Electroconvulsive Therapy in the Presence of Cerebral Cavernous Hemangioma: A Case Report With Controlled Hemodynamic Strategy.

Electroconvulsive therapy (ECT) is an effective treatment for severe and treatment-resistant bipolar depression, yet intracranial vascular malformations such as cerebral cavernous malformations raise safety concerns due to transient ECT-induced increases in blood pressure and intracranial pressure, potentially elevating the risk of hemorrhage. We report a case of a woman in her mid-60s with treatment-resistant bipolar depression and severe symptom burden (MADRS 45; HAMD-17 35; HAMD-21 39) in whom brain MRI incidentally revealed a 6×4 mm cavernous hemangioma in the left precentral gyrus without

A retrospective study of structural brain lesions identified by magnetic resonance imaging in 114 cats with neurological signs.

BACKGROUND AND AIM: Magnetic resonance imaging (MRI) has been widely used as a non-invasive modality to evaluate neurological organ structures. However, brain MRI studies in cats with neurological signs are limited. This study evaluated the association between patient characteristics, neurological signs, and brain lesion locations identified by MRI. Blood profiles of cats with presumptive inflammatory and structural brain lesions were also determined. MATERIALS AND METHODS: Medical records of 114 cats that underwent brain MRI were retrospectively reviewed. Cats were categorized into five grou

Paraganglioma of the cervical sympathetic trunk.

We report the case of a 17-year-old man who presented with left-sided Horner's syndrome. Magnetic resonance imaging revealed a spindle-shaped cervical tumor in the left paravertebral space. During operation, a tumor originating from the left sympathetic trunk was found. The histopathologic analysis showed a sympathetic paraganglioma. The sympathetic trunk is an extremely rare location for a cervical paraganglioma; only a few cases have been reported in the literature.

3-MeO-PCP intoxication in two young men: First in vivo detection in Italy.

3-MeO-PCP or 3-methoxyphencyclidine is a derivative of phencyclidine. It acts as a dissociative anesthetic and it has allegedly hallucinogenic and sedative effects. There are almost no documented intoxication cases and references about its pharmacology and toxicity in literature. This study presents two concomitant intoxication cases due to consumption of 3-MeO-PCP and alcohol. A 19 (A) and a 21 years old (B) men were brought to Santa Maria Nuova Hospital in a comatose state (Glasgow score 3). They showed respiratory acidosis, right anisocoria with mydriatic pupils and hypothermia. Toxicologic

Pediatric Lemierre syndrome of rhinosinusogenic origin complicated by bilateral cavernous sinus thrombosis: A case report.

BACKGROUND: Lemierre syndrome is a rare but potentially life-threatening septic thrombophlebitis classically following an oropharyngeal infection, most often associated with Fusobacterium necrophorum. Atypical head and neck sources and unusual patterns of venous thrombosis have increasingly been reported. CASE PRESENTATION: We report the case of a 13-year-old boy admitted with fever, meningeal syndrome, left cervical swelling, ptosis, anisocoria, and altered consciousness following a short history of dental pain and fever. Contrast-enhanced cerebral and cervicothoracic computed tomography est

Isolated Unilateral Oculomotor Neuropathy Following A Presumed Infection Treated With Corticosteroids in a Child.

BACKGROUND Oculomotor neuropathy represents a very small proportion of cranial nerve palsies in children. It may result from both congenital and acquired causes. Acquired causes include vasculopathies, inflammation, infections, and neoplasms. However, only a limited number of studies in adults and children have identified post-infectious isolated oculomotor neuropathy, and even fewer have investigated the therapeutic role of corticosteroids in its treatment. CASE REPORT A previously healthy 4-year-old Saudi boy presented to the emergency department with acute onset of substantial drooping of t

Poverty and Vision: The Effect of Title 1 Status on Vision Screening Referral Rates in School-Aged Children in Western South Dakota.

PURPOSE: Interventions such as eye exams and glasses are used to correct visual problems that may lead to amblyopia, an irreversible decrease in visual acuity. Children with limited access to these interventions are more likely to have unaddressed visual problems that can lead to amblyopia or negatively impact school performance. This study compared vision screening results of children in schools with Title 1 or Non-Title 1 designation to investigate the link between poverty and vision. METHODS: Data from KidsFIRST vision screenings conducted with the SPOTTM photoscreener performed in Rapid C

Surgical Venous Drainage Disconnection from Cavernous Sinus Dural Arteriovenous Fistula and Ruptured Varix.

BACKGROUND: Cavernous sinus dural arteriovenous fistulas (CS dAVFs) occasionally behave aggressively (e.g., intracranial hemorrhage, venous infarction, seizures) depending on the drainage flow and presence of a collateral route of cortical or basal cerebral venous drainage. When a CS dAVF with aggressive behavior is encountered, a radical cure is required to avoid catastrophic deficits. However, conventional transvenous cavernous sinus (CS) embolization via the inferior petrosal sinus does not always achieve shunt obliteration. We herein report a case of surgical venous drainage disconnection

Marek's Disease in Backyard Chickens, A Study of Pathologic Findings and Viral Loads in Tumorous and Nontumorous Birds.

Marek's disease (MD) is a major cause of mortality in backyard chickens. The diagnosis of MD is complex, however, and knowledge of Marek's disease virus (MDV) in spontaneous field cases such as in backyard chickens is largely unknown. In this study, 40 backyard chickens with a presumptive MD diagnosis based on histologic lymphoid infiltrations in peripheral nerves with and without lymphomas were investigated. Twenty-eight of the birds were submitted to the diagnostic laboratory for disease explorations, and 12 chickens were from a flock in which some members demonstrated anisocoria and pupil i

Emergency decompressive craniectomy after removal of convexity meningiomas.

BACKGROUND: Convexity meningiomas are benign brain tumors that are amenable to complete surgical resection and are associated with a low complication rate. The aim of this study was to identify factors that result in acute postoperative neurological worsening after the removal of convexity meningiomas. METHODS: Clinical evaluation and neuroradiological analysis of patients who underwent removal of a supratentorial convexity meningioma were reviewed. Patients were selected when their postoperative course was complicated by acute neurological deterioration requiring decompressive craniectomy.

Monitoring intracranial pressure in patients with malignant middle cerebral artery infarction: is it useful?

OBJECT: Intracranial pressure (ICP) monitoring is increasingly used in the treatment of patients with malignant middle cerebral artery (MCA) infarction. However, neurological deterioration may exist independent from intracranial hypertension. This study aimed to present the findings of continuous ICP monitoring in a cohort of patients with malignant MCA infarction and to correlate these findings with clinical and radiological features. METHODS: The authors studied a prospective cohort of 25 patients with malignant MCA infarction consecutively admitted to the neurotrauma intensive care unit of

Clinical and topographic magnetic resonance characteristics of suspected brain infarction in 40 dogs.

Medical records of 40 dogs presented for evaluation of acute-onset, nonprogressive, intracranial dysfunction by means of magnetic resonance imaging (MRI) diagnosis of brain infarction were reviewed. Location of the brain infarcts was: 11 of 38, telencephalic; 8 of 38, thalamic/midbrain; 18 of 38, cerebellar; and 3 of 38, multifocal. Telencephalic infarcts developed within the territory of the middle cerebral (4/11), rostral cerebral (2/11), and striate (5/11) arteries. Thalamic/midbrain infarcts developed within the territory of perforating arteries of the caudal portion of the thalamus and ro

[A case of alpha coma in acute brainstem dysfunction--consecutive electroencephalograms and evoked potentials].

A 31-year-old woman was admitted because of severe headache and dysarthria in December 1985. Neurological examination on admission revealed severe impairment of consciousness, anisocoria, absent light reflex on the right side, and evidence of left hemiparesis, but other brainstem reflexes were intact. A CT scan taken shortly after arrival demonstrated a large hematoma in the right temporal lobe and the right cerebellum. Breathing became irregular and intubation was needed. An emergency operation was performed. After the operation the patient remained comatose without any spontaneous respiratio

Incomplete Vogt-Koyanagi-Harada Syndrome Presenting With Sunset Glow Fundus, Vitiligo, Preserved Vision, and Incidental CA 19-9 Elevation.

Vogt-Koyanagi-Harada (VKH) disease is a T cell-mediated autoimmune attack on melanocytes in the eyes, ears, skin, and central nervous system. It is characterized by granulomatous panuveitis and often results in significant visual loss. We report a 60-year-old woman referred for suspected chorioretinal inflammation following one month of treatment for anterior uveitis. Remarkably, visual acuity was preserved at 6/6 in both eyes. There was no history of ocular surgery or trauma. Examination revealed trace anterior chamber cells and mild anisocoria. Choroidal folds were present without serous ret

The Usefulness of Reduced Anion Gap and the Possibility of Pupillary Findings in the Treatment of Acute Bromvalerylurea Intoxication.

Bromvalerylurea is found as an over-the-counter analgesic and hypnotic drug in Japan and can be purchased at drugstores or over the Internet. Therefore, both acute poisoning due to large doses taken in suicide attempts and chronic poisoning due to continuous use for chronic pain have been observed. We report a case of acute BVU poisoning due to the use of an over-the-counter hypnotic sedative for a suicide attempt. A 34-year-old woman was referred to our ICU with unexplained disturbance of consciousness, respiratory failure, and shock. During ICU management, when her pupil diameter was measure

Insight Into Uncommon Territory: Exploring Internuclear Ophthalmoplegia in Artery of Percheron Infarct.

Internuclear ophthalmoplegia (INO), a neurological disorder is characterized by horizontal gaze palsy because of a lesion in the medial longitudinal fasciculus, a neural pathway that is mainly responsible for coordinating the movements of the eye. INO presents with diplopia and impaired adduction of the affected eye, accompanied by abducting eye nystagmus. The condition also arises from different etiologies which include multiple sclerosis, encephalitis, Lyme disease, HIV, and herpes zoster. Artery of Percheron (AOP) infarction is a subtype of bilateral thalamic infarction that poses a unique
