these teeth combining exposure and surgically-assisted forced eruption. METHODS: This retrospective single-group cohort study followed 260 impacted mandibular second molars. The molars were exposed and surgically uprighted. An orthodontic bracket was bonded to aid in orthodontic traction, and the wound was packed to prevent soft tissue

Aggregatibacter actinomycetemcomitans Causing Empyema Necessitans and Pyomyositis in an Immunocompetent Patient.

Empyema necessitans is a relatively rare clinical entity in which the empyema extends through the parietal pleura into the adjacent soft tissue and musculature of the chest wall. It usually occurs due to inadequate treatment of a primary lung infection. Aggregatibacter (formerly Actinobacillus) actinomycetemcomitans is a facultative anaerobic gram-negative coccobacillus that is part of the normal oral flora. Infections due to this organism usually result from aspiration in conjunction with dental disease or trauma to the oral mucosa resulting in pneumonia or empyema. It often coinfects with Ac
