Endogenous pH shifts facilitate spreading depression by effect on NMDA receptors.

Rapid extracellular alkalinizations accompany normal neuronal activity and have been implicated in the modulation of N-methyl-D-aspartate (NMDA) receptors. Particularly large alkaline transients also occur at the onset of spreading depression (SD). To test whether these endogenous pH shifts can modulate SD, the alkaline shift was amplified using benzolamide, a poorly permeant inhibitor of interstitial carbonic anhydrase. SD was evoked by microinjection of 1.2 M KCl into the CA1 stratum radiatum of rat hippocampal slices and recorded by a proximal double-barreled pH microelectrode and a distal

Subcortical nuclei and glymphatic system alterations in migraine with aura: a comparative 7T multimodal MRI analysis.

BACKGROUND: Although cortical spreading depression is well-established in migraine with aura (MA), the potential involvement of subcortical nuclei and the glymphatic system (GS) in its pathogenesis—and their utility in distinguishing MA from migraine without aura (MO)—remains poorly investigated. Therefore, we employed multimodal 7T MRI—integrating functional, structural, quantitative susceptibility mapping (QSM), and GS metrics (perivascular space (PVS) morphology, choroid plexus volume, and analysis along the perivascular space (ALPS) index)—to enhance the understanding of MA and to characte

Regional cerebral blood flow (rCBF) studies in migraine and epilepsy.

Intracarotid injection of 133-Xenon and recording of wash out of radioactivity by 254 external stationary detectors was used to measure rCBF. Initial slope values were calculated by a computer, translated into color code and displayed on a TV-screen. rCBF in patients with epilepsy has been the object of former studies. Those who had a cortical focus showed corresponding high blood flow values during attack and during EEG paroxysms without clinical attacks. On the contrary, all other patients with a cortical focus were normal. Patients with complex partial seizures displayed no abnormalities in

Endocannabinoid Modulation in Headache: Mechanisms, Models, and Translational Therapies.

Headache disorders, including migraine, tension-type headache, trigeminal autonomic cephalalgias, post-traumatic headache and medication overuse headache, represent a major global health burden and remain difficult to treat despite therapeutic advances. The endocannabinoid system (ECS) has emerged as a key regulator of neural, vascular, and immune processes central to headache pathophysiology. Through coordinated actions of CB1 and CB2 receptors, the endogenous ligands anandamide (AEA) and 2-arachidonoylglycerol (2-AG), and their metabolic enzymes, the ECS modulates trigeminovascular activity,

Neural Circuit Dysfunctions in Migraine: A Stage-Specific Dynamic Perspective.

Migraine is increasingly recognized as a cyclical brain state disorder rather than an isolated vascular or nociceptive condition. Its clinical phases may reflect dynamic reorganization of interacting neural circuits involved in sensory processing, homeostasis, pain modulation, and cognition. This review aims to synthesize current evidence and propose a stage-specific neural circuit dynamic model of migraine, linking each clinical phase to distinct patterns of circuit dysfunction and recovery. A narrative review of studies published between January 2010 and December 2024 was conducted using Pub

Polyunsaturated fatty acid supplement alleviates depression-incident cognitive dysfunction by protecting the cerebrovascular and glymphatic systems.

INTRODUCTION: Depression, the most prevalent mood disorder, has high comorbidity with cerebrovascular disease and cognitive decline. However, there is little understanding of the cellular mechanisms involved in depression and its comorbid cerebrovascular damage and cognition impairment. Here, we tested the prediction that the chronic unpredictable mild stress (CUMS) mouse model would manifest in disturbed glymphatic function and that dietary supplementation with polyunsaturated fatty acids (PUFA) could ameliorate these deficits while alleviating the depression-associated cognitive decline. Classical migraine: symptoms between visual aura and headache onset.

The gap between the end of the visual aura and headache onset in classical migraine has been called the free interval. In a retrospective study of twenty-five migraineurs who had noted a gap, only three reported feeling normal at that time: twenty-two described alterations in mood, detachment from the environment or other people, fears, disturbances of speech or thought, or somatic symptoms. The interval lasted less than an hour in seventeen of the twenty-two but in five persisted for 1 to 5 hours. These symptoms suggest involvement of the frontal and temporal cortices as well as the hypothala

Changes of migraine aura with advancing age of patients.

AIM: Given the similar presentation of migraine aura and acute ischemic stroke, advancing patient age might change the characteristics of migraine with aura (MA) and be clinically important. Clinical data, however, are limited. Experimental studies indicate a decrease in the magnitude of cortical spreading depression (CSD), the pathophysiological correlate of migraine aura, with advancing age. Our study aimed to assess the influence of age on the clinical features of MA. METHODS: Three hundred and forty-three patients were interviewed using a structured questionnaire. The questions covered th

[Migraine as a cerebrovascular risk factor].

INTRODUCTION: Migraine has become an important vascular risk factor during the past few years, along with the presence of white matter and clinically silent ischaemic lesions. Whether these findings contribute to the migraine becoming chronic has been a source of debate. People with chronic migraine also have a less favourable metabolic profile. An exhaustive review of the literature has been made in order to try to clarify the relationship between migraine and vascular risk factors. DEVELOPMENT: Migraine, particularly with aura and in women < 45 years-old, is associated with an increased ris

A new method to model electroconvulsive therapy in rats with increased construct validity and enhanced translational value.

Electroconvulsive therapy is the most effective therapy for major depressive disorder (MDD). The remission rate is above 50% in previously pharmacoresistant patients but the mechanisms of action are not fully understood. Electroconvulsive stimulation (ECS) in rodents mimics antidepressant electroconvulsive therapy (ECT) in humans and is widely used to investigate the underlying mechanisms of ECT. For the translational value of findings in animal models it is essential to establish models with the highest construct, face and predictive validity possible. The commonly used model for ECT in roden

Simultaneous investigation of the neuronal and vascular compartments in the guinea pig brain isolated in vitro.

We describe a new method for studying the interactions between vascular tone changes and neuronal activity in the arterially perfused isolated brain of the adult guinea pig maintained in vitro. Electrophysiological recordings were performed in the piriform and entorhinal cortices with the entire arterial bed preserved or after vascular restriction to the territories of median and posterior cerebral arteries of one hemisphere. The changes in vascular tone were measured by means of a pressure transducer. The arterial pressure was 53.77+/-12.74 mmHg in control conditions at 30 degreesC. Intralumi

Effects of somatostatin, a somatostatin agonist, and an antagonist, on a putative migraine trigger pathway.

OBJECTIVE: To determine whether somatostatin (SST) could be a cortico-brainstem neurotransmitter involved in producing the headache of migraine. BACKGROUND: There is evidence to support the idea that a cortico-brainstem-trigeminal nucleus neuraxis might be responsible for producing migraine headache; we have suggested that SST may be one of the neurotransmitters involved. METHODS: Rats were anesthetised and prepared for recording neurons in either the periaqueductal gray matter (PAG) or nucleus raphe magnus (NRM), as well as the trigeminal nucleus caudalis (TNC). Involvement of pro-nociceptive 5-HT2A receptor in the pathogenesis of medication-overuse headache.

OBJECTIVES: To determine the involvement of 5-HT(2A) (5-HT(2A)) receptor in the process of trigeminal plasticity induced by chronic analgesic exposure and in the process of inflammatory-induced thermal hyperalgesia. BACKGROUND: Derangement in 5-HT(2A) serotonin receptor has been reported to implicate in pathogenesis of medication-overuse headache. No clear explanation concerning the precise roles of these receptors in the process. METHODS: Wistar rats were daily administered with paracetamol (200 mg/kg) for 30 days. On the next day, ketanserin, a 5-HT(2A) antagonist, or saline was given prio

Mapping cortical change in Alzheimer's disease, brain development, and schizophrenia.

This paper describes algorithms that can identify patterns of brain structure and function associated with Alzheimer's disease, schizophrenia, normal aging, and abnormal brain development based on imaging data collected in large human populations. Extraordinary information can be discovered with these techniques: dynamic brain maps reveal how the brain grows in childhood, how it changes in disease, and how it responds to medication. Genetic brain maps can reveal genetic influences on brain structure, shedding light on the nature-nurture debate, and the mechanisms underlying inherited neurobeha

Implications of high homocysteine levels in migraine pain: An experimental study of the excitability of peripheral meningeal afferents in rats with hyperhomocysteinemia.

OBJECTIVES: Investigation of chronic homocysteine action on the excitability and N-methyl-D-aspartate (NMDA) sensitivity of the peripheral trigeminovascular system of rats. BACKGROUND: Migraine is a neurological disease that affects 15%-20% of the general population. Epidemiological observations show that an increase of the sulfur-containing amino acid homocysteine in plasma-called hyperhomocysteinemia-is associated with a high risk of migraine, especially migraine with aura. In animal studies, rats with hyperhomocysteinemia demonstrated mechanical allodynia, photophobia, and anxiety, and hig

Octadecylamine-Modified CuO NPs Enabling Highly Selective In Vivo Ascorbic Acid Potentiometric Detection with Enhanced Sulfide Tolerance.

Although currently reported in vivo ascorbic acid (AA) sensors based on galvanic redox potentiometry (GRP) feature good selectivity toward biological molecules, the interference from sulfides (e.g., H2S) is still a great challenge. Herein, we demonstrate a novel shielding strategy for highly selective AA assay without the interference from H2S. The electrode was constructed by confinement of octadecylamine (ODA)-modified CuO nanoparticles (NPs) on a carbon fiber electrode (CFE), in which CuO NPs rapidly convert H2S into CuS, minimizing its electrochemical redox contribution, while the ODA laye

A critical review of the neurovascular nature of migraine and the main mechanisms of action of prophylactic antimigraine medications.

INTRODUCTION: Migraine involves neurovascular, functional, and anatomical alterations. Migraineurs experience an intense unilateral and pulsatile headache frequently accompanied with vomiting, nausea, photophobia, etc. Although there is no ideal preventive medication, frequency in migraine days may be partially decreased by some prophylactics, including antihypertensives, antidepressants, antiepileptics, and CGRPergic inhibitors. However, the mechanisms of action involved in antimigraine prophylaxis remain elusive. AREAS COVERED: This review recaps some of the main neurovascular phenomena rel

Update on noninvasive neuromodulation for migraine treatment-Vagus nerve stimulation.

Noninvasive neurostimulation methods are particularly suited for migraine treatment thanks to their most favorable adverse event profile. Among them, noninvasive vagus nerve stimulation (nVNS) has raised great hope because of the role the vagus nerve is known to play in pain modulation, inflammation and brain excitability. We will critically review the clinical studies performed for migraine attack treatment and migraine prevention with the GammaCore® device, which allows cervical vagus nerve stimulation. nVNS is effective for the abortive treatment of migraine attacks, although the effect siz

Migraine with aura: which patients are most at risk of stroke?

The complex association between migraine (M) and ischemic stroke (IS) is discussed. Epidemiological studies and meta-analyses show that M with aura (MA) and not M without aura, doubles the risk of IS. The risk is higher for female gender, young age and higher headache attacks frequency. Smoking habit and oral contraceptives, especially if associated, increase stroke risk. The underlying pathogenetic mechanisms are not completely understood, but it is hypothesized that a particular brain susceptibility to cortical spread depression could explain the association between MA and IS. The absolute r

Magnesium and Migraine.

Migraine is a widespread and intricate neurological condition that involves various factors and is marked by recurring headache episodes. Migraine is among the ten neurological conditions accounting for the greatest disability in the whole population, the leading cause of disability for children and adolescents aged 5-19 years, and the second cause of disability for adults aged 20-59 years. Magnesium deficiency is also a very common condition resulting from diverse reasons, including insufficient dietary consumption or increased loss through the gastrointestinal or renal system.
