Observation, Aspiration, or Tube Thoracostomy for Primary Spontaneous Pneumothorax: A Systematic Review, Meta-Analysis, and Cost-Utility Analysis.

BACKGROUND: Primary spontaneous pneumothorax (PSP) has several commonly used management strategies: observation, aspiration, and chest tube placement. Economic modelling of pooled data comparing techniques has not been performed. RESEARCH QUESTION: Based on studies from the past 20 years, which approach to management of PSP delivers the highest utility? STUDY DESIGN AND METHODS: A systematic review of PSP management strategies (observation, aspiration, or chest tube placement) included in the Medline and EMBASE databases from January 1, 2000, through April 10, 2020, was conducted. A Cost-Benefit Analysis of Osseointegrated Prostheses for Lower Limb Amputees in the US Health Care System.

BACKGROUND: Patients with transfemoral and transtibial amputations generally rely on socket-suspended (SS) prostheses for ambulation. The use of these aids can be complicated by poor fit, leading to tissue damage, pain at the socket-limb interface, and inability to ambulate. Osseointegrated implants (OIs) directly anchor a prosthesis to the patient's residual limb, eliminating these issues. However, they require customized components and additional surgeries. The purpose of this study was to conduct the first cost-benefit analysis of OI prostheses compared to SS prostheses for lower limb amput

Tree-based data mining for safety assessment of first COVID-19 booster doses in the Vaccine Safety Datalink.

BACKGROUND: The Centers for Disease Control and Prevention's Vaccine Safety Datalink (VSD) has been performing safety surveillance for COVID-19 vaccines since their earliest authorization in the United States. Complementing its real-time surveillance for pre-specified health outcomes using pre-specified risk intervals, the VSD conducts tree-based data-mining to look for clustering of a broad range of health outcomes after COVID-19 vaccination. This study's objective was to use this untargeted, hypothesis-generating approach to assess the safety of first booster doses of Pfizer-BioNTech (BNT162

Attribution of country level foodborne disease to food group and food types in three African countries: Conclusions from a structured expert judgment study.

BACKGROUND: According to the World Health Organization, 600 million cases of foodborne disease occurred in 2010. To inform risk management strategies aimed at reducing this burden, attribution to specific foods is necessary. OBJECTIVE: We present attribution estimates for foodborne pathogens (Campylobacter spp., enterotoxigenic Escherichia coli (ETEC), Shiga-toxin producing E. coli, nontyphoidal Salmonella enterica, Cryptosporidium spp., Brucella spp., and Mycobacterium bovis) in three African countries (Burkina Faso, Ethiopia, Rwanda) to support risk assessment and cost-benefit analysis in t

Model-based assessment of the safety of community interventions with primaquine in sub-Saharan Africa.

BACKGROUND: Single low-dose primaquine (SLD-PQ) is recommended in combination with artemisinin-based combination therapy to reduce Plasmodium falciparum transmission in areas threatened by artemisinin resistance or aiming for malaria elimination. SLD-PQ may be beneficial in mass drug administration (MDA) campaigns to prevent malaria transmission but uptake is limited by concerns of hemolysis in glucose-6-phosphate dehydrogenase (G6PD)-deficient individuals. The aim of this study was to improve the evidence on the safety of MDA with SLD-PQ in a sub-Saharan African setting. METHODS: A nonlinear

Cost-effectiveness of couples' voluntary HIV counselling and testing in six African countries: a modelling study guided by an HIV prevention cascade framework.

INTRODUCTION: Couples' voluntary HIV counselling and testing (CVCT) is a high-impact HIV prevention intervention in Rwanda and Zambia. Our objective was to model the cost-per-HIV infection averted by CVCT in six African countries guided by an HIV prevention cascade framework. The HIV prevention cascade as yet to be applied to evaluating CVCT effectiveness or cost-effectiveness. METHODS: We defined a priority population for CVCT in Africa as heterosexual adults in stable couples. Based on our previous experience nationalizing CVCT in Rwanda and scaling-up CVCT in 73 clinics in Zambia, we estim

Environmental determinants of distribution of freshwater snails and trematode infection in the Omo Gibe River Basin, southwest Ethiopia.

BACKGROUND: Determination of infection rates of snail populations is one of the basic tools for epidemiological studies of snail borne diseases. In this study, we opted to determine the trematode infection of freshwater snails in the Omo-Gibe River Basin, southwest Ethiopia. METHODS: We collected snail samples from 130 observation sites in lakes, wetlands, rivers, reservoirs and irrigation canals surveyed during the dry season (March to May) in 2016. The snail samples were examined for trematode infections by cercarial shedding immediately after collection. Habitat conditions, water quality,

Spatial correlation of linear energy transfer and relative biological effectiveness with suspected treatment-related toxicities following proton therapy for intracranial tumors.

PURPOSE: The enhanced relative biological effectiveness (RBE) at the end of the proton range might increase the risk of radiation-induced toxicities. This is of special concern for intracranial treatments where several critical organs at risk (OARs) surround the tumor. In the light of this, a retrospective analysis of dose-averaged linear energy transfer (LETd ) and RBE-weighted dose (DRBE ) distributions was conducted for three clinical cases with suspected treatment-related toxicities following intracranial proton therapy. Alternative treatment strategies aiming to reduce toxicity risks are

Propulsive Power in Cross-Country Skiing: Application and Limitations of a Novel Wearable Sensor-Based Method During Roller Skiing.

Cross-country skiing is an endurance sport that requires extremely high maximal aerobic power. Due to downhill sections where the athletes can recover, skiers must also have the ability to perform repeated efforts where metabolic power substantially exceeds maximal aerobic power. Since the duration of these supra-aerobic efforts is often in the order of seconds, heart rate, and pulmonary VO2 do not adequately reflect instantaneous metabolic power. Propulsive power (P prop) is an alternative parameter that can be used to estimate metabolic power, but the validity of such calculations during cro

Operation of a brain-computer interface walking simulator for individuals with spinal cord injury.

BACKGROUND: Spinal cord injury (SCI) can leave the affected individuals with paraparesis or paraplegia, thus rendering them unable to ambulate. Since there are currently no restorative treatments for this population, novel approaches such as brain-controlled prostheses have been sought. Our recent studies show that a brain-computer interface (BCI) can be used to control ambulation within a virtual reality environment (VRE), suggesting that a BCI-controlled lower extremity prosthesis for ambulation may be feasible. However, the operability of our BCI has not yet been tested in a SCI population.

Modelling the effect of function and disease activity on costs and quality of life in rheumatoid arthritis.

OBJECTIVE: When treatments with the potential to change the natural history of a disease are introduced, their longer-term effect on costs and quality of life (utility) has to be estimated using economic models. However, to remain useful tools, models must be updated when new information becomes available. Our earlier models in rheumatoid arthritis (RA) have been based on functional status, but it has recently been shown that disease activity might have an independent effect on utility. The objective of this study was to improve the model by incorporating the effect of a subjective measure of

Experimental and Monte Carlo Insights into the Biofilm Control of Klebsiella pneumoniae Using Green-Synthesized Ag/Ni(OH)2/Clove Nanocomposites.

One of the most significant challenges with disease control in humans and animals is biofilm formation by Klebsiella pneumoniae (K. pneumoniae). The current study investigated the prevalence of K. pneumoniae in fecal and milk samples from cattle and humans, along with their antimicrobial susceptibility patterns and biofilm formation, using conventional methods. In addition, the efficacy of Ag/Ni(OH)2/Clove and Ag/Ni(OH)2/C nanocomposites was comparatively evaluated to combat resistance and biofilm-associated traits. K. pneumoniae was recovered from 48.0%, 38.0% and 43.3% of the fecal, milk and

Ethical evaluation of urology specialists' knowledge and attitudes on the application of medical (chemical) castration for sexual offenders: a perspective from Turkey.

BACKGROUND: Court-ordered medical (chemical) castration has pre-occupied Türkiye's legal and medical communities since the short-lived 2016 by-law that named urologists as key implementers. How do the very physicians expected to administer the intervention actually view it? This nationwide survey probes urology specialists' knowledge, ethical reasoning and practical attitudes toward medical castration for sexual offenders. METHODS: A nationwide, cross-sectional online survey was conducted between April 2020 and October 2024. A 53-item questionnaire covering sociodemographic data, knowledge, e

Toward the automation of family semantic grids: a pilot exploration of text mining for evidence-informed systemic-relational psychotherapy.

INTRODUCTION: Retracing the theme of what can legitimately be considered "evidence" in psychotherapy, the present work shows the application of an ad hoc, rule-based text mining program specifically designed to analyze therapy session transcripts. Developed within a systemic-relational framework, and grounded in Semantic Polarities Theory, the program works through a hybrid human-in-the-loop approach and provides data for subsequent descriptive and inferential analyses on meaning transformation starting from consultation sessions. PARTICIPANTS: The present study examines the clinical case of

Patterns and prevalence of ophthalmic self-medication in Bulgaria: results from a cross-sectional survey.

BACKGROUND: Ophthalmic self-medication, defined as the use of over-the-counter eye medications without prescription, represents a significant public health concern due to potential complications such as delayed diagnosis, antimicrobial resistance, and vision impairment. Despite its global prevalence, data on this practice in Bulgaria remain limited. This study aimed to comprehensively assess the prevalence, patterns, determinants, and self-reported outcomes of ophthalmic self-medication among adults in Bulgaria. METHODS: A cross-sectional online survey was conducted between March and May 2025

Cost-utility analysis of simultaneous initiation of finerenone and empagliflozin for type 2 diabetes mellitus complicated with chronic kidney disease.

BACKGROUND: Type 2 diabetes mellitus complicated with chronic kidney disease (T2DM-CKD) is a common and severe complication of T2DM that predisposes patients to end-stage renal disease (ESRD), substantially increasing mortality risk and long-term medical economic burden. Currently, SGLT-2 inhibitors are used as the foundational treatment regimen in clinical practice. As validated by the global multicenter randomized controlled CONFIDENCE trial, simultaneous initiation of finerenone plus an SGLT-2 inhibitor can further enhance renal protection and delay kidney disease progression. Despite these
