Neural regulation and manipulation of pancreatic β cells.

Pancreatic β cells are regulated by integrative neural signals beyond simple autonomic efferent control. Emerging evidence highlights the critical roles of afferent nerve signals and the gut-liver-brain-pancreas network in regulating β cells. Harnessing these intrinsic neural pathways may offer innovative therapeutic strategies for restoring β cell function and mass.

[Study of autonomic dysfunction in patients with obstructive sleep apnea syndrome to head-up tilt test].

Although it is well known that autonomic dysfunction in obstructive sleep apnea syndrome (OSAS) is associated with hypertension, its relationship to hypotension and orthostatic dysregulation is still unclear. We examined the response of blood pressure (BP) and cardiovascular autonomic function to head-up tilt (HUT) test in patients with OSAS. In this study, 14 patients(mean age: 65+/-2 years old, male/female: 11/3) with diagnosed OSAS by overnight polysomnography and 84 healthy subjects(mean age: 62+/-1 years old, male/female: 46/38) underwent HUT test(from 5 to 10 min at 45 degrees). Brain stem auditory evoked potentials in patients with multiple system atrophy with progressive autonomic failure (Shy-Drager syndrome).

Brain stem potentials from three groups of patients, namely those with pure progressive autonomic failure, Parkinson's disease and multisystem atrophy with progressive autonomic failure (Shy-Drager syndrome) were compared with each other and a group of normal subjects. In virtually all the patients with multisystem atrophy with progressive autonomic failure the brain stem potentials were abnormal in contrast to normal findings with Parkinson's disease. The closely associated group of patients with progressive autonomic failure alone also revealed no abnormalities of the BAEP. This separation o

Enteric nervous system: sensory transduction, neural circuits and gastrointestinal motility.

The gastrointestinal tract is the only internal organ to have evolved with its own independent nervous system, known as the enteric nervous system (ENS). This Review provides an update on advances that have been made in our understanding of how neurons within the ENS coordinate sensory and motor functions. Understanding this function is critical for determining how deficits in neurogenic motor patterns arise. Knowledge of how distension or chemical stimulation of the bowel evokes sensory responses in the ENS and central nervous system have progressed, including critical elements that underlie

Through the Looking-Glass: Psychoneuroimmunology and the Microbiome-Gut-Brain Axis in the Modern Antiretroviral Therapy Era.

OBJECTIVE: Depression, substance use disorders, and other neuropsychiatric comorbidities are common in people with HIV (PWH), but the underlying mechanisms are not sufficiently understood. HIV-induced damage to the gastrointestinal tract potentiates residual immune dysregulation in PWH receiving effective antiretroviral therapy. However, few studies among PWH have examined the relevance of microbiome-gut-brain axis: bidirectional crosstalk between the gastrointestinal tract, immune system, and central nervous system. METHODS: A narrative review was conducted to integrate findings from 159 art

Disorders of gut-brain interaction through the lens of polyvagal theory.

This paper introduces a metric capable of tracking a hypothetical brainstem "switching" mechanism involved in regulating the afferent influence of blood pressure on the vagal efferent control of heart rate. In theory, this metric could be applied to evaluate the "efficiency" of brainstem pathways involved in common mechanisms of autonomic function involving the vagal influences on the gut as well as the heart. Thus, by exploring the dynamic "efficiency" of the brainstem feedback circuit linking heart rate to posture, a clinically relevant index of vagal flexibility might be extracted that woul

Clinical Perspectives on Sleep-Pain Interactions.

Sleep disturbances and chronic pain frequently co-occur and interact through complex, bidirectional pathways that span neurobiological, psychological, and behavioral domains. This work synthesizes current evidence on the mechanisms linking sleep and pain, such as neuroinflammation, autonomic imbalance, impaired pain modulation, circadian disruption, and cognitive-affective processes. Longitudinal and microlongitudinal studies consistently demonstrate that poor sleep is a robust predictor of pain onset, escalation, and chronification, while pain also meaningfully disrupts sleep continuity and q

Do the organs of the labyrinth differentially influence the sympathetic and parasympathetic systems?

It has long been recognized that the vestibular system plays a major role in autonomic control. The nature of this control remains in dispute, however, as some evidence points to a vestibularly mediated parasympathetic activation, whereas other evidence points to a sympatho-excitatory role for labyrinthine outputs. A theoretical explanation is offered that attempts to resolve this issue by postulating that the utricles exert a predominantly sympatho-excitatory influence via their interactions with brain noradrenergic pathways, while the semicircular canals (and possibly saccules) increase para

Interleukin-6 blockade improves autonomic dysfunction in rheumatoid arthritis.

Autonomic nervous system (ANS) involvement in rheumatoid arthritis (RA) is well recognised and contributes to arrhythmia and sudden death. However, there is no study documented the therapeutic efficacy on autonomic neuropathy (AN) in RA. This is the first reported observation of improvement in AN with interleukin-6 (IL-6) blockade with tocilizumab in RA. We report a case of 61-year old female with seropositive RA with severe disease activity, investigated for autonomic neuropathy. A battery of non invasive tests was used for accurate assessment of AN function based on assessment of peripheral

Toward autonomous artificial intelligence agents in sports science: a modular framework for development, validation, and implementation.

Despite widespread artificial intelligence (AI) adoption in sports science for predictive analytics, current systems operate as passive tools requiring continuous human monitoring and intervention at every decision point. Autonomous AI agent systems capable of 24/7 monitoring, independent reasoning, and proactive action execution remain academically unexplored in sports science contexts. Unlike passive analytics that await human analysis or conversational interfaces requiring explicit prompting, autonomous agents operate continuously, detecting patterns and implementing interventions without h

Mechanistic Interpretation of Beat-to-Beat Cardiovascular Variability in Hypertension Using a Closed-Loop Cardiovascular Model.

Beat-to-beat heart rate variability (HRV) and blood pressure variability (BPV) are gaining attention as noninvasive markers of autonomic dysfunction, particularly, for improving hypertension management. However, interpretation is complicated by coexisting autonomic dysregulation and vascular mechanical alterations in hypertension. This study used a numerical model to disentangle these contributions, which cannot be isolated in vivo. A closed-loop mathematical model was developed to simulate cardiovascular variability during head-up tilt (HUT), incorporating four heart chambers, systemic and pu

Does maternal heart rate variability mediate the association between postpartum depressive symptoms and preterm neonate heart rate variability (ProMote study)?

We investigated whether maternal heart rate variability (HRV) mediates the association between maternal postpartum depressive symptoms and preterm infant autonomic regulation in the early postnatal period. Data derived from 82 mothers and 97 preterm infants (gestation<37 weeks) (Mean [SD] gestational age (GA): 33.4 [2.15] weeks). We assessed preterm neonate HRV within 24 h after birth (T1), on days 3-4 (T2), and for neonates born before 35 weeks GA, at 35-36 weeks postmenstrual age (PMA) (T3). Maternal HRV measurements were carried out between the 3rd and 6th day after birth. HRV features incl

Directional Brain Connectivity Changes Induced by Heart Rate Variability Biofeedback: A Spectral Dynamic Causal Modeling Study.

Heart rate variability biofeedback (HRVB) is a non-pharmacological intervention that trains individuals to breathe at their cardiovascular resonance frequency to enhance autonomic regulation. While HRVB improves cardiovascular function across clinical conditions, the directional neural pathways through which it modulates the central autonomic network (CAN) remain poorly understood. Using a publicly available open-access dataset, we conducted a secondary analysis of effective (directional) neural connectivity (EC) data from 32 healthy participants randomized to an 8-week HRVB intervention (n =

Multimodal assessment of autonomic dysfunction in amyotrophic lateral sclerosis.

BACKGROUND AND PURPOSE: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disorder with predominant progressive degeneration of motor neurons and motor deficits, but non-motor symptoms (NMS) such as cognitive and behavioural deficits are frequent and underestimated in current diagnostic pathways. Autonomic dysfunction has occasionally been described, although its frequency and relevance are unclear. The aim of this study was to investigate the role of the autonomic nervous system in ALS using a multimodal approach. METHODS: Thirty-seven ALS patients and 40 healthy sex- and age-matche

Cold pressor test in spinal cord injury-revisited.

STUDY DESIGN: Systematic review. OBJECTIVES: A spinal cord injury (SCI) commonly results in alterations of cardiovascular physiology. In order to investigate such alterations, the cold pressor test (CPT) has been used as an established challenge test. This review summarizes the basic physiology underlying a CPT, discusses potential mechanisms responsible for abnormal pressor responses following SCI, and highlights the utility of CPT in the SCI population. SETTING: Canada and Switzerland. METHODS: We have completed a comprehensive review of studies that have investigated the effect of foot o

Harlequin syndrome in a patient with putative autoimmune autonomic ganglionopathy.

The author presents a patient with Harlequin and Horner syndromes as part of an autoimmune autonomic ganglionopathy and suggests implication for work-up and management. In general, Harlequin and Horner syndromes are reported to be caused by either a structural lesion of the sympathetic pathway or, when no structural lesion is found, are presumed to be idiopathic. In this paper, a 76 year old man developed a Harlequin and Horner syndromes in the setting of subacute autonomic failure and other systemic features. The patient's symptoms improved with a short course of intravenous methylprednisolon

Functional connectome of brainstem nuclei involved in autonomic, limbic, pain and sensory processing in living humans from 7 Tesla resting state fMRI.

Despite remarkable advances in mapping the functional connectivity of the cortex, the functional connectivity of subcortical regions is understudied in living humans. This is the case for brainstem nuclei that control vital processes, such as autonomic, limbic, nociceptive and sensory functions. This is because of the lack of precise brainstem nuclei localization, of adequate sensitivity and resolution in the deepest brain regions, as well as of optimized processing for the brainstem. To close the gap between the cortex and the brainstem, on 20 healthy subjects, we computed a correlation-based

Autonomic dysreflexia after spinal cord injury: central mechanisms and strategies for prevention.

Spinal reflexes dominate cardiovascular control after spinal cord injury (SCI). These reflexes are no longer restrained by descending control and they can be impacted by degenerative and plastic changes within the injured cord. Autonomic dysreflexia is a condition of episodic hypertension that stems from spinal reflexes initiated by sensory input entering the spinal cord caudal to the site of injury. This hypertension greatly detracts from the quality of life for people with cord injury and can be life-threatening. Changes in the spinal cord contribute substantially to the development of this
