The peripheral vestibular system is faced by a sensory ambiguity, where primary otolith afferents respond identically to translational (inertial) accelerations and changes in head orientation relative to gravity. Under certain conditions, this sensory ambiguity can be resolved using extra-otolith cues, including semicircular canal signals. Here we review and summarize how neurons in the vestibular nuclei, rostral fastigial nuclei, cerebellar nodulus/uvula, and thalamus respond during combinations of tilt and translation. We focus primarily on cerebellar cortex responses, as nodulus/uvula Purki

A 10-year-old boy with Duchenne muscular dystrophy (DMD) underwent uvulopalatopharyngoplasty and tonsillectomy for obstructive sleep apnea (OSA). He has snored loudly during nocturnal sleep since infancy, and OSA developed one and a half years after he was wheel chair-bound at 8 years. Magnetic resonance imaging (MRI), conventional spirometry and overnight polysomnography were performed before and after the surgery. MRI before surgery revealed moderate tonsillar hypertrophy and a long uvula which narrowed the upper airway. After the surgery, his vital capacity (VC) was not changed (% VC; from

The projections from the pontine nuclei and the necleus reticularis tegmenti pontis (N.r.t.) onto the flocculus, uvula, and the paramedian lobule were studied with retrograde transport of horseradish peroxidase n the rhesus monkey. The main findings are as follows: There is a conspicuous tendency for labeled cells to occur in numerous discrete clusters in the pontine nuclei after injections of these parts of the cerebellum. There appears to be very limited overlap between pontine cell groups projecting to the flocculus, the uvula, and the paramedian lobule, respectively. The flocculus appears

The removal of the uvula for minor pharyngeal lesions is unusual in Western otorhinolaryngological practice. However, uvulectomy is a common procedure for "throat problems' in Nigeria and some other African countries by traditional surgeons. This is because it is believed that the elongated uvula is the root cause of all throat problems. These traditional surgeons are usually barbers by profession and only practice surgery on a part-time basis. Most of their patients do well but some have to be rushed to the hospital with severe post-operative bleeding. Attention of Europeans and other foreign

It is common practice in many parts of Africa for traditional healers to remove the uvula to prevent infections and other disorders associated with the throat. We report two cases of children presenting with recurrent tonsillitis. On examination both children showed complete absence of the uvula and prominent tonsils. It was later revealed that they both had undergone uvulectomy, performed by traditional healers in Eritrea. We report this because this practice, although commonly performed, is little known to otolaryngologists practising outside Africa. In addition, although velopharyngeal comp

Two groups of patients undergoing uvulopalatopharyngoplasty were studied. Group I patients underwent en bloc resection of tonsils and palate with the line of excision less than 1.5 cm anterior to the uvula-soft palate junction. Group II patients underwent tonsillectomy followed by palatectomy with a margin 1.5 to 2.0 cm anterior to the uvula-soft palate junction. Only 1 of 7 patients in Group I was objectively improved, whereas 11 of 13 patients in Group II improved. We conclude that a generous resection of soft palate and tonsillar tissue is mandatory for success of uvulopalatopharyngoplasty

AIM: To investigate otolaryngological abnormalities associated with congenital nasolacrimal duct obstruction (CNLDO) and their effect on the prognosis.
METHODS: 65 consecutive cases of CNLDO were followed up with routine otorhinolaryngological examination with tympanometry.
RESULTS: Otitis media with effusion (OME) and uvula bifida were detected in 44.6% and 9.2% of the children, respectively. Medical treatment and probing were less effective in patients with OME (p<0. 05).
CONCLUSIONS: OME and uvula bifida are significant anomalies associated with CNLDO and the former has a marked effect o

A case of familial occurrence of syngnathia congenita in mother and child is described; this syndrome has been seen only sporadically until now. Due to the distinct appearance of the intraoral chords--they were situated on both sides between the ridges of the jaw and also between the left half of the bifid uvula and the retrolingual region--a relationship to the autosomal-dominantly transmitted Cleft Palate Lateral Synechiae Syndrome (CPLS-Syndrome) is suspected. Complete penetrance with different expressivity may explain the different clinical manifestations of the cases seen up to now and th

Mapping with local lesions, electrical or chemical stimulation, or recording evoked field potentials or unit spikes revealed localized representations of cardiovascular functions in the cerebellum. In this review, which is based on literatures in the field (including our own publications), I propose that the cerebellum contains five distinct modules (cerebellar corticonuclear microcomplexes) dedicated to cardiovascular control. First, a discrete rostral portion of the fastigial nucleus and the overlying medial portion of the anterior vermis (lobules I, II and III) conjointly form a module that

Retrograde transganglionic labeling techniques with biotinylated dextran amine (BDA) were used to examine the terminal field structure and topographical patterns of innervation within the vestibular sensory end organs of vestibular primary afferent neurons projecting to the cerebellar uvula/nodulus and flocculus lobules in the gerbil. Robust, dark labeling in the cristae ampullares suggested that the vast majority of the terminals of afferent neurons were of the dimorphic type. The majority (94% to the uvula/nodulus and 100% to the flocculus) innervates the peripheral zones of each of the thre

Congenital curved nail of the fourth toe represents an unusual deformity in which the nail of the fourth toe curves in a plantar direction. It is unknown why only this toe is affected; however, the initial descriptions suggest that this is a mesodermal defect. We describe a case of congenital curved nail of the fourth toe associated with bifid uvula. There are only 2 reports mentioning the association of this nail abnormality with cleft palate and/or lip. In our patient, there was an association with bifid uvula that is considered a minor form of palatine fissure. There seems to be an associat

We describe a rare case of a previously healthy 30-year-old man who presented to the Emergency Department (ED) with bilateral peritonsillar abscesses. The clinical presentation of fever, trismus and odynophagia was consistent with pertonsillar abscess (PTA), but the presence of symmetric tonsillar swelling and midline uvula confounded the diagnosis. The true incidence of bilateral peritonsillar abscesses is unknown, but the incidence of unsuspected contralateral peritonsillar abscess identified at tonsillectomy has been reported to be between 1.9% and 24%. The diagnosis of bilateral peritonsil

In 17 illustrations, a modification of Fujita's technique of UPPP is presented. This operation consists of excising redundant velar tissues from the free margin of the soft palate, tonsillar pillars, and uvula without reducing the muscles of the velum. In addition, the uvula muscle is almost completely preserved. Seventy patients, with habitual snoring or with obstructive sleep apnea syndrome, who underwent this surgery were followed-up from six to 24 months and did not show any palatal insufficiency with air and liquid escape through the nose. Additional remarks are made concerning the diffic

The anatomical, physiological, and behavioral evidence for the involvement of three regions of the cerebellum in oculomotor behavior is reviewed here: (1) the oculomotor vermis and paravermis of lobules V, IV, and VII; (2) the uvula and nodulus; (3) flocculus and ventral paraflocculus. No region of the cerebellum controls eye movements exclusively, but each receives sensory information relevant for the control of multiple systems. An analysis of the microcircuitry suggests how sagittal climbing fiber zones bring visual information to the oculomotor vermis; convey vestibular information to the
