Interleukin-6 in synovial fluid and HLA-DR expression in synovium from patients with temporomandibular disorders.

Interleukin-6-dependent mouse hybridoma cell line KD83 was used to test the biologic activity of interleukin-6 in synovial fluid from 37 patients with temporomandibular disorders. The results showed that the interleukin-6 level was greater than 100 U/mL in 13 of 18 patients with degenerative joint disease and in five of 12 patients with temporomandibular disc displacement. However, the interleukin-6 level was less than 100 U/mL (range, 20 to 75 U/mL) in all patients with masticatory muscle disorder. It has been found that degenerative joint disease tends to have acute and chronic stages, and i

Reduced joint distance during TMJ movement in the posterior condylar position.

Biomechanical loading reduces joint distance and has a causative relationship with disc displacement in temporomandibular joint and/or osteoarthritis. Condylar movement and pathways during mouth opening and closing are different depending on the condylar position in the glenoid fossa. Therefore, physical loading on the articular disc or condylar head would also be different in different condylar positions. The aim of this study was to evaluate the 3-dimensional changes of joint distance in different anterior-posterior condylar positions. We divided 52 temporomandibular joints into anterior, co

Ultrasound assessment of increased capsular width in temporomandibular joint internal derangements: relationship with joint pain and magnetic resonance grading of joint effusion.

OBJECTIVE: The relationship between radiologic evidence of effusion in the temporomandibular joint (TMJ) and the occurrence of clinical symptoms (e.g., pain) is still unclear. Increased capsular width (CW) measured in ultrasonographic imaging (USI) of the TMJ was considered to be an indirect marker of TMJ effusion. The purpose of this study was to evaluate the relationship between the grades of magnetic resonance imaging (MRI)-depicted joint effusion (JE), increased CW measured in USI, and joint pain in TMJ internal derangement (ID) patients. STUDY DESIGN: During a 4-year period, 91 patients

A clinical, MRI, and EMG analysis comparing the efficacy of twin blocks and flat occlusal splints in the management of disc displacements with reduction.

Internal derangement of the temporomandibular joint (TMJ) is characterized by an abnormal disc-condyle relationship. Of all the various treatment modalities used in the management of disc displacements, flat occlusal and anterior repositioning splints are the most commonly used. Myofunctional appliances (such as bite-jumping appliances) are also advocated to treat anterior disc displacements. The present study compares the efficacy of twin blocks with conventional flat occlusal splints in patients with anterior disc displacement with reduction using clinical examination, MRI, and EMG. Temporomandibular joint internal derangement and osteoarthrosis: are effusion and bone marrow edema prognostic indicators for arthrocentesis and hydraulic distention?

PURPOSE: Arthrocentesis and hydraulic distention of the temporomandibular joint (TMJ) has proven to be an effective modality in treating patients exhibiting clinical findings consistent with the diagnosis of disc displacement without reduction. The purpose of this study was to investigate whether the magnetic resonance imaging (MRI) variables of effusion and/or bone marrow edema may predict treatment outcomes of arthrocentesis and hydraulic distention of the TMJ. PATIENTS AND METHODS: The study group comprised 37 consecutive patients with TMJ pain, who were assigned a unilateral clinical TMJ
