The spectrum of plasma cell neoplasia in oral pathology.

Plasma cell tumors are lymphoid neoplastic proliferations of B cells that may be classified as multiple myeloma (MM), solitary bone plasmacytoma (SBP) and extramedullary plasmacytoma (PEM). These types of neoplasia are typically found in adults and may occur as disseminated tumors of the bone marrow or in some cases as solitary bone or extramedullary tumors. Most SBP eventually develop into MM, whereas only 30% of the PEM do so. Oral manifestations in the form of oral and maxillofacial lesions are often the first sign of the disease. Treatment of these neoplastic tumors varies depending on the

Intraoral contact allergy to materials used in dental practice. A critical review.

Intraoral contact allergy reactions are clinically poorly differentiated and understood, and are not very commonly described in the specialized literature. Although such reactions appear to be scantly relevant, evidence suggests that they may be more frequent than previously believed. No single or specific clinical picture of intraoral contact allergy exists, though lichenoid reactions appear to be the most common manifestations. Epicutaneous patch testing, together with the clinical manifestations, constitute the most widely used diagnostic approach in such situations. Metals used in dental p

Lingual thyroid: a clinical case.

Lingual thyroid is an abnormal formation appearing as the result of a deficient descent during embryological development of the thyroid gland through the thyroglossal duct to its normal pretracheal location. The lesion consists of a tumor mass of thyroid tissue located at the base of the tongue, in the region of the foramen caecum linguae. The size can vary from a few millimeters to several centimeters in diameter. More than 400 cases of lingual thyroid have been documented in the literature to date. Lingual thyroid has been identified in 10% of the tongues examined in some autopsy series. Its

Tuberous sclerosis: presentation of a clinical case with oral manifestations.

Tuberous sclerosis (TS) is a genetic disorder affecting multiple body systems, and resulting from alterations in cell differentiation and proliferation. The disease is characterized by the development of benign hamartomatous tumors: neurofibromas and angiofibromas, located in the skin, central nervous system, mucosas and other organs. Abnormal neural cell migration plays an important role in the neurological dysfunctions found in TS, the predominant features being mental retardation, seizures and behavioral disorders. The condition is produced by mutations in genes TSC1 of chromosome 9q34 and

Kindler syndrome: a clinical case.

Kindler's syndrome is a congenital disorder with simultaneous clinical characteristics of two congenital diseases: dystrophic epidermolysis bullosa and congenital poikiloderma. A review is made of the literature, with the presentation of the case of a 51-year-old woman who after a number of years of implant therapy for oral rehabilitation (total upper and partial lower arch edentulous status) developed xerostomia and a series of whitish and hyperkeratotic lesions associated with other atrophic erythematous lesions lining the entire oral mucosa. The clinical case and treatment are described, wi

Osseous choristoma of the oral soft tissue. Case report.

Oral osseous choristoma is a rare developmental alteration, their principal localization is nearly to base of the tongue (foramen caecum). The oral mucosa localization of osseous choristoma is extremely rare. At date only 10 cases of oral mucosa osseous choristoma had been reported. In the present paper we reported a new case of oral mucosa osseous choristoma in a Klippel-Feil syndrome patient. A review of available literature was made. We presented a 28 years old female patient who showed into the right oral mucosa, a hard, mobile, and asymptomatic mass, with minimum 4 years of evolution. The

Etiopathogenic analysis of the caries on three patients with Noonan Syndrome.

INTRODUCTION: Noonan syndrome is a genetic disease frequently characterized by short height, winged neck and a thoracic defect, among other alterations. However, there are no reports that mention oral health complications. In this article three children are presented with this syndrome in which multiple caries are observed, determining in each case which factors increase their susceptibility to caries. CLINICAL CASES: 1. A 4 year old girl who suffered neonatal sepsis, pyelocalyceal ecstasia, congenital hypertrophic pyloric stenosis, cardiopathy, hypoacusia and psychomotor delay. Presented mul

Orofacial neurogenic pain and maxillofacial ischemic osteonecrosis. A review.

Cavitary alveolar osteopathy was described as an oral disorder of infectious origin characterized by the presence of osteopathic alveolar cavity lesions of significant size though radiologically undetectable and secondary to dental extractions for chronic infectious processes of the alveolar bone of the jaws. Such cavitary alveolar osteopathy has been implicated as a common cause in the origin of idiopathic trigeminal neuralgia and atypical facial pain. The concept of cavitary alveolar osteopathy caused by ischemic necrosis of alveolar bone was introduced in 1992. Recent coagulation studies ha

Idiopathic external root resorption associated to hypercalciuria.

Although external root resorption (ERR) is a physiological process in deciduous dentition, it is very infrequent in permanent dentition - where the phenomenon is related to the existence of inadequate occlusal forces, periodontal pathology and microtraumatisms, etc. However, in many cases root resorption cannot be attributed to any concrete cause; such cases are defined as idiopathic external root resorption (IERR). Epidemiological studies have found that the underlying cause can only be established in 5% of all ERR. The present study describes three cases of IERR with different degrees of inv

Lesions of the oral mucosa in cocaine users who apply the drug topically.

The use and abuse of cocaine is increasingly frequent in many countries, and the associated problems are increasingly evident. The effects of cocaine in the oral cavity vary depending on the form used and the route of self-administration. In the present study we describe the lesions observed in four patients with a history of topical self-application of cocaine to the oral and/or nasal mucosa, with the aim of relieving pain produced by cocaine-induced cluster headache. In three of the four patients this practice has led to erythematous lesions, while the remaining patient showed gingival reces

Differential diagnosis and therapeutic approach to periapical cysts in daily dental practice.

The diagnosis and therapeutic approach to periapical cysts is an extremely controversial concern for dentists. Furthermore, as this complaint represents the most frequent cystic lesion of the maxilla, together with the fact that its differential diagnosis with chronic apical periodontitis presents special difficulty, the question takes on even greater importance. The purpose of this article is to assess the validity of the various diagnostic techniques used to differentiate between both pathologies and make a critical analysis of the controversy surrounding the therapeutic approach to suspecte

Odontogenic cysts. Analysis of 856 cases.

Odontogenic cysts (OC) are one of the main causes of jaw destruction. Information about these lesions in the Mexican population is scant. And for this reason the purpose of this work is to describe the frequency of the different varieties of OC recorded in two oral pathology services in Mexico City. As well as to compare the findings with those previously reported in other studies and to analyze the association of these lesions with the gender of the affected patients and the type of oral pathology service. There were a total of 856 OC; of these, 449 (52.5%) occurred in men, 403 in women (47%)

Prevalence of oral lesions in the 6-year-old pediatric population of Oviedo (Spain).

OBJECTIVE: The present study investigates the prevalence of oral diseases in children in the city of Oviedo (Spain). METHODS: A representative sample was selected from among the 6-year-old children in Oviedo (n= 786), involving a protocol developed for pediatric oral diseases. RESULTS: A total of 344 lesions were detected in 243 children. The most common disorder was saburral tongue (16,02%), followed by traumatisms (12.17%) and geographic tongue (4.48%). The prevalence of aphthous stomatitis was 2.24% and herpes labialis was 1,6%. Ankyloglossia was observed in 2.08% of cases, and a hypertro

Kikuchi-Fujimoto disease. A case report.

Histologically, Kikuchi-Fujimoto disease (KFD) is a necrotising, self-limiting lymphadenitis. It typically affects the lymph nodes of the neck and it is often accompanied by a high fever. The present paper documents the case of a 28-year old male carrier of KFD. From the x-ray images, the KFD clinically appeared to be a submandibular salivary gland tumor. An ultrasound study led us to believe that it was more likely a branchial cyst. Malignant lymphoma can be mistaken for KFD, thus the diagnosis must be precise and the differentiation is crucial.

Peripheral ossifying fibroma. Report of a case and review of the literature.

Peripheral ossifying fibroma, a reactive gingival disorder known under the generic term of epulis, is widely considered to originate from the cells of the periodontal ligament. A 39-year-old woman presented with a well circumscribed, hard pedunculated exophytic tumor measuring 2 cm in size, without ulceration. The lesion had developed three months before in the interdental space, between the lower left lateral incisor and canine. No radiological signs of involvement of the alveolar ridge were observed, and the clinically manifest diastema was confirmed. The histological study of the specimen a

Recurrent infection of a complex odontoma following eruption in the mouth.

Odontomas are the most common odontogenic tumors, and are classified as either compound or complex lesions. Although its underlying etiology remains unclear, a number of factors appear to be involved (e.g., prior trauma). The lesions are generally diagnosed in the second decade of life, often in the context of routine dental X-ray examinations. A complex odontoma is described in a 22-year-old woman referring discomfort due to overinfection following its aperture into the oral cavity in the distal alveolar region of 2.6. Initial treatment consisted of antibiotics and antiinflammatory drugs. Com

Trigeminal neuralgia associated to basilar artery dolichoectasia.

A case of trigeminal neuralgia attributed to basilar artery dolichoectasia is described. Underlying vascular malformations and tumors of the cerebellopontine angle can be detected in patients with trigeminal neuralgia by means of noninvasive techniques such as computed tomography and magnetic resonance imaging; such diagnostic tools are therefore indicated in patients with trigeminal neuralgia to discard possible central nervous disorders.
