Cartilage changes link retrognathic mandibular growth to TMJ disc displacement in a rabbit model.

Recent experimental research demonstrated that non-reducing temporomandibular joint (TMJ) disc displacement in growing rabbits impaired mandibular growth. TMJ disc displacement is also shown to induce histological changes of the condylar cartilage. The authors hypothesized that the severity of these changes would correlate to the magnitude of mandibular growth. Bilateral non-reducing TMJ disc displacement was surgically created in 10 growing New Zealand White rabbits. Ten additional rabbits constituted a sham operated control group. Aided by tantalum implants, growth was cephalometrically dete

Virtual restoration of anatomic jaw relationship to obtain a precise 3D model for total joint prosthesis construction for treatment of TMJ ankylosis with open bite.

Temporomandibular joint (TMJ) reconstruction with a TMJ Concepts total joint prosthesis (TMJ Concepts, Ventura, USA) requires a precise 3D model of the jaws in centric occlusion. The authors present a virtual procedure for repositioning the lower jaw in centric occlusion to obtain a precise stereolithographic model for TMJ reconstruction using a custom-made total joint prosthesis in a case of TMJ ankylosis and anterior open bite.

Analysis of microarchitectural changes in a mouse temporomandibular joint osteoarthritis model.

OBJECTIVE: Little is known about the natural progression of the disease process of temporomandibular joint (TMJ) osteoarthritis (OA), which affects approximately 1% of the US population. The goal of this study was to examine the early microarchitectural and molecular changes in the condylar cartilage and subchondral bone in biglycan/fibromodulin (Bgn/Fmod) double-deficient mice, which develop TMJ-OA at 6 months. METHODS: TMJs from 3-month-old (n=44) and 9-month-old (n=52) wild-type (WT n=46) and Bgn/Fmod (n=50) double-deficient mice were evaluated. Micro-CT analysis of the subchondral bone (n

Histologic changes associated with experimental partial anterior disc displacement in the rabbit temporomandibular joint.

AIMS: To correlate histologic changes with the stress developed by various disc interferences via a model of partial anterior disc displacement in the rabbit temporomandibular joint (TMJ). METHODS: Eighteen male New Zealand rabbits were operated on to expose the temporomandibular disc without severing its attachments. A suture was passed around the lateral part of the disc over the condylar attachments and the 2 strands were fixed in the orbital cavity. In 9 rabbits, a resorbable suture was used to secure the disc displacement. In the other 9, a nonresorbable suture was fixed with a nickel-ti

Craniofacial asymmetry and temporomandibular joint internal derangement in female adolescents: a posteroanterior cephalometric study.

Unilateral or bilateral pathology of the osseous components of the temporomandibular joint (TMJ) can result in pronounced facial asymmetry because of dissimilar size and shape of the right and left sides of the mandible. To date, it is unknown whether abnormalities of the soft tissues of the TMJ are associated with greater than normal craniofacial asymmetry. In this study, we investigated the amount of craniofacial asymmetry in female orthodontic patients with unilateral or bilateral TMJ internal derangement (TMJ ID) relative to the amount in female patients without TMJ ID. The total sample co

Does Segmental Mandibulectomy Involving Critical Functional Sites Affect Quality of Life?

PURPOSE: Segmental mandibulectomy (SM) is a morbid procedure, often indicated in the treatment of oral cavity pathology; however, the effect on quality of life resulting from resection of specific mandibular subsites has not previously been investigated. This study's primary aim was to evaluate differences in Health-Related Quality of Life (HRQoL) among patients who underwent segmental mandibulectomy with condylectomy (SMc+) or not (SMc-), and secondarily among patients who underwent SM with symphyseal resection (SMs+) or not (SMs-). METHODS: A single-centre cross-sectional study was performe

Hemifacial microsomia: use of the OMENS-Plus classification at the Royal Children's Hospital of Melbourne.

Hemifacial microsomia is the most common facial congenital disability after cleft lip and palate, but as yet its pathogenesis remains unknown. Clinical classification systems have evolved over the last 30 years from those classifying only single components of the disorder, to those classifying according to the combination of deformities, to the most recent systems that grade each anatomical component separately, such as the Orbit, Mandible, Ear, Nerve, and Soft tissue (OMENS) system. The aim of the present study was to review the classification of patients with hemifacial microsomia treated by

Pseudogout in the temporomandibular joint with imaging, arthroscopic, operative, and pathologic findings. Report of an unusual case.

The authors present a case of a 60-year-old woman with a destructive painful condition in the right temporomandibular joint (TMJ) that proved to be calcium pyrophosphate crystal deposits at subsequent biopsy examination. The patient presented with the chief complaints of pain and limitation that had not resolved with splint therapy, medications, and habit control. Magnetic resonance imaging studies showed internal derangement without reduction. Right TMJ arthroscopy with manipulation of the jaw under anesthesia showed unique findings of fronds of synovial tissue in the posterior joint space an

Celecoxib exerts protective effects on extracellular matrix metabolism of mandibular condylar chondrocytes under excessive mechanical stress.

OBJECTIVE: Excessive mechanical stress is considered a major cause of temporomandibular joint osteoarthritis (TMJ-OA). High magnitude cyclic tensile strain (CTS) up-regulates pro-inflammatory cytokines and matrix metalloproteinases (MMPs) in chondrocytes, while selective cyclooxygenase (COX)-2 inhibition has been shown to be beneficial to cytokine-induced cartilage damage. However, the effect of selective COX-2 inhibitors on mechanically stimulated chondrocytes remains unclear. This study evaluated the effect of celecoxib, a selective COX-2 inhibitor, on extracellular matrix (ECM) metabolism o

Effects of enzymatic degradation after loading in temporomandibular joint.

Synovial fluid of the joint decreases friction between the cartilage surfaces and reduces cartilage wear during articulation. Characteristic changes of synovial fluid have been shown in patients with osteoarthritis (OA) in the temporomandibular joint (TMJ). OA is generally considered to be induced by excessive mechanical stress. However, whether the changes in synovial fluid precede the mechanical overloading or vice versa remains unclear. In the present study, our purpose was to examine if the breakdown of joint lubrication affects the frictional properties of mandibular condylar cartilage an

Comparative evaluation of condylar position in symptomatic (TMJ dysfunction) and asymptomatic individuals.

CONTEXT: The etiopathogenesis of temporomandibular joint (TMJ) disorders has been attributed to multiple factors and, while the importance of centric relation (CR) has been acknowledged in dental literature, the role of condylar position and CR-CO (CO-centric occlusion) discrepancy in TMJ dysfunction (TMD) has been a source of controversy. AIM: To establish the relationship between condyle position and TMD. SETTING AND DESIGN: This was a case-control study to evaluate condylar displacement and interarch CR-CO discrepancy in symptomatic subjects (with TMD) and compare them with asymptomatic s

Maxillofacial trauma--developments, innovations and controversies.

Despite seat belt and alcohol legislation, craniofacial trauma still remains a common health problem and significant workload in many maxillofacial units. Although management has evolved considerably from "wiring teeth together", complex fractures can still result in cosmetic and functional deformity. Today's challenge is to consistently restore patients back to their pre-injury form and function-but this is not always possible. Greater understanding and developments have significantly improved outcomes, although controversy still exists in some areas. This review outlines some of these topics

Three-dimensional analysis system for orthognathic surgery patients with jaw deformities.

INTRODUCTION: Traditionally, lateral and frontal cephalograms are used with facial photographs to evaluate a patient's maxillofacial skeletal and facial soft-tissue morphology. However, the enlargement and distortion of 2-dimensional radiography made it difficult to accurately conceptualize the patient's anatomy. The purpose of this article was to introduce a new method for comparing 3-dimensional (3D) standard values of the maxillofacial skeletal and facial soft-tissue morphology before and after orthognathic surgery. METHODS: Normative 3D standard values of the maxillofacial skeletal and fa

A newer approach in positioning teeth for dental prosthetics using lateral cephalometric, trans-cranial radiographs, and the Denar-Witzig articulator: a case of hypodontia in an adolescent patient.

Traditionally in full denture prosthetics, anterior teeth are set on the models, independent of the effects on the face. More enlightened dentists, will then adjust the wax-up for speech, and some effects on the lips. Consideration is infrequently given to restoring the face by repositioning the teeth and mandible. Rarely is thought given to the effects of occlusion on the posture of the body. This report uses several techniques to restore facial esthetics and body posture. The techniques used include a cephalometric radiograph, transcranial radiographs, an articulator that has an adjustable "

Location of the glenoid fossa after a period of unilateral masticatory function in young rabbits.

Changes in glenoid fossa position and skull morphology after a period of unilateral masticatory function were studied. The right-side maxillary and mandibular molars in twenty-seven 10-day-old rabbits were ground down under general anaesthesia. The procedure was repeated twice a week, until the rabbits were 50 days old. Fourteen rabbits were then killed and 13 left to grow to age 100 days. Nine 50-day-old and sixteen 100-day-old rabbits with unmodified occlusions served as controls. Three-dimensional measurements were made using a machine-vision technique and a video-imaging camera. Worsening of pre-existing TMJ dysfunction following sagittal split osteotomy: a study of three cases.

When patients seeking treatment for malocclusion also suffer from temporomandibular joint (TMJ) disorders, it is hard to predict the result of simultaneous treatment of both conditions, or to plan for its different goals, because of unpredictable changes in the relationship between the disk, the fossa and the condylar head. Prediction is harder in cases of presurgical TMJ hypomobility, especially those with adhesion in the upper TMJ compartment. Authors differ widely on the likely effect of orthognathic surgery on TMJ disorders. This paper reports three cases in which TMJ disorders worsened af

Morphological variations and differences of the mandible and midfacial skeleton across different sagittal skeletal patterns and sexes: a three-dimensional geometric morphometric analysis.

BACKGROUND: Maxillofacial morphological analysis is essential for orthodontic diagnosis, treatment planning, and outcome evaluation. However, previous two-dimensional or landmark-based studies can hardly comprehensively analyze three-dimensional morphology. Therefore, this study aimed to analyze morphological variations and differences of the mandible and midfacial skeleton across different sagittal skeletal patterns and sexes using three-dimensional geometric morphometrics, while also providing novel perspectives and methodologies for morphological analysis in orthodontics. METHODS: The midf
