Multidrug-resistant bacteria with ESBL genes: a growing threat among people living with HIV/AIDS in Nepal.

BACKGROUND: Bacterial opportunistic infections are common in people living with HIV/AIDS (PLHA). Besides HIV-TB co-infection, lower respiratory tract infections (LRTIs) due to multidrug-resistant (MDR) bacteria cause significant morbidity and mortality among PLHA. This study identified bacterial co-infection of the lower respiratory tract and detected plasmid-mediated blaTEM and blaCTX-M genes among Extended-Spectrum β-Lactamase (ESBL) producing isolates from sputum samples in PLHA. METHODS: A total of 263 PLHA with LRTIs were enrolled in this study, out of which, 50 were smokers, 70 had prev

Ethnobotanical plants used in the management of symptoms of tuberculosis in rural Uganda.

BACKGROUND: Tuberculosis (TB) caused by Mycobacterium tuberculosis is the 13th leading cause of death worldwide. The emergence of multidrug-resistant TB (MDR-TB) poses a major health security threat. Plants have traditionally been used as a source of medicine, since olden days and 80% of the communities in Africa still rely on herbal medicines for their healthcare. In many parts of Uganda, some plants have shown ethno-pharmacological prospects for the treatment of TB, and yet they have not been fully researched. AIM: This study aimed to document plant species used traditionally by the herbali

Infections in solid organ transplantation in special situations: HIV-infection and immigration.

With the advent of highly active antiretroviral therapy in 1996, patients infected with HIV are now living longer and are dying from illnesses other than acquired immunodeficiency syndrome (AIDS). Liver disease due to chronic hepatitis C is now a leading cause of mortality among HIV-infected patients in the developed world. The prevalence of end-stage kidney or heart disease is also increasing among HIV-infected patients. For these patients, solid organ transplantation (SOT) is the only therapeutic option and HIV infection alone is not a contraindication. Accumulated experience in North Americ

Computational Discovery of Phytochemicals Targeting InhA of Mycobacterium tuberculosis: Insights from Molecular Docking, MMGBSA, ADME Profiling, and Molecular Dynamics Simulations.

INTRODUCTION: Tuberculosis (TB) is one of the most serious global health issues, with the increasing number of multidrug-resistant TB cases emphasizing the need for new therapeutic approaches. Phytochemicals, with their diverse structures and favorable safety profiles, are a largely unexplored area for anti-TB drug development. METHODS: An ethnobotanical study and literature analysis identified 310 medicinal plants traditionally used to treat respiratory infections, which produced 4,087 phytochemicals. Their structures were obtained from PubChem or drawn using ChemDraw, and pharmacokinetic pr

[Reconsideration of the admission and discharge criteria of tuberculosis patients in Japan].

Japanese Ministry of Health, Labour and Welfare issued new criteria for admission and discharge of tuberculosis patients in 2007. The criteria for admission are extended for the patients of the risk of Mycobacterium tuberculosis transmission and of the possible risk of treatment failure. The criteria for discharge are consisted of the 3 factors, (1) symptoms (cough, fever, etc.) are free after the standard chemotherapy of more than 2 weeks, (2) three different sputum smears are negative for acid-fast bacilli after the standard chemotherapy of more than 2 weeks, (3) patients are estimated to ad

Amikacin exposure in MDR-TB patients in Uganda: Revisiting old drugs in a new era of resistance - A pharmacokinetic assessment.

BACKGROUND: Amid rising resistance to bedaquiline, aminoglycosides remain an important fallback option for multidrug-resistant tuberculosis (MDR-TB) treatment, particularly in high-burden settings. Their use is limited by nephro- and ototoxicity, which is associated with cumulative drug exposure. In this study we investigated amikacin exposure in Ugandan MDR-TB patients using a validated limited sampling strategy and compared the results to previously published data from a Western European cohort. METHODS: In this single-centre prospective observational study, 29 MDR-TB patients received amik

Long-term tolerance and effectiveness of moxifloxacin therapy for tuberculosis: preliminary results.

The resurgence of tuberculosis is a major problem. Increasing multiple resistance to current drugs used for therapy, non-compliance to therapy or co-morbidity are challenging problems that do not allow use of standard therapy in all patients. Quinolones are claimed to be active drugs in TB infection. Moxifloxacin shows the highest intracellular concentration in vitro and in experimental animals, but long-term tolerability is unknown. Our aim was to observe in compliant patients, not eligible for standard therapy, the effect of 6 months of therapy with moxifloxacin, isoniazid and rifampin. Nine

N-acetyltransferase genotypes and the pharmacokinetics and tolerability of para-aminosalicylic acid in patients with drug-resistant pulmonary tuberculosis.

The aim of this study was to examine the relationships between N-acetyltransferase genotypes, pharmacokinetics, and tolerability of granular slow-release para-aminosalicylic acid (GSR-PAS) in tuberculosis patients. The study was a randomized, two-period, open-label, crossover design wherein each patient received 4 g GSR-PAS twice daily or 8 g once daily alternately. The PAS concentration-time profiles were modeled by a one-compartment disposition model with three transit compartments in series to describe its absorption. Patients' NAT1 and NAT2 genotypes were determined by sequencing and restr

Metagenomic profiling of pathogens and antibiotic resistome in influent of six municipal wastewater treatment plants: a descriptive analysis of plant-specific microbial hazards.

INTRODUCTION: Wastewater treatment plants (WWTPs) serve as critical nodes for monitoring urban biological hazards, yet the raw influent-the primary entry point for pathogens and antibiotic resistance genes (ARGs)-remains less characterized compared to treated effluent, particularly at the level of individual facilities, as most prior studies have pooled samples or focused on post-treatment matrices. METHODS: In this descriptive study, we performed metagenomic sequencing on influent samples collected from six municipal WWTPs, with each plant treated as an independent unit to profile its specif

Nationwide HIV-, MDR-TB survey in Japan and collaborative study in the Philippines.

BACKGROUND: Although the prevalence of pulmonary tuberculosis (TB) and HIV infection in Japan is low, careful monitoring of these two diseases is necessary. We conducted a nationwide survey on multidrug resistant (MDR)-TB (2011-2013) and HIV-TB (2007-2014) to understand the mode of prevention and the effect of therapy. A study on MDR-TB and HIV in San Lazaro Hospital (SLH) in the Philippines was also conducted. These studies introduced an international collaborative study against the global epidemics of HIV-TB/MDR-TB. METHODS: The nationwide survey of MDR-TB was done in hospitals that treat T

Addressing the drug-resistant tuberculosis challenge through implementing a mixed model of care in Uganda.

Worldwide, Drug-resistant Tuberculosis (DR-TB) remains a big problem; the diagnostic capacity has superseded the clinical management capacity thereby causing ethical challenges. In Sub-Saharan Africa, treatment is either inadequate or lacking and some diagnosed patients are on treatment waiting lists. In Uganda, various health system challenges impeded scale-up of DR-TB care in 2012; only three treatment initiation facilities existed, with only 41 of the estimated 1010 RR-TB/MDR-TB cases enrolled on treatment yet 300 were on the waiting list and there was no DR-TB treatment scale-up plan. Performance of the MeltPro MTB Assays in the Diagnosis of Drug-Resistant Tuberculosis Using Formalin-Fixed, Paraffin-Embedded Tissues.

OBJECTIVES: The MeltPro MTB assays for detection of resistance to antituberculosis (TB) drugs perform well in genotypic drug susceptibility testing (DST) of clinical samples, but their effectiveness with formalin-fixed, paraffin-embedded (FFPE) tissues is unknown. METHODS: FFPE tissues were obtained from 334 patients with TB. Susceptibility to rifampicin (RIF), isoniazid (INH), and fluoroquinolones was examined using the MeltPro MTB assays, with Xpert MTB/RIF (Xpert) and/or phenotypic DST (pDST) results as references. Samples with discordant results were analyzed by multiplex polymerase chain

Community-based treatment of advanced HIV disease: introducing DOT-HAART (directly observed therapy with highly active antiretroviral therapy).

In 2000, acquired immunodeficiency syndrome (AIDS) overtook tuberculosis (TB) as the world's leading infectious cause of adult deaths. In affluent countries, however, AIDS mortality has dropped sharply, largely because of the use of highly active antiretroviral therapy (HAART). Antiretroviral agents are not yet considered essential medications by international public health experts and are not widely used in the poor countries where human immunodeficiency virus (HIV) takes its greatest toll. Arguments against the use of HAART have mainly been based on the high cost of medications and the lack
