Association between VA providers' reported use of care strategies and emergence delirium: a national retrospective cross-sectional survey.

BACKGROUND: Emergence delirium (ED) is a short-term behavioral condition associated with risk of harm that may occur when a patient awakens from anesthesia. A national survey was conducted to evaluate our hypothesis that Veterans Health Administration (VHA) anesthesia providers who reported routinely screening patients for ED risk and selecting medication strategies associated with lower ED risk would be less likely to report estimated ED occurrence. METHODS: A retrospective cross-sectional self-report survey was designed by staff at Veterans Affairs' Pittsburgh Healthcare System (PHS) and la

Concurrent activation of multiple programmed cell death pathways predicts 28-day mortality in pediatric sepsis: a retrospective PICU cohort study with differential sedative agent use between survivors and non-survivors.

OBJECTIVE: To systematically characterize the expression profiles of biomarkers associated with multiple programmed cell death (PCD) pathways-including apoptosis, pyroptosis, necroptosis, and ferroptosis-in pediatric intensive care unit (PICU) patients with sepsis, elucidate their associations with organ dysfunction severity and clinical prognosis, and identify independent predictors of 28-day all-cause mortality in children. METHODS: A retrospective cohort study was conducted on 208 pediatric sepsis patients (aged 1 month to 18 years) admitted to the PICU of Jingzhou First People's Hospital

The impact of sedation depth on the occurrence of delirium and prognosis in intensive care unit patients: a meta-analysis.

BACKGROUND: Delirium and poor clinical prognoses are prevalent among ICU patients, particularly those requiring mechanical ventilation. While sedation is a cornerstone of intensive care, the impact of its depth remains controversial; deep sedation is often linked to adverse outcomes, whereas light sedation may improve recovery trajectories. We conducted a systematic review and meta-analysis to clarify the association between sedation depth and delirium incidence, mortality, and ICU length of stay (LOS). METHODS: Following PRISMA 2020 guidelines and PROSPERO registration (CRD420251054377), we

Impact of ideal vs actual body weight on analgesic and sedative requirements in critically ill patients with obesity.

PURPOSE: Analgosedation is the standard of care for managing mechanically ventilated critically ill patients. Most sedative medications are lipophilic, with increased volume of distribution in patients with obesity; however, data about dosing these drugs in critically ill patients with obesity are limited. In 2022, our institution changed to an ideal body weight (IBW) dosing strategy for all weight-based continuous infusions of analgesia and sedation agents, including fentanyl, ketamine, propofol, and dexmedetomidine. This study evaluated whether utilizing IBW dosing for critically ill patient

Elevated TDP-43 serum levels associated with postoperative delirium following major cardiac surgery.

BACKGROUND: Postoperative delirium is a recurring complication among vulnerable patients undergoing major cardiac surgery. While delirium has been associated with prodromal dementia, there is minimal evidence to support the causality of this nuanced relationship. Clarification as to how postoperative delirium might lead to neurodegenerative dementias, perhaps through evidence of contemporaneous biomarkers, would heighten the plausibility of a causal correlation. TAR DNA-binding protein 43 (TDP-43), a nuclear protein essential for transcriptional events, has been linked to pathological aggregat

Admission rate for bronchiolitis of newborns and infants in Italian neonatal intensive care units in 2021: a survey of the Italian Society of Neonatology - Intensive Care of Early Childhood Study Group.

BACKGROUND: The shortage of Pediatric Intensive Care Unit (PICU) beds among some Italian regions is a major concern, especially during epidemics. During respiratory syncytial virus (RSV) bronchiolitis peak, Neonatal Intensive Care Units (NICU) often admit infants and toddlers requiring advanced respiratory support. We conducted a survey to quantify children hospitalized for RSV bronchiolitis in NICU in 2021 and to examine the adherence to treatment guidelines. METHODS: Early Childhood Intensive Care Working Group of the Italian Society of Neonatology (SIN) conducted a survey distributed to SI

Effect of opioid-free anaesthesia on postoperative delirium in elderly patients undergoing gastrointestinal surgery: study protocol for a single-centre, prospective, randomized controlled trial.

BACKGROUND: Postoperative delirium (POD) is a common acute and transient form of brain dysfunction in elderly patients following surgery that can lead to serious adverse clinical outcomes and even death. Although existing studies have preliminarily investigated the effects of opioid-free anaesthesia (OFA) on POD, high-quality evidence on these effects for elderly patients undergoing gastrointestinal surgery remains limited. This study aims to investigate the effects of OFA on the development of POD in elderly patients following gastrointestinal surgery. METHODS AND ANALYSIS: This single-centr

Use of opioids and opioid alternatives during general anesthesia: a pan-Canadian survey among anesthesiologists.

PURPOSE: While there is limited patient-centred evidence (i.e., evidence that is important for patients and end-users) to inform the use of pharmacologic opioid minimization strategies (i.e., the use of opioid alternatives) for adult surgical patients requiring general anesthesia, such strategies are increasingly being adopted into practice. Our objectives were to describe anesthesiologists' beliefs regarding intraoperative opioid minimizing strategies use and utility, and to explore important clinical decision-making factors. METHODS: We conducted a pan-Canadian web-based survey of anesthesi

The Management and Prevention of Delirium in Elderly Patients Hospitalised in Intensive Care Units: A Systematic Review.

BACKGROUND: Delirium or an acute confusional state (ACS) is characterised as being a frequent and complex hospital complication in older adult patients, which can affect their level of independence and increase patient morbidity and mortality. Critically ill patients in the intensive care unit (ICU) frequently develop ICU delirium, leading to longer hospital and ICU stays, increased mortality and long-term impairment. OBJECTIVES: This review aims to assess existing evidence of interventions that can be considered effective for the management and prevention of delirium in ICUs, reducing short-

Delirium in Adults With COVID-19-Related Acute Respiratory Distress Syndrome: Comparison With Other Etiologies.

BACKGROUND AND OBJECTIVES: Neurologic complications have been associated with COVID-19, including delirium. Such complications have been reported to be frequent among intensive care unit (ICU)-admitted patients. We hypothesized that the rate of neurologic complications would be higher in COVID-19 associated acute respiratory distress syndrome (ARDS) than those who develop ARDS from a different cause. METHODS: We conducted a retrospective cohort study in the adult ICU of Lausanne University Hospital, including all consecutive patients fulfilling the Berlin criteria for ARDS hospitalized betwee

Bradycardia in Patients With COVID-19: A Calm Before the Storm?

Cardiac manifestations of coronavirus disease 19 (COVID-19), including arrhythmia, have been described in the literature. However, to our knowledge, association of COVID-19 with bradycardia has not been reported. This case study describes sinus bradycardia as a potential manifestation of COVID-19. This is a retrospective case series of four patients with laboratory-confirmed severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, admitted to St. Luke's University Health Network ICU between 24 March 2020 and 5 April 2020. Medical records of these patients were reviewed using the

Removal of a vertebral metastatic tumor compressing the spinal nerve roots via a single-port, transforaminal, endoscopic approach under monitored anesthesia care.

Spinal cord or nerve root compression from an epidural metastasis occurs in 5-10% of patients with cancer and in up to 40% of patients with preexisting nonspinal bone metastases. Most metastatic spine diseases arise from the vertebral column, with the posterior half of the vertebral body being the most common initial focus, and/or the paravertebral region, tracking along the spinal nerves to enter the spinal column via the intervertebral foramina. An 82-year-old man diagnosed with sigmoid colon cancer and liver metastases experienced intractable pain described as being like an electric shock o

POFA trial study protocol: a multicentre, double-blind, randomised, controlled clinical trial comparing opioid-free versus opioid anaesthesia on postoperative opioid-related adverse events after major or intermediate non-cardiac surgery.

INTRODUCTION: Reducing opioid consumption during and after surgery has been recommended for more than 10 years. Opioid-free anaesthesia (OFA) is a multimodal anaesthesia associating hypnotics, NMDA antagonists, local anaesthetics, anti-inflammatory drugs and α-2 agonists. Proofs of the effect of OFA on reducing opioid-related adverse effects after major or intermediate non-cardiac surgery are still scarce. We hypothesised that the reduced opioid consumption allowed by OFA compared with standard of care will be associated with a reduction of postoperative opioid-related adverse events. METHODS
