Long-Term Outcomes of Reverse Total Shoulder Arthroplasty: A Follow-up of a Previous Study.

BACKGROUND: Despite the increasing numbers of reverse total shoulder arthroplasty (RTSA) procedures, the long-term results have been rarely reported. We previously reported early outcomes of a cohort of patients treated with a Grammont-style RTSA. The purpose of this study was to evaluate the outcomes after a minimum of 10 years, and to document prosthetic survival and complications. METHODS: Clinical outcome assessment was based on the absolute and relative Constant scores and the active range of motion. Radiographic evaluations of scapular notching, tuberosity osteolysis, and periprosthetic

Arthroscopic-Assisted Latissimus Dorsi Transfer for the Management of Irreparable Rotator Cuff Tears: Short-Term Results.

BACKGROUND: Irreparable rotator cuff tears associated with shoulder functional impairment represent a challenge, especially in young and active patients. Latissimus dorsi muscle-tendon transfer is performed to replace the irreversibly lost contractile elements in patients with irreparable tears of the posterosuperior aspect of the rotator cuff. METHODS: From 2008 to 2010, we enrolled twenty-seven patients (mean age, sixty years; range, forty-six to sixty-seven years) with irreparable, full-thickness rotator cuff tears involving at least two tendons who underwent arthroscopic-assisted latissim

The diagnosis and prognosis of impingement syndrome in the shoulder with using quantitative SPECT assessment: a prospective study of 73 patients and 24 volunteers.

BACKGROUND: Diagnosing impingement syndrome without rotator cuff tear usually depends on the physical examination and roentgenography, and obtaining objective evidence for this condition is at best difficult. The purpose of this study was to ascertain whether quantitatively assessing this condition with using single photon emission computerized tomography (SPECT) can diagnose impingement syndrome and predict the postoperative results. METHODS: Before executing arthroscopic or open treatment, SPECT was performed on 73 patients and 24 volunteers and these people were followed up for 2 years. An

Lateral Scapular Winging and Shoulder Pain Following Posterior Fossa Surgery: A Case Report.

The spinal accessory nerve (SAN) provides motor innervation to the sternocleidomastoid and trapezius muscles. Injury to the distal segment of this nerve can result in trapezius denervation, leading to lateral scapular winging (LSW), shoulder pain, and impaired arm elevation. While iatrogenic SAN injury most commonly occurs during cervical lymph node biopsies and neck dissections, it may also arise following posterior fossa neurosurgical procedures due to prolonged traction, compression, or mechanical stress during surgical positioning or, less commonly, accidental direct ligation. Recognizing

Delayed union of a scapular fracture-an unusual cause of persistent shoulder pain.

INTRODUCTION: Scapular fractures in athletes are rare, although they have been reported in football and baseball players. Most scapular fractures heal with nonoperative management; delayed union is rarely a problem. CASE SUMMARY: A 15-yr-old male fell onto his posterior shoulder after he had been pushed into the boards and then fell to the ice during a hockey check. He was diagnosed with a rotator cuff injury at an outside facility. Initial radiographs were negative. The patient was not immobilized, and he continued to play hockey despite intermittent pain. Seven months later, he presented to

Exploring cellular changes in ruptured human quadriceps tendons at single-cell resolution.

Tendon ruptures in humans have often been studied during the chronic phase of injury, particularly in the context of rotator cuff disease. However, the early response to acute tendon ruptures remains less investigated. Quadriceps tendons, which require prompt surgical treatment, offer a model to investigate this early response. Therefore, this study aimed to explore the early cellular changes in ruptured compared to healthy human quadriceps tendons. Quadriceps tendon samples were collected from patients undergoing fracture repair (healthy) or tendon repair surgery (collected 7-8 days post-inju

Diagnosis and treatment of chronic painful shoulder: review of nonsurgical interventions.

Chronic painful shoulder is a common complaint, yet its initial pathogenesis may have multiple etiologies as well as multiple and perhaps distinct primary or secondary etiologies responsible for its final clinical presentation. While the clinical presentation of pain and limitation of motion is easily defined, there are no standardized criteria for the differential diagnosis of various chronic shoulder pain syndromes, much less a consensus on their treatment. At present, there are limited nonsurgical options and medications approved for the treatment of chronic painful shoulder, and, with limi

Surgical management of internal impingement in overhead athletes: a systematic review.

BACKGROUND: We aimed to review all published evidence on the surgical management of internal impingement and associated conditions in athletes of overhead sports. METHODS: Our search included studies reporting outcomes of any surgical intervention on shoulder internal impingement and its sequelae in overhead athletes. The primary outcome was return to sport at pre-injury level or higher in the main text (RTS+). Secondary outcomes included return to sport at any level (RTS), shoulder function, and complications. No quantitative analyses were possible due to the nature of the evidence included.

Suprascapular Nerve Entrapment: Current Concepts and Recent Advances.

Introduction The suprascapular nerve is inherently vulnerable to entrapment, as it is relatively constrained by its surrounding anatomy: proximally crossing the suprascapular notch; or more distally over the spinoglenoid notch. Despite this, suprascapular nerve entrapment is relatively uncommon, and has until recently been an underappreciated cause of shoulder pain and dysfunction.Causes and assessmentAetiology is typically due to traction or compression nerve injury, and a number of high-risk variants in anatomy have now been described. The symptoms are best investigated with magnetic resonan

Adipose-Derived Mesenchymal Stem Cell Treatments and Available Formulations.

PURPOSE OF REVIEW: The use of human adipose-derived mesenchymal stem cells (ADSCs) has gained attention due to its potential to expedite healing and the ease of harvesting; however, clinical evidence is limited, and questions concerning optimal method of delivery and long-term outcomes remain unanswered. RECENT FINDINGS: Administration of ADSCs in animal models has been reported to aid in improved healing benefits with enhanced repair biomechanics, superior gross histological appearance of injury sites, and higher concentrations of growth factors associated with healing compared to controls.

Integrating soft and hard tissues via interface tissue engineering.

The enthesis, or interface between bone and soft tissues such as ligament and tendon, is prone to injury and often does not heal, even post surgical intervention. Interface tissue engineering represents an integrative strategy for regenerating the native enthesis by functionally connecting soft and hard tissues and thereby improving clinical outcome. This review focuses on integrative and cell-instructive scaffold designs that target the healing of the two most commonly injured soft tissue-bone junctions: tendon-bone interface (e.g., rotator cuff) and ligament-bone interface (e.g., anterior cr
