Patients with gout can be cured in primary care.

Gout affects 2.5% of the total UK population and is four times more common in men than women. The peak prevalence and incidence in the UK is in those aged 80-84 years. Gout is associated with comorbidities such as nephrolithiasis, chronic renal impairment, metabolic syndrome, depression and heart disease. It is also associated with increased mortality. Untreated gout can result in disabling irreversible peripheral joint damage and chronic usage-related pain. However, gout is curable. The pathogenic agents that cause gout i.e.urate crystals can be eliminated through a combination of effective p

Comparative analysis of exon 10 and non-exon 10 variants in children with familial mediterranean fever: a retrospective cohort study.

UNLABELLED: Familial Mediterranean fever (FMF) is the most common inherited autoinflammatory disorder caused by MEFV variants. Exon 10 variants, particularly M694V, are strongly associated with severe disease, but the impact of non-exon 10 variants in children remains less defined. To evaluate genotype-phenotype correlations in pediatric FMF by comparing patients with homozygous exon 10, compound heterozygous exon 10, and exon 10 combined with non-exon 10 variants. This single-center retrospective cohort study included 477 children fulfilling Eurofever/PRINTO criteria between 2018 and 2025. Pa

Anti-gout active ingredients from natural plant medicines and their related mechanisms.

ETHNOPHARMACOLOGICAL RELEVANCE: The earliest known treatise on "gout" is found in Hippocratic Corpus, which was regarded as the "disease of kings" in early Europe. In ancient China, gout was documented in medicine text "Suwen", where it was categorized under the classification of "bi syndrome". Throughout human history, a wide range of herbal medicines and natural remedies, such as colchicine, have been used in the treatment of gout. OBJECTIVE: Gouty arthritis (GA) requires long-term pharmacotherapy, underscoring the critical need for agents with minimal adverse effects. Natural products, kno

Inhaled Budesonide for COVID-19 in People at Higher Risk of Complications in the Community: The UK National Community Randomi.

Background The effectiveness of repurposed treatments with supportive evidence for higher risk individuals with COVID-19 in the community is unknown. In the UK PRINCIPLE national platform trial we aimed to determine whether 're-purposed medicines' (hydroxychloroquine, azithromycin, doxycycline, colchicine, inhaled budesonide, and other interventions) reduced time to recovery and COVID-19 related hospitalisations/deaths among people at higher risk of COVID-19 complications in the community. We mainly report the findings for budesonide arm here. Methods Participants in this multicentre, open-lab

Rhabdomyolysis: an evaluation of 475 hospitalized patients.

Rhabdomyolysis is a common and potentially lethal clinical syndrome that results from acute muscle fiber necrosis with leakage of muscle constituents into blood. Myoglobinuria is the most significant consequence, leading to acute renal failure (ARF) in 15%-33% of patients with rhabdomyolysis. Rhabdomyolysis occurs from inherited diseases, toxins, muscle compression or overexertion, or inflammatory processes, among other disorders. In some cases, no cause is found. We describe 475 patients from the Johns Hopkins Hospital inpatient records between January 1993 and December 2001 for the following

Atypical multi-drug resistance (MDR): low sensitivity of a P-glycoprotein-expressing human T lymphoblastoid MDR cell line to classical P-glycoprotein-directed resistance-modulating agents.

Verapamil, cyclosporin A (CsA), the cyclosporin derivative SDZ PSC 833 and the novel cyclopeptolide SDZ 280-446 were tested for their capacity to chemosensitize a P-glycoprotein (Pgp)-expressing multi-drug resistant (MDR) variant of the CEM human T lymphoblastoid cell subline (CCRF ACTD 400+). That MDR-CEM cell subline had been previously selected for MDR by actinomycin D and displayed a very high resistance phenotype: 3700-fold for actinomycin D, 3900-fold for vincristine, 1200-fold for taxol, 1000-fold for daunomycin (DAU) and 400-fold for colchicine. Corticotropin-releasing factor in olivocerebellar climbing-fiber system of monkey (Saimiri sciureus and Macaca fascicularis): parasagittal and regional organization visualized by immunohistochemistry.

An antiserum directed against the human form of corticotropin-releasing factor (CRF) was utilized for immunohistochemical visualization of the distribution of this peptide in the inferior olivary nucleus and cerebellum of 2 monkey species (Saimiri sciureus, Macaca fascicularis). Colchicine pretreatment was not used. In both species, immunoreactivity was evident in the vast majority of neurons in the inferior olivary nucleus, with perikarya in the medial accessory olive exhibiting especially intense staining. In cerebellum, no labeled perikarya were present, but immunoreactive axons exhibiting

Brain and muscle Arnt-like protein-1 (BMAL1) controls circadian cell proliferation and susceptibility to UVB-induced DNA damage in the epidermis.

The role of the circadian clock in skin and the identity of genes participating in its chronobiology remain largely unknown, leading us to define the circadian transcriptome of mouse skin at two different stages of the hair cycle, telogen and anagen. The circadian transcriptomes of telogen and anagen skin are largely distinct, with the former dominated by genes involved in cell proliferation and metabolism. The expression of many metabolic genes is antiphasic to cell cycle-related genes, the former peaking during the day and the latter at night. Consistently, accumulation of reactive oxygen sp

Whole thoracal spinal cord involvement in case of neuro-Behçet's disease.

Behçet's disease is a chronic inflammatory, multisystem vasculitis. Neurological involvement is one of the most serious manifestations of Behçet's disease. Although brain stem and diencephelon are the most affected areas in neuro-Behçet's disease, spinal cord involvement are rarely seen. We report a case of paraplegia caused by completely thoracic cord involvement of Behçet's disease in a 20-year-old woman.

[Livedoid vasculopathy: A case report].

Livedoid vasculopathy (LV) is a painful and thrombotic obliterative vascular disorder characterized by spontaneous thrombus formation within skin capillaries or small veins, ultimately leading to localized hypoxia and the development of skin ulcers. LV is a rare condition predominantly affecting young women, with an incidence rate of approximately 1/100 000. Clinically, it manifests as recurrent reticular green spots and painful ulcers on the distal lower extremities, which subsequently heal, leaving behind white and atrophic scars, termed "white atrophy". The skin manifestations of the diseas

The RECLAIM adaptive platform trial for the evaluation of treatments for post-COVID condition in the Netherlands: core protocol.

BACKGROUND: Post-COVID condition (PCC) encompasses a heterogeneous spectrum of pathological responses to SARS-CoV-2 infection, with unknown individual predispositions. No evidence-based, curative treatments are available. Many patients therefore resort to off-label use of medications originally developed for other indications. However, the efficacy and safety of repurposed drugs in the context of PCC are unknown. METHODS: RECLAIM is a phase III, randomised, controlled, adaptive platform trial. The trial population consists of adult patients who have had persistent PCC symptoms, including fati

The systemic immune-inflammation index in coronary heart disease: a narrative review of thromboinflammation, phenotype-dependent utility, and clinical translation.

Coronary heart disease (CHD) is essentially a thrombo-inflammatory disease, and its key pathological mechanism lies in an acute thrombotic storm driven by the interaction between platelets, neutrophils, and lymphocytes. Existing inflammatory markers, which reflect only a single dimension, struggle to accurately capture this complex network. The Systemic Immune-Inflammation Index (SII), calculated using the formula (platelets × neutrophils/Lymphocytes), integrates the three dimensions of thrombosis, inflammation, and immunity, providing a novel composite indicator for quantifying the thrombo-in
