Population aging and changing hospitalization risks in Germany: a decomposition of changes in inpatient cases, 2005-2021.

BACKGROUND: Inpatient care improves and maintains population health but requires a clear understanding of the drivers of inpatient care utilization. In Germany, inpatient cases increased from 16.5 to 19.4 million between 2005 and 2019 and dropped tremendously during the Covid-19 pandemic. We investigate how population aging and changing hospitalization risks in the German population contributed to the changes in inpatient case numbers remunerated according to the German Diagnosis-Related Groups (G-DRG) system between 2005 and 2021. METHODS: The analysis is based on more than 280 million cases

[Early detection of prostate cancer by PSA testing: the results of a qualitative study on barriers caused by physicians in Austria implementing informed decision making].

OBJECTIVES: In the context of the Austrian Periodic Health Examination and on the basis of current evidence based medicine a systematic screening for prostate-specific antigen (PSA) is inadvisable. General practitioners (GPs) are expected to inform their patients about risks and benefits of screening before undergoing a PSA test. Ideally, this information corresponds to the model of informed decision making (IDM).The aim of this qualitative pilot-study is to explore factors, which gain insight into GPs' practice of counselling about the PSA test. METHODS: This qualitative pilot study involves

Plasma metabolites associated with endometriosis in adolescents and young adults.

STUDY QUESTION: What are the plasma metabolomics profiles associated with endometriosis in adolescents and young adults? SUMMARY ANSWER: Our findings show dysregulation of plasma metabolomic profiles in adolescents and young adults with endometriosis, revealing systemic elevation of fatty acyls and ceramides in endometriosis cases compared to controls. WHAT IS KNOWN ALREADY: Endometriosis is a gynecologic disease often presenting with severe pelvic pain impacting around 200 million reproductive-aged women worldwide. However, little is known about the pathophysiology and molecular features of

Obstacles and Opportunities for Albuminuria Testing On the Basis of the Perspective of Primary Care: A Qualitative Study.

KEY POINTS: Pay-for-performance indicators and lack of knowledge about new drugs limit general practitioners' ability to identify target populations and perform urinary protein assays. Choosing between several possible assays is associated with confusion and exposes general practitioners to the risk of inappropriate referrals to nephrologists. Revising pay-for-performance indicators, drafting multidisciplinary guidelines, raising multiprofessional collaboration, and patient awareness should be considered. BACKGROUND: Albuminuria testing is an easy way to identify, early on, a higher risk of c

Postsymptomatic restoration of SMN rescues the disease phenotype in a mouse model of severe spinal muscular atrophy.

Transfusion of blood and blood products has been associated with transmission of infectious agents. However, it is probable that blood products are currently very safe and that pooled virus-inactivated products from remunerated donors are now safer than untreated single voluntary donor components. Although the transmission events of the past and the present are reasonably well understood, reliance on a linear approach to predict safety in the future is open to criticism. Indeed, it was not possible to predict the extent or consequences of the AIDS epidemic or of hepatitis C transmission. Moreo

Estimating the Proportion of Transplant-Eligible Patients in Diffuse Large B-Cell Lymphoma: Insights from German Hospital Billing Data.

INTRODUCTION: Treatment, according to guidelines, for second-line and beyond (2L+) therapy of diffuse large B-cell lymphoma (DLBCL) differentiates between transplant-eligible and transplant-ineligible patients. Transplant-eligible patients typically receive salvage immunochemotherapy followed by high-dose chemotherapy and autologous stem cell transplantation (HD-ASCT) or chimeric antigen receptor T-cell therapy. Although literature suggests a 50-50 distribution between transplant-eligible and transplant-ineligible patients, real-world evidence from Germany is lacking. This study aimed to estim

Implementing pharmacist-led home medicines reviews within fracture liaison services for better post-fracture care: a qualitative study of patient and practitioner experience.

UNLABELLED: Home medicines reviews present a novel intervention supporting continuity of care between fracture liaison services and primary care. Home medicines review pharmacists deliver multifaceted, collaborative interventions that can bridge gaps in care, promote patient empowerment, facilitate a GP-patient discussion about bone health, and improve coordination of care. PURPOSE: Medication review has potential to reduce the risk of falls and fractures and improve transition of care from a specialist fracture liaison service to primary care. This study was designed to examine and evaluate

Wellness activities address inequities.

The medical model is no longer accepted by many as the best means of achieving optimal health. Financial constraints are pushing more efficient and effective ways to deliver services. In Saskatchewan, greater emphasis is being placed on wellness activities (preventive medical counselling, clinical work with other professionals, training, teaching and research and institutional medical administrative duties). We sought to determine if predicted support for these activities was related to equity of income as perceived by physicians. The study design was a cross-sectional study of all 1462 physic

Medical assistance in dying (MAiD) in Canada: practical aspects for healthcare teams.

In this paper we document some of the practical aspects of implementing medical assistance in dying (MAiD) since it became legal in Canada in 2016. The percentage of annual deaths in Canada due to MAiD varies widely, ranging from less than 0.5% in some areas to over 5% in others. By the end of 2019, approximately 13,000 people had an assisted death in Canada (1.6% of all deaths). The average age is 73 years and the majority have cancer (64%), followed by end-stage organ failure (17%), and neurological disease (11%). The safeguards in Canadian law include having two witnesses sign the patient r

Preferences for Internet Plus Nursing Services Among Nurses and Patients in China: A Systematic Review of Discrete Choice Experiments.

BACKGROUND: Against the backdrop of an aging population, the traditional nursing model struggles to meet diverse needs. Despite the significant social benefits of internet plus nursing services, misaligned preferences between nurses and patients hinder their widespread adoption. Discrete choice experiments (DCEs) are a key tool for quantifying stakeholder preferences and improving service adoption. No review has systematically synthesized evidence on patients' and nurses' preferences for internet plus nursing services. OBJECTIVE: To systematically review DCEs conducted in China that examined

Global financing of strabismus surgery: a literature review supported by an international survey.

PURPOSE: Strabismus surgery is a well-established intervention with demonstrated functional and psychosocial benefits, supported by extensive clinical and economic literature. Despite this evidence, reimbursement policies remain highly heterogeneous worldwide, often influenced by the persistent ambiguity between functional and cosmetic indications. A comprehensive synthesis of existing cost data, combined with real-world reimbursement practices, is required to better understand barriers to equitable access to care. METHODS: We conducted a targeted literature review to identify published data

Are we still too late? Timing of orchidopexy.

An undescended testis is the most common genitourinary disease in boys. The German guidelines, first published in 2009, proposed the timing of orchidopexy to be before 12 months of age. The aim of the study was to analyze the implementation of these guidelines 10 years after publication. The national cumulative statistics of hospital admissions, provided by the Institute for the Remuneration System in Hospitals (InEK), and the statistics concerning procedures performed in private pediatric surgical practices of the professional association of pediatric surgeons (BNKD) regarding the time of sur

How closely are outcome questionnaires correlated to patient satisfaction after cervical spine surgery for myelopathy?

OBJECTIVE: Patient-reported outcomes (PROs) have become the standard means to measure surgical outcomes. Insurers and policy makers are also increasingly utilizing PROs to assess the value of care and measure different aspects of a patient's condition. For cervical myelopathy, it is currently unclear which outcome measure best reflects patient satisfaction. In this investigation, the authors evaluated patients treated for cervical myelopathy to determine which outcome questionnaires best correlate with patient satisfaction. METHODS: The Quality Outcomes Database (QOD), a prospectively collect

Temporal Trends of Adults Gaining or Losing Enrolment in Primary Care Medical Homes in Ontario, Canada: A Retrospective Serial Cohort Analysis.

Introduction Patient attachment to a primary care medical home has emerged as a key strategy to strengthen population health in high-income countries, including the U.S., Canada, New Zealand, the United Kingdom, and other European countries. In Canada, Ontario introduced primary care medical homes beginning in 2002, with capitation-based and interdisciplinary team-based models becoming increasingly prominent over time. Citing cost concerns, however, various government policies were subsequently introduced to limit physician entry into these types of medical homes. Entry into medical homes, rem
