Pyogenic abscesses and parasitic diseases.

Parasitic diseases which during their course in the host switch the immune system from a T helper 1 to a T helper 2 response may be detrimental to the host, contributing to granuloma formation, eosinophilia, hyper-IgE, and increased susceptibility to bacterial and fungal infections. Patients and animals with acute schistosomiasis and hyper-IgE in their serum develop pyogenic liver abscess in the presence of bacteremia caused by Staphylococcus aureus. The Salmonella-S. mansoni association has also been well documented. The association of tropical pyomyositis (pyogenic muscle abscess) and pyogen

Anakinra long-term efficacy and safety in the management of Schnitzler's syndrome and latent tuberculosis infection.

UNLABELLED: The current research addresses dynamics of the COVID-19 pandemic's disruption to goal pursuit. Specifically, we examined the effects of disengaging from frozen goals (goals for which progress had been disrupted due to COVID-19). In May 2021, we asked participants (N = 226) what percentage of their goals were COVID-frozen goals and asked them to report their engagement in one such goal (vs. an active goal): the degree to which they think about the goal, ruminate about the goal, and are committed to the goal. Participants also reported on two facets of their recent well-being: psycho

[Inflammatory myopathy following acute meningoccemia in a properdin-deficient patient: A case report].

INTRODUCTION: Myalgia is a classical sign in invasive meningococcal diseases (IMD), but severe and persistent myalgia following an IMD have never been reported to date. CASE REPORT: A 20-year-old man presented with purpura fulminans and meningitis caused by Neisseria meningitidis serogroup Y, revealing properdin deficiency. Although meningitis symptoms improved after antibiotherapy, initial myalgia of the lower limbs increased, associated with mild rhabdomyolysis. Magnetic resonance imaging (MRI) revealed an increased STIR (Short TI inversion recovery) signal of both quadriceps muscles, witho

A Late and Complex Presentation of Hereditary Haemochromatosis.

We report a case of a 78-year-old male with a complex presentation that first diverted our attention from the underlying hereditary haemochromatosis (HH). A fit patient who initially came with leg pain and eventually died within 3 months of presenting with several syndromes relatable to HH that uncommonly manifest together. His initial presentation was pyomyositis in the thigh muscles followed by a diagnosis of myelodysplasia - refractory anaemia with excess blasts (RAEB), congestive cardiac failure and liver abscesses. End-stage heart failure and recurrent infections were the main causes of t

Imaging of Musculoskeletal Soft-Tissue Infections in Clinical Practice: A Comprehensive Updated Review.

Musculoskeletal soft-tissue infections include a wide range of clinical conditions that are commonly encountered in both emergency departments and non-emergency clinical settings. Since clinical signs, symptoms, and even laboratory tests can be unremarkable or non-specific, imaging plays a key role in many cases. MRI is considered the most comprehensive and sensitive imaging tool available for the assessment of musculoskeletal infections. Ultrasound is a fundamental tool, especially for the evaluation of superficially located diseases and for US-guided interventional procedures, such as biopsy

Salmonella neck abscess as an opportunistic infection in diabetes mellitus.

Salmonella neck infections represent an uncommon cause of focal salmonellosis. While the incidence of nontyphoid salmonellosis is estimated at over 2 million cases annually, extraintestinal manifestations account for less than 1% of cases. This paper describes two patients with Salmonella neck abscesses as the initial presentation of diabetes mellitus. The first patient was diagnosed as having Salmonella enterica serotype Enteritidis sternocleidomastoid pyomyositis and the second patient Salmonella enterica serotype Typhimurium parapharyngeal abscess. Both patients had elevated hemoglobin A1c

Infiltrative cervical lesions causing symptomatic occipital neuralgia.

BACKGROUND: Occipital neuralgia is a well-recognized cause of posterior head and neck pain that may associate mild sensory changes in the cutaneous distribution of the occipital nerves, lacking a recognizable local structural aetiology in most cases. Atypical clinical features or an abnormal neurological examination are alerts for a potential underlying cause of pain, although cases of clinically typical occipital neuralgia as isolated manifestation of lesions of the cervical spinal cord, cervical roots, or occipital nerves have been increasingly reported. CASE REPORTS: We describe two cases

Imaging of musculoskeletal soft tissue infections.

Prompt and appropriate imaging work-up of the various musculoskeletal soft tissue infections aids early diagnosis and treatment and decreases the risk of complications resulting from misdiagnosis or delayed diagnosis. The signs and symptoms of musculoskeletal soft tissue infections can be nonspecific, making it clinically difficult to distinguish between disease processes and the extent of disease. Magnetic resonance imaging (MRI) is the imaging modality of choice in the evaluation of soft tissue infections. Computed tomography (CT), ultrasound, radiography and nuclear medicine studies are con

A case of varicella complicated by cellulitis and scarlet fever due to Streptococcus pyogenes.

We report a 4-year-old boy with cellulitis and scarlet fever due to streptococcal infection following the onset of varicella. He developed a painful ulcer and subcutaneous induration on the left shoulder and a small, light-red-colored rash on the trunk at approximately the same time as the development of vesicles over the entire body. Streptococcus pyrogenes was isolated from samples from the posterior intranasal space and the ulcer on the shoulder. The clinical symptoms improved with the administration of antibiotics and intravenous drip infusion, but it took approximately one month from the

[The association between human toxocariasis and pyogenic abscesses].

The association between hepatic abscesses and schistosomiasis mansoni was confirmed by clinical and experimental studies. Other parasites may cause systemic immunologic changes and local structural alterations in the affected organs that can facilitate the seeding of these areas by bacteria. Tropical pyomyositis, pyogenic liver and renal abscesses are frequent diseases in tropical areas. The visceral larva migrans syndrome is caused by the presence, in the human body, of larvae of worms that have other animals as their definitive host, most commonly being caused by Toxocara canis. The larvae m

Ultrasound of Musculoskeletal Infections.

Ultrasound (US) can play an important role in differentiating infectious processes from other musculoskeletal diseases. US is the imaging modality of choice to diagnose superficial abscesses. These abscesses may have variable echogenicity and their detection can be facilitated by ultrasonographic dynamic evaluation and color Doppler imaging. Septic arthritis either in a native or prosthetic joint may have devastating complications on the joint function. Early diagnosis requires joint fluid analysis. US is very sensitive in the detection of joint effusions and is helpful to guide the arthrocent

Sonographic Findings of Common Musculoskeletal Diseases in Patients with Diabetes Mellitus.

Diabetes mellitus (DM) can accompany many musculoskeletal (MSK) diseases. It is difficult to distinguish the DM-related MSK diseases based on clinical symptoms alone. Sonography is frequently used as a first imaging study for these MSK symptoms and is helpful to differentiate the various DM-related MSK diseases. This pictorial essay focuses on sonographic findings of various MSK diseases that can occur in diabetic patients.

A challenging case of septic sacroiliitis with psoas involvement.

OBJECTIVE AND IMPORTANCE: We report on a rare case of septic arthritis of the sacroiliac joint in a young patient without apparent predisposing factors. CLINICAL PRESENTATION: A 22-year-old female presented with severe right pelvic and gluteal pain and systemic illness. She had a recent diagnosis of bilateral sacroiliitis. INTERVENTION: Systemic illness quickly subsided after initiation of intravenous antibiotic treatment. Subsequently, gluteal pain gradually diminished. CONCLUSION: Pyogenic sacroiliitis should be taken into account in patients with sacroiliitis and fever onset. Magnetic re

Rheumatic manifestations of HIV/AIDS.

Rheumatic manifestations of HIV infection encompass a wide spectrum of disorders, arising from direct viral effects, immune dysregulation, opportunistic infections, or antiretroviral therapy (ART)-induced complications. These manifestations can occur at any stage of HIV, and include arthralgia, HIV-associated arthritis, spondyloarthropathies (SpA), inflammatory myopathies, vasculitides, and opportunistic musculoskeletal infections. Arthralgia is common and self-limiting, whereas HIV-associated arthritis mimics rheumatoid arthritis. SpA, particularly reactive arthritis, psoriatic arthritis, and

Intermuscular hydatid cyst in the thigh musculature: a case report.

INTRODUCTION: Echinococcus granulosus, a tapeworm, is responsible for causing hydatid disease. Hydatid cysts rarely affect the musculoskeletal system, occurring in less than 5% of cases. This case report presents the clinical presentation, diagnosis, and management of muscle echinococcosis. CASE PRESENTATION: A 28-year-old Black African female presented with a 4-month history of left anteromedial thigh swelling. Despite mild burning pain, she remained functional. While pyomyositis was initially considered, ultrasound and magnetic resonance imaging suggested hydatid disease. Surgical excision

[Septic arthritis and transient synovitis of the hip].

CLINICAL/METHODICAL ISSUE: Differentiating between septic arthritis and transient synovitis can be challenging but is very important as a late diagnosis of septic arthritis can lead to sepsis and joint damage. For correct diagnosis and prediction of complications, the right combination of physical examination, laboratory and radiological studies is needed. STANDARD RADIOLOGICAL METHODS: Hip ultrasound is easy to learn and has a high sensitivity for joint effusion. Faster diagnosis and therapy are possible due to increasing use of ultrasound. Magnetic resonance imaging (MRI) is primarily used

Muscle infections caused by Salmonella species: case report and review.

We describe a patient with salmonella pyomyositis and review 30 other cases reported during the past 4 decades. Men outnumbered women by 2.9 to 1, and the median age of the patients was 51 years. Approximately one-half the cases were caused by Salmonella enteritidis. Infected vascular aneurysms were observed in seven patients. Prior salmonella infections and local trauma or lesions were common. Diverse underlying conditions, mainly diabetes and human immunodeficiency virus infection, were present in 81% of the patients, and the psoas muscle was involved in 55% of the cases. One-third of the pa

Orthopaedic surgery and HIV disease in Africa.

Human immunodeficiency virus (HIV) infection has radically changed African orthopaedic practice within a decade. In Lusaka, a third of adults are infected, but most have no physical signs of the disease. Early experience showed that closed fractures healed normally, the risk of sepsis during osteosynthesis was increased and most open fractures became septic. Major orthopaedic surgery in HIV-positive patients has increased risks of sepsis which rise steeply in those with physical signs of HIV disease. Musculoskeletal infections such as tropical pyomyositis, adult haematogenous long-bone osteomy

Dieulafoy's lesion in a paediatric patient on selective serotonin reuptake inhibitors (SSRIs) therapy following short-term treatment with non-steroid anti-inflammatory drugs (NSAIDs).

We present a case of severe upper gastrointestinal bleeding due to Dieulafoy's lesion in a boy in early adolescence, likely triggered by a short-term treatment of anti-inflammatory drugs (NSAIDs) and ongoing selective serotonin reuptake inhibitor (SSRI) therapy. The patient initially presented with hip pain, fever, vomiting and diarrhoea and was treated with NSAIDs for suspected reactive coxitis. He was later diagnosed with pyomyositis. His history included anxiety disorder treated with SSRIs. Clinical findings revealed elevated inflammatory markers, joint infection with Staphylococcus aureus
