A novel online education module to teach clinicians how to correctly identify ultrasonographic features of mediastinal lymph nodes during endobronchial ultrasound.

BACKGROUND: Ultrasonographic features can be used to predict mediastinal lymph node malignancy during endobronchial ultrasonography. Despite the validity of using these features for this purpose, the features are not being widely used in clinical practice. This may be attributable to the absence of educational programs that teach clinicians how to identify the features. To address this knowledge gap, we developed an online educational module to teach clinicians how to correctly interpret ultrasonographic features. METHODS: The module was designed using corrective feedback and test-enhanced le

The utility of a smartphone electronic referral application in facilitating telemedicine in pulmonology.

BACKGROUND: A streamlined referral system for respiratory diseases is essential, and telemedicine offers a viable solution, particularly within the public healthcare sector. There is a paucity of data on the utility of the Vula medical referral application in pulmonology, a field that lends itself to telemedicine. OBJECTIVES: To perform a 1-year retrospective observational study to assess the Vula medical referral application utility in a tertiary referral centre in Cape Town, South Africa (SA). METHODS: This retrospective study was conducted at a large tertiary-level hospital located in Cap

An Investigation of the Pediatric Rigid Bronchoscopy Complication with Three Different Anesthesia Regimes.

BACKGROUND: Foreign body aspiration is common in children and poses a significant risk of morbidity and mortality. Rigid bronchoscopy is the most common method for removing aspirated foreign bodies. OBJECTIVES: Anesthesiologists play a critical role in managing these procedures, aiming to find the best strategies with the fewest complications. This study aims to compare anesthesia-related complications during rigid bronchoscopy in children using muscle relaxants versus no muscle relaxants. METHODS: In this clinical trial, 60 eligible children were randomly divided into three equal groups: SP

Influenza vaccination in children with pulmonary disease during the COVID-19 pandemic.

INTRODUCTION: Influenza is associated with considerable respiratory morbidity and mortality. Healthcare authorities recommend immunization of all children as vaccinations protect vulnerable populations, minimize influenza triggered asthma attacks, and reduced the burden of respiratory illnesses during the SARS-CoV-2 pandemic. Medical professionals should counsel parents of children with chronic lung disease to receive annual influenza vaccinations as part of supportive care. We aimed to describe adherence to influenza vaccination in respiratory patients and identify potential reasons for non-v

Contribution of four common pulmonary function tests to diagnosis of patients with respiratory symptoms: a prospective cohort study.

BACKGROUND: Few studies of the diagnostic value of pulmonary function testing are available. We assessed the diagnostic contribution of four basic pulmonary function tests: spirometry, lung volume, airway resistance, and diffusing capacity. METHODS: In this prospective cohort study, we enrolled patients presenting to a pulmonologist with respiratory symptoms but no clear diagnosis from 33 hospitals in Belgium. Each patient had spirometry, lung volume, airway resistance, and diffusing capacity testing and all other tests necessary for a definitive diagnosis. Clinical history and pulmonary func

Evaluation of Alexithymia Level in Children With Cystic Fibrosis.

The present study sought to determine the extent of alexithymia within the cystic fibrosis (CF) population and examine how it relates to both the severity of the illness and the effectiveness of patient disease management strategies. The study sample included adolescents aged 14 to 18 years, consisting of individuals with confirmed CF and age-matched healthy controls. Patients with CF exhibited significantly higher total scores on the Toronto Alexithymia Scale (TAS-20) compared to the control group (P = .013). A significant negative correlation was also observed between TAS-20 scores and force

Non-infectious pulmonary and gastrointestinal manifestations in primary antibody deficiencies: lessons for the clinic.

INTRODUCTION: Primary antibody deficiencies (PADs), especially common variable immunodeficiency (CVID), are clinically significant inborn errors of immunity due to complex phenotypes and long-term complications. This review provides an updated overview of pulmonary and gastrointestinal manifestations in PADs, focusing on CVID. AREAS COVERED: We conducted a structured literature review of original articles, reviews, and guidelines from the last 10 years, using databases such as PubMed and Scopus. The focus was on immunopathogenesis, clinical features, and treatment of noninfectious pulmonary a

Acquired Tracheoesophageal Fistulas: A Case Report and Review of Diagnostic and Management Challenges.

Acquired, nonmalignant tracheoesophageal fistulas (TEFs) often occur in the setting of prolonged use of endotracheal or tracheostomy tubes due to trauma and erosion of the tracheal wall inflicted by tube cuffs or direct tracheal contact. In this report, we present a patient with a tracheostomy who presented with recurrent aspiration pneumonia and was found to have a large TEF that was difficult to treat. We also discuss the diagnostic and management challenges concerning TEFs. TEFs, especially if large, lead to recurrent aspiration pneumonia and can be challenging to manage. Definitive managem

Anti-tubercular therapy for intraocular tuberculosis: A systematic review and meta-analysis.

Intraocular tuberculosis remains a diagnostic and management conundrum for both ophthalmologists and pulmonologists. We analyze the efficacy and safety of anti-tubercular therapy (ATT) in patients with intraocular tuberculosis and factors associated with favorable outcome. Twenty-eight studies are included in this review, with a total of 1,917 patients. Nonrecurrence of inflammation was observed in pooled estimate of 84% of ATT-treated patients (95% CI 79-89). There was minimal difference in the outcome between patients treated with ATT alone (85% successful outcome; 95% CI 25-100) and those w

Colonoscopy-associated pneumothorax: a case of tension pneumothorax and review of the literature.

A 64-year-old woman presented with severe abdominal pain and was found to have a large fecolith in the sigmoid colon with resulting bowel obstruction. During a therapeutic colonoscopy, she developed severe shortness of breath and hypoxia, and was found to have a tension pneumothorax. We review the potential mechanisms by which pneumothorax may occur following colonoscopy. In addition, the eight previously published cases are reviewed. Pneumothorax, with or without pneumomediastinum, can occur through a variety of mechanisms following colonoscopy. Although rarely reported, this may represent an

Ultrasound-assisted transthoracic biopsy: cells or sections?

Physicians increasingly use transthoracic ultrasound (US) as an aid for diagnostic procedures. At the bedside, US helps to visualize neoplasms in the chest wall, pleura, peripheral lung, and anterior mediastinum involving or abutting the pleura. Histology specimens from cutting-needle biopsies have been shown to be safe and effective. This prospective study determined the yield and safety of US-guided fine-needle aspiration biopsy (FNAB) as a first-line bedside investigation. We recruited 97 consecutive patients, and of these, 85 underwent both cutting-needle biopsy and FNAB. These were adequa

Multidisciplinary management of interstitial lung disease in autoimmune rheumatic diseases: an Italian Delphi consensus.

BACKGROUND: Although guidelines recommend a multidisciplinary team (MDT) approach for the diagnosis and management of systemic autoimmune rheumatic disease-associated interstitial lung disease (SARD-ILD), they lack guidance on MDT composition and function. This Delphi consensus project aimed to define a shared MDT model for managing patients with SARD-ILD. METHODS: A questionnaire was circulated to an expert panel of 77 Italian pulmonologists, rheumatologists, immunologists, and internal medicine specialists, with statements rated over two voting rounds using a 5-point Likert scale. RESULTS: R

Progressive Pulmonary Fibrosis and Interstitial Lung Abnormalities: AJR Expert Panel Narrative Review.

Progressive pulmonary fibrosis (PPF) and interstitial lung abnormalities (ILA) are relatively new concepts in interstitial lung disease (ILD) imaging and clinical management. Recognition of signs of PPF and identification and classification of ILA are important tasks during chest high-resolution CT interpretation to optimize management of patients with ILD and those at risk of developing ILD. However, in professional society guidance, the role of imaging surveillance remains unclear for stable patients with ILD, asymptomatic patients with ILA who are at risk of progression, and asymptomatic pa

The First Case of Metastatic Pancreatic Leiomyosarcoma Derived from the Urinary Bladder Diagnosed Using an Endoscopic Ultrasound-guided Fine-needle Biopsy.

We herein report the first case of metastatic pancreatic leiomyosarcoma derived from the urinary bladder diagnosed by an endoscopic ultrasound-guided fine-needle biopsy (EUS-FNB) in a 65-year-old woman. The patient had undergone total cystectomy for bladder leiomyosarcoma. Four years thereafter, a nodule was observed in her left lung on chest computed tomography. Suspecting primary lung cancer, pulmonologists at our hospital recommended a thoracoscopic lung biopsy, which the patient refused. Five years post-cystectomy, fluorodeoxyglucose positron emission tomography revealed enlargement of the

Multidisciplinary Perioperative Care for Children with Neuromuscular Disorders.

Children with neuromuscular diseases present unique challenges to providing safe and appropriate perioperative care. Given the spectrum of disease etiologies and manifestations, this is a population that often requires specialized multidisciplinary care from pediatricians, geneticists, neurologists, dieticians, and pulmonologists which must also be coordinated with surgeons and anesthesiologists when these patients present for surgery. Several of these diseases also have specific pharmacologic implications for anesthesia, most notably mitochondrial disease and muscular dystrophies, which put t

Pulmonary benign metastasizing leiomyoma presented as acute respiratory distress.

Benign metastasizing leiomyoma (BML) is a very rare condition and is characterized by the presence of benign smooth muscle tumours in organs distant from the uterus, most commonly the lung. It generally affects women of reproductive age and prognostic is usually excellent. However, the course of the disease is unpredictable. We report here the case of a 76-year-old woman with a previous medical history of uterine benign leiomyomas in whom BML was acutely revealed by a respiratory distress due to voluminous pulmonary and pleural leiomyomas requiring surgical extraction. Clinical evolution was r

Late onset toxicities associated with the use of CDK 4/6 inhibitors in hormone receptor positive (HR+), human epidermal growth factor receptor-2 negative (HER2-) metastatic breast cancer patients: a multidisciplinary, pan-EU position paper regarding their optimal management. The GIOCONDA project.

The personalization of therapies in breast cancer has favoured the introduction of new molecular-targeted therapies into clinical practice. Among them, cyclin-dependent kinases 4 and 6 (CDK4/6) inhibitors have acquired increasing importance, with the approval in recent years of palbociclib, ribociclib, and abemaciclib in combination with endocrine therapy. Currently, no guidelines are available to monitor and manage potential long-term toxicities associated with the use of these drugs. A multidisciplinary panel of European oncologists, was supported by a pharmacologist, a hematologist, a hepat
