The influence of unilateral disc displacement on stress in the contralateral joint with a normally positioned disc in a human temporomandibular joint: an analytic approach using the finite element method.

OBJECTIVES: To investigate the influence of unilateral disc displacement (DD) in the temporomandibular joint (TMJ) on the stress in the contralateral joint, with a normally-positioned disc, during clenching. STUDY DESIGN: A finite element model of the TMJ was constructed based on MRI and 3D-CT of a single patient with a unilateral DD. A second model with bilateral normally-positioned discs served as a reference. The differences in stress distribution in various TMJ components during clenching were predicted with these models. RESULTS: In the unaffected joint of the unilateral DD model, the l

Occlusal effects on longitudinal bone alterations of the temporomandibular joint.

The pathological changes of subchondral bone during osteoarthritis (OA) development in the temporomandibular joint (TMJ) are poorly understood. In the present study, we investigated the longitudinal alterations of subchondral bone using a rat TMJ-OA model developed in our laboratory. Changes in bone mass were examined by micro-CT, and changes in osteoblast and osteoclast activities were analyzed by real-time PCR, immunohistochemistry, and TRAP staining. Subchondral bone loss was detected from 8 weeks after dental occlusion alteration and reached the maximum at 12 weeks, followed by a repair ph

Development of temporomandibular joint ankylosis in rats using stem cells and bone graft.

64 male Wistar rats were used: 24 for the removal of stem cells, 4 as a control group, and 36 for the experiment, in which either stem cells or bone graft was used. The rats were divided into groups according to the type of procedure and time span (15, 30 or 60 days). The joints were submitted to histological study in order to score the ankylosis. The mean differences between initial and final maximal mouth opening (MMO) were gradually increased from 15 to 60 days, for all times of evaluation for both groups, being statistically significant at 15 days (p=0.045) in the bone-graft group. When bo

Do patients treated with bimaxillary surgery have more stable condylar positions than those who have undergone single-jaw surgery?

PURPOSE: Because condylar positioning after sagittal split ramus osteotomy of the mandible has been known to affect postoperative skeletal stability, accurate positional assessment of the temporomandibular joint after orthognathic surgery is vital to maximize stability of the surgery. The purpose of this study was to evaluate condylar changes after single-jaw and double-jaw surgeries in mandibular prognathism patients by comparing 3-dimensional angular and positional changes of the condylar heads in groups of patients receiving combined maxillary posterior impaction and mandibular setback and

Reconstruction of ankylosed temporomandibular joint: Sternoclavicular grafting as an approach to management.

A retrospective analysis of 15 cases of ankylosed temporomandibular joint (TMJ) reconstructed, with sternoclavicular joint graft (SCG), during the period 2002-2007 was undertaken. All cases were analyzed for functional adaptation of the graft, considering maximum interincisal opening, and protrusive and laterotrusive movement of the jaw. Significant improvement was noticed in all cases except one, although maximum improvement was seen 3-6 months postoperatively. Radiological evaluation was carried out at regular intervals for 2-3 years to assess the anatomical adaptation of the graft. No major

Analysis of the performance of different orthodontic devices for mandibular symphyseal distraction osteogenesis.

The aim of this study was to investigate the performance of different orthodontic devices for mandibular symphyseal distraction osteogenesis (MSDO). Two performance parameters were analysed, the first of which concerned the stability guaranteed by a distractor in the fracture gap under mastication loads and the second the level of reliability with which a distractor transfers a given expansion to the mandibular bone, inasmuch as the more reliable the device the smaller the difference between the degree of expansion provided to the device and the displacement achieved on the mandibular arch. Studies of changes in occlusion after the insertion of complete dentures (part II).

The aim of this clinical experimental study was to investigate whether, and what, changes in occlusion occur between 3 weeks and 1 year after insertion of complete dentures. Twenty-six edentulous patients, who already had their jaw relation registered for part I of this study some 357 days before, were re-examined. The same special non-arcon measuring articulator was used for determination of any positional differences between the presently recorded central condylar position with inter-occlusal gap and the equilibrated intercuspal position, from approximately 1 year previous. Differences were

Minimal Access Retromandibular Approach to Fractures of Vertical Segment of the Mandible: A Versatile Technique.

Introduction  Fractures of the condylar region of the mandible, although fairly common, also generate the most debate regarding management-both closed and open treatment options have been recommended and shown to yield good results. We present our experience with a minimal access retromandibular approach to fractures of the vertical mandible. Materials and Methods  This is a retrospective study of all patients who underwent open reduction and internal fixation for fractures of the vertical mandible (condyle, subcondylar region, ramus) using a retromandibular approach at a tertiary care hospita

Class III malocclusion with complex problems of lateral open bite and severe crowding successfully treated with miniscrew anchorage and lingual orthodontic brackets.

In this article, we report the successful use of miniscrews in a patient with an Angle Class III malocclusion, lateral open bite, midline deviation, and severe crowding. Simultaneously resolving such problems with conventional Class III treatment is difficult. In this case, the treatment procedure was even more challenging because the patient preferred to have lingual brackets on the maxillary teeth. As a result, miniscrews were used to facilitate significant asymmetric tooth movement in the posterior and downward directions; this contributed to the camouflage of the skeletal mandibular protru

Expression of chondromodulin-1 in the temporomandibular joint condylar cartilage and disc.

BACKGROUND: The temporomandibular joint (TMJ) cartilage consists of condylar cartilage and disc and undergoes continuous remodeling throughout post-natal life. To maintain the integrity of the TMJ cartilage, anti-angiogenic factors play an important role during the remodeling process. In this study, we investigated the expression of the anti-angiogenic factor, chondromodulin-1 (ChM-1), in TMJ cartilage and evaluate its potential role in TMJ remodeling. METHODS: Eight TMJ specimens were collected from six 4-month-old Japanese white rabbits. Safranin-O staining was performed to determine proteo

Mandibular reconstruction in Goldenhar syndrome using temporalis muscle osteofascial flap.

Goldenhar syndrome is a well-known developmental anomaly of the maxillofacial skeleton and hemifacial soft tissue. Maxillofacial anomalies of that syndrome can be managed by a variety of means such as inlay- or onlay-applied nonvascularized bone grafts, vascularized osteocutaneous/osseous flaps, or distraction osteogenesis. Vascularized full-thickness calvarial bone grafting is an important option for mandibular reconstruction for cases in which, for one reason or another, other techniques are not available, not applicable, or have failed.
