Impact of preoperative electrophysiological intervention on occurrence of peri/postoperative supraventricular tachycardia following Fontan surgery.

BACKGROUND: Little is known about the effects of preoperative electrophysiological study (EPS) and catheter ablation (CA) in Fontan surgery candidates with supraventricular tachycardia (SVT). OBJECTIVE: The purpose of this study was to investigate the clinical impact of EPS-guided intervention in Fontan surgery candidates with preceding SVT events. METHODS: A total of 109 consecutive patients with a history of SVT before Fontan surgery were divided into 3 groups: 44 in whom EPS with CA was attempted (CA group); 21 in whom EPS without CA was attempted (EPS group); and 44 in whom EPS was not p

Electrophysiological features and radiofrequency catheter ablation of accessory pathways associated with atrioventricular discordance.

INTRODUCTION: Although a high prevalence of the presence of an accessory pathway (AP) associated with atrioventricular (AV) discordance has been reported, a case series of its characteristics and the results of catheter ablation (CA) have not been sufficiently documented. METHODS AND RESULTS: We retrospectively examined 11 consecutive patients with atrioventricular discordance who underwent CA for atrioventricular reciprocating tachycardia (AVRT) via an AP and planned cardiac surgery after CA. Orthodromic AVRTs were induced in 10 patients via AP, but no antidromic/duodromic AVRT was induced i

The Value of Functional Fascicular Block in Patients With Atrioventricular Re-Entrant Tachycardia.

BACKGROUND: Changes in retrograde conduction time (Δ ventriculoatrial interval [ΔVAI]) during functional complete bundle branch block (BBB) have been well established for localizing the accessory pathway (AP) in atrioventricular re-entrant tachycardia (AVRT). OBJECTIVES: This study investigated whether ΔVAI during functional fascicular block (FB) provides more detailed AP localization and the significance of incomplete BBB in AVRT. METHODS: A total of 419 patients (age 22 ± 16 years; 39% male) with single non-decremental APs (left free wall, n = 207; septal, n = 159; and right free wall, n =

[Intravenous ablation of the atrio-ventricular junction in patients with supraventricular tachyarrhythmias].

Since the first successful therapeutic DC ablation of the AV junction in 1986, we have treated 20 symptomatic patients with drug-refractory supraventricular tachyarrhythmias (average of 6 antiarrhythmic drugs prior to the ablation attempt). The primary rhythm disturbances necessitating ablation were: AV nodal reentrant tachycardia (50% of pts), atrial flutter or fibrillation, with an uncontrolled rapid ventricular response (40%), atrioventricular reentrant tachycardia using an accessory pathway (20%), atrial tachycardia (10%), and junctional reciprocating tachycardia (5%). Percutaneous cathete

Intermittent versus Persistent Wolff-Parkinson-White Syndrome in Children: Electrophysiologic Properties and Clinical Outcomes.

BACKGROUND: Intermittent Wolff-Parkinson-White (WPW) syndrome is considered to have a lower risk of sudden death. Fewer data exist regarding electrophysiologic (EP) characteristics and the natural history of intermittent WPW in children. METHODS: All patients with WPW age 1-18 years at a single institution (1996-2013) were reviewed. Patients with intermittent preexcitation were compared to those with loss of preexcitation on Holter/exercise testing and those with persistent preexcitation. High-risk accessory pathway (AP) was defined as AP effective refractory period (APERP), block cycle lengt

First experience with zero-fluoroscopic ablation for supraventricular tachycardias using a novel impedance and magnetic-field-based mapping system.

AIMS: Zero- and near-zero-fluoroscopic ablation techniques reduce the harmful effects of ionizing radiation during invasive electrophysiology procedures. We aimed to test the feasibility and safety of a zero-fluoroscopic strategy using a novel integrated magnetic and impedance-based electroanatomical mapping system for radiofrequency ablation (RFA) of supraventricular tachycardias (SVTs). METHODS: We retrospectively studied 92 consecutive patients undergoing electrophysiology studies with/without RFA for supraventricular tachycardia (SVT) performed by a single operator at a single center. Manifest Preexcitation-Related Cardiac Dysfunction in the Absence of Tachyarrhythmia: A Systematic Review of Literature.

Cardiac dysfunction secondary to manifest ventricular pre-excitation via accessory pathways (AP) in the absence of tachyarrhythmias is rarely studied. To evaluate pathophysiology, AP location impact, diagnostic approach, and role of catheter ablation (CA) in pre-excitation-related cardiac dysfunction without tachyarrhythmias. Electronic databases were queried for WPW/pre-excitation and cardiac dysfunction-related studies without age restrictions. A total of 34 studies met inclusion criteria, including 27 case reports/series (42 patients, 62% female) and 7 observational studies (412 participant

A Novel Wave-speed Mapping Method for Visualizing Accessory Pathways in Wolff-Parkinson-White Syndrome.

INTRODUCTION: Catheter ablation (CA) is the first-line treatment with a high success rate for patients with symptomatic Wolff-Parkinson-White syndrome, while three-dimensional (3D) mapping systems enable the identification of accessory pathways (APs). We aimed to develop a novel mapping method using wave-speed mapping (WSM) to determine AP locations and CA outcomes. METHODS AND RESULTS: This study included 19 patients diagnosed with atrioventricular (AV) reciprocating tachycardia. We compared local activation time (LAT) mapping and WSM, and analyzed the relationship between the "high-speed is

An unusual ectopic ventricular rhythm in a young woman.

A case of a 22-year-old young pregnant woman with palpitations and near syncope is presented. Holter monitoring showed very frequent premature beats and runs of wide complex tachycardia, refractory to antiarrhythmic drugs. Electrophysiologic evaluation disclosed spontaneous automatism arising in an atriofascicular pathway. Differential diagnosis is discussed.

Left ventricular dyssynchrony in pre-excitation syndrome: effect of accessory pathway location and reversibility after ablation therapy.

We sought to evaluate the effects and reversibility of different locations of accessory pathways (AP) on left ventricular dyssynchrony (LVdys). The acute and chronic effects of AP were evaluated in a canine model (n = 11) and in patients with pre-excitation syndrome (n = 25). Pre-excitation was simulated in the canine model by applying VDD-type epicardial ventricular pacing near the atrioventricular (AV) groove with 50-ms AV interval after median thoracotomy, at five different sites in each animal. For the simulation of pre-excitation through the septal accessory pathway, right basal septal pa

Fascicular ventricular tachycardia: experience with radiofrequency ablation.

Fascicular ventricular tachycardia (VT), the commonest form of idiopathic left VT, occurs more frequently in young males without structural heart disease and usually presents as paroxysmal palpitations. It is subdivided into two more common subtypes, posterior and anterior. A macro-reentrant circuit involving a considerable and variable extent of the left interventricular septum is presumed to be the underlying arrhythmogenic mechanism. A slow conduction zone with particular sensitivity to verapamil participates in the circuit and it seems that diastolic potentials (DP) represent the electrica

Radiofrequency catheter ablation of ventricular tachycardia in patients without structural heart disease.

BACKGROUND: Radiofrequency energy has been used safely and successfully to eliminate accessory pathways in patients with the Wolff-Parkinson-White syndrome and the substrate for atrioventricular nodal reentrant tachycardia. However, this form of ablation has had only limited success in eliminating ventricular tachycardia in patients with structural heart disease. In contrast, direct-current catheter ablation has been used successfully to eliminate ventricular tachycardia in patients with and without structural heart disease. The purpose of this study was to test whether radiofrequency energy c

Intra-cardiac ultrasound guided approach for catheter ablation of typical right free wall accessory pathways.

BACKGROUND: Right free wall accessory pathways (AP) are difficult to treat with catheter ablation as ablation catheter (AC) instability at the tricuspid annulus often precludes successful procedure. The aim of our study was to test a novel intra-cardiac echocardiography (ICE) guided technique for AC placement. Feasibility and success rates were observed. METHODS: Eight consecutive patients (aged 29 ± 21 years, 4 female) with Wolff-Parkinson-White syndrome and a right free wall AP were included in the study. Sudden death of an infant with cardiac, nervous system and genetic involvement--a case report.

UNLABELLED: We present a case of sudden death of a 1-month-old male infant with heart, brainstem and genetic polymorphism involvement. Previously considered quite healthy, the child died suddenly and unexpectedly during sleep. The autopsy protocol included an in-depth anatomopathological examination of both the autonomic nervous system and the cardiac conduction system, and molecular analysis of the serotonin transporter gene promoter region, in which a specific genetic condition seems to be associated with sudden infant death. Histological examination revealed the presence of congenital cardi

Acute and long-term outcome of transvenous cryothermal catheter ablation of supraventricular arrhythmias involving the perinodal region.

OBJECTIVE: Cryoenergy is a new valuable treatment option to perform ablation close to the atrioventricular (AV) node in the cure of supraventricular tachycardias because of its favourable properties, such as the possibility of creating reversible lesions. The aim of this study was to report our experience on the effectiveness and safety of catheter cryoablation performed in "critical areas" to treat a large cohort of patients with supraventricular arrhythmias. METHODS: One hundred and thirty-one patients suffering from supraventricular tachycardias underwent catheter cryoablation using a 7F c

Accessory Pathway Antegrade Electrophysiologic Features Among Wolff-Parkinson-White Patients: The Risk in Relation to the Location.

BACKGROUND: Wolff-Parkinson-White (WPW) syndrome is a disorder characterized by presence of an accessory pathway (AP) which predisposes patients to tachyarrhythmia and sudden death. The aim of this study was to evaluate the prevalence of high-risk electrophysiologic AP features among WPW patients referred for electrophysiological study (EPS) and to explore whether the AP location can predict the high-risk nature of the AP. METHODS: This descriptive observational study was carried out on 70 patients with WPW subjected to invasive EPS. All patients were subjected to determination of AP anterogr

Frequency of fractionated ventricular activation and atrial/ventricular electrogram amplitude ratio at successful ablation target of accessory pathways in patients with Ebstein's anomaly.

INTRODUCTION: Knowledge about local electrogram and atrial/ventricular electrogram amplitude ratio (A/V ratio) at the true atrioventricular ring for successful ablation of accessory pathways (APs) in patients with Ebstein's anomaly is limited. METHODS: Twenty-two adults with Ebstein's anomaly and APs were managed by 24 sessions of radiofrequency catheter ablation (RFCA). A right atrial/ventricular angiogram with or without 3-dimensional electroanatomic mapping was performed to delineate the true atrioventricular ring and atrialized right ventricle (ARV). Electrograms of successful targets wer
