"I want what the white lady gets…": Deconstructing racial disparities in gynecological cancer.

Despite decades of diversity and inclusion initiatives in healthcare, racial disparities in gynecological cancer diagnosis, treatment, and outcomes persist and, in some cases, are worsening. Black women continue to experience later-stage diagnoses, diminished provider communication, and poorer survivorship outcomes. This paper presents findings from five focus groups conducted with individuals (n = 26) diagnosed with gynecologic cancers, guided by an intersectional lens rooted in Black feminist theory. Using thematic analysis, four recurring themes emerged: (1) diagnostic delays and dismissal,

Adolescence Does Not Divert Sex Offending Adolescents of Color from Criminal Legal System Involvement.

After the United States Civil War, during Reconstruction, Southern states targeted Black youth and men for incarceration and forced labor, often charging them with rape, spawning the Black male rapist myth. This study explores evidence of a Reconstruction-era ethos in present-day treatment of youth of color accused of sexual assault. Specifically, we examined effects of perpetrator age and race on legal outcomes in 382 alleged child sexual abuse cases. There were 291 adult perpetrators (91% men, M age = 34; 76% White, 20% Black, 2% biracial, 2% Latinx, and 1% Asian) and 91 adolescent perpetrat

An opportunity to be grateful for? Exploring discourses about international medical graduates from India and Pakistan to the UK between 1960 and 1980.

INTRODUCTION: Following India and Pakistan gaining independence from British colonial rule, many doctors from these countries migrated to the UK and supported its fledgling National Health Service (NHS). Although this contribution is now widely celebrated, these doctors often faced hardship and hostility at the time and continue to face discrimination and racism in UK medical education. This study sought to examine discursive framings about Indian and Pakistani International Medical Graduates (IPIMGs) in the early period of their migration to the UK, between 1960 and 1980. METHODS: We assembl

From the Heart to the Brain: Building Bridges to a Better Future.

This Presidential Address was delivered at the International Stroke Conference in March 2021, during the coronavirus pandemic. Dr Elkind, the President of the American Heart Association (AHA) at the time, is a vascular neurologist with a research focus on stroke epidemiology. This address interweaves personal reflections on a career in clinical neurology, stroke research, and public health with a discussion of the role of the AHA in improving cardiovascular health at multiple levels. Throughout its history, the AHA has had leaders representing many different areas of cardiovascular science and

Water insecurity: An agenda for research and call to action for human biology.

Water insecurity-the lack of adequate and safe water for a healthy and productive life-is one of the greatest threats facing humans in the coming century. By 2030, half of the world is expected to be living in water-stressed conditions, given current climate change scenarios. A key goal of the UN Water Action Decade and Sustainable Development Goal 6 is to improve water security for the three billion people globally affected, but the future looks grim. For many communities, from Cape Town, South Africa to Flint, United States, the imagined dystopian future of severe water shortages has already

Perceptions of Needs, Assets, and Priorities Among Black Men Who Have Sex With Men With HIV: Community-Driven Actions and Impacts of a Participatory Photovoice Process.

Black men who have sex with men (MSM) with HIV experience significant health inequities and poorer health outcomes compared with other persons with HIV. The primary aims of this study were to describe the needs, assets, and priorities of Black MSM with HIV who live in the Southern United States and identify actions to improve their health using photovoice. Photovoice, a participatory, collaborative research methodology that combines documentary photography with group discussion, was conducted with six Black MSM with HIV. From the photographs and discussions, primary themes of discrimination an

The Political Future of Social Medicine: Reflections on Physicians as Activists.

The academic discipline of social medicine has always had a political and policy advocacy component, in addition to its core functions of research and teaching. Its origins lie in the 18th and 19th centuries, in the work of Johann Peter Frank and Rudolph Virchow, among others. Virchow's dictum that "politics is nothing else but medicine on a large scale" highlights that most social determinants of health are politically determined and shape population health. Yet despite intense epidemiological and sociological research on the social determinants of health, less attention has been paid to this

The Anchored Testimony Calendar: A Black Feminist Method for Retrospective Behavioral Health Inquiry with Justice-Impacted Young Adults and Caregivers.

Retrospective behavioral health instruments calibrated for chronological precision and discrete-symptom reconstruction can fail people whose lives have been organized by structural racism, sexism, and economic inequality. We describe the Anchored Testimony Calendar (ATC), a Black feminist adaptation of the event history calendar developed within the Epidemiological Project Involving Children in the Court (EPICC), a longitudinal study of justice-impacted young adults and caregivers launched in 2014. ATC pre-populates an interviewer-facing four-domain grid-behavioral and psychiatric health, educ

Qualitative Analysis of Views of Psychedelic-Assisted Therapy in Low-Income, Urban, Black Americans.

INTRODUCTION: Black Americans are significantly underrepresented in psychedelic trials despite significant mental health disparities. Growing research suggests that psychedelic-assisted therapy (PAT) is an effective treatment for post-traumatic stress disorder and depression. It is important to understand perceptions of PAT among populations with unmet mental health needs. METHODS: We conducted four focus groups to understand perceptions of psychedelic therapy within a low-income, Black, urban population. METHODS: Three main themes were identified using an inductive thematic analysis approac

In our responses to the overdose epidemic, we cannot forget pregnant and postpartum people.

In 2021, there were over 100,000 drug overdose deaths in the United States (US). Death rates have increased faster among women than men, particularly among Black and Indigenous people. Although drug overdose is a leading cause of pregnancy-associated deaths, birthing people are rarely emphasized in discussions of overdose and research and services remain limited. Data show increases in drug use and deaths among women of child-bearing age, with risks continuing in the postpartum period. Harms experienced by birthing people who use drugs occur in the context of broader inequities in maternal mor

Effects of Neighborhood-level Data on Performance and Algorithmic Equity of a Model That Predicts 30-day Heart Failure Readmissions at an Urban Academic Medical Center.

BACKGROUND: Socioeconomic data may improve predictions of clinical events. However, owing to structural racism, algorithms may not perform equitably across racial subgroups. Therefore, we sought to compare the predictive performance overall, and by racial subgroup, of commonly used predictor variables for heart failure readmission with and without the area deprivation index (ADI), a neighborhood-level socioeconomic measure. METHODS AND RESULTS: We conducted a retrospective cohort study of 1316 Philadelphia residents discharged with a primary diagnosis of congestive heart failure from the Univ

Racialized Mass Incarceration and the War on Drugs: A Critical Race Theory Appraisal.

The purpose of this column is to summarize important aspects of the racialized War on Drugs, including (a) the school-to-prison pipeline, (b) the for-profit prison system ("prison industrial complex"), (c) racialized mass incarceration, and (d) the disproportionately negative impact of the War on Drugs on families and communities of color. Analysis of critical race theory (CRT), the study of the relationships between race, racism, and power, will provide a cohesive framework for examining these four aspects. CRT maintains that American laws, including antidiscrimination laws, are structured to

Area-Level Social Determinants of Alcohol-Related Mortality: Knowledge Gaps and Implications for Community Health.

PURPOSE: Rates of alcohol-related mortality (including deaths attributed to chronic alcohol use as well as acute causes involving alcohol) have been increasing in the United States, particularly for certain population subgroups, such as women. This review summarizes associations of area-level social determinants of health with alcohol-related mortality. These determinants, measured at the community, county, or state level, include alcohol control policies, health care availability, and a community's socioeconomic environment. Examining multiple geographic levels illuminates how macro-level soc

Decolonizing global health: a scoping review.

We summarize research on decolonizing global health and highlight existing gaps, including the lack of a formal definition and clear aims for the movement. We examine the decolonization of global health curricula and authorship in indexed journals, aiming to build a shared understanding among global health scholars. The review included studies from all global regions to ensure comprehensive geographical representation. Of the 155 identified records, most were published from 2021 onward and predominantly originated from the Global North. Studies focusing solely on racism were excluded. Addition

Systemic inequities in alcohol licensing: Case studies in eight Aotearoa New Zealand communities.

INTRODUCTION: Many countries, including Aotearoa New Zealand, have socioeconomic and ethnic inequities in alcohol outlet density, yet the potential contribution of alcohol licensing systems is almost unexplored. After licensing reforms in Aotearoa in 2012, community groups and Māori (the Indigenous people) continued to struggle to influence decisions, prompting calls for reform and authority for Māori reflecting Te Tiriti o Waitangi obligations. This study explored factors in the failure of public objections in under-resourced neighbourhoods. METHODS: In a descriptive, multimethod qualitative

"I Wasn't Presented With Options": Perspectives of Black Veterans Receiving Care for Uterine Fibroids in the Veterans Health Administration.

INTRODUCTION: Black women with uterine fibroids experience greater symptom severity and worse treatment outcomes compared with their White counterparts. Black veterans who use Veterans Health Administration (VA) health care experience similar disparities. This study investigated the experiences of Black veterans receiving care for uterine fibroids at VA. METHODS: We identified Black veterans aged 18 to 54 years with newly diagnosed symptomatic uterine fibroids between the fiscal years 2010 and 2012 using VA medical record data, and we recruited participants for interviews in 2021. Multidimensional Determinants of Food and Nutritional Insecurity Among Older Adults: A Scoping Review.

Background/Objectives: Food and nutritional insecurity (FNI) is a major social determinant of health that disproportionately affects older adults, with significant implications for their health, nutrition, and well-being. In this context, this scoping review aims to map and synthesize the available scientific evidence on the main determinants of FNI among older adults, considering socioeconomic, health-related, functional, psychosocial, and structural factors. Methods: A scoping review was conducted in accordance with the Joanna Briggs Institute methodology and reported following the PRISMA-Sc

Navigating fetal conditions through a Reproductive Justice lens: lessons from composite cases in the United States.

BACKGROUND: Every day, pregnant people get the news that the futures they imagined for their families are at risk due to a fetal condition. It may be something relatively minor, like a small benign growth on a lung, or something major, like a life-threatening heart or developmental problem. These fetal conditions sit at the intersection of maternity care, high-risk pregnancy services, fetal therapy, family planning, pediatric specialty care, community supports, insurance structures, and policy environments. Despite the profound inequities embedded in these systems, Reproductive Justice has rar

Blue Circle Health: A Novel Patient-Centered Model of Health Care Delivery for Low-Income Patients With Type 1 Diabetes.

Analog insulins, insulin pumps, and continuous glucose monitors (CGM) have revolutionized type 1 diabetes (T1D) treatment over the last 50 years. Nevertheless, less than 20% of patients in the United States reach guideline-based HbA1c targets. The dysfunctional delivery of U.S. health care has further worsened glycemic outcomes among structurally disadvantaged groups such as non-Hispanic Black and low-income populations. Administrative complexities resulting from mixed insurance coverage and delivery systems, incongruity between effective policies and reimbursement, structural racism, and impl
