OBJECTIVE: Chiari malformation type I (CM-I) involves the herniation of the cerebellar tonsils through the foramen magnum. CM-I is associated with both obstructive sleep apnea (OSA) and central sleep apnea (CSA) in children. The primary management of symptomatic CM-I remains surgical decompression. There is, however, a paucity of data evaluating the efficacy of decompression surgery on outcomes related to sleep-disordered breathing (SDB). The objective of this study was to evaluate SDB outcomes, specifically the need for respiratory support following decompression in pediatric patients with CM

BACKGROUND AND OBJECTIVES: Surgical decompression is commonly required to relieve symptoms of cervical radiculopathy and allow for functional recovery. There are only a handful of studies analyzing return-to-work (RTW) outcomes after cervical spine surgery for cervical radiculopathy. This study seeks to elucidate RTW outcomes and to identify predictors preventing RTW in patients surgically treated for cervical radiculopathy in a Swedish nationwide prospective registry.
METHODS: A nationwide cohort analysis was conducted using prospectively gathered data from the Swedish Spine Registry. All pa

INTRODUCTION: Global evidence on the epidemiology of prevalent diving-related injuries (DRI) different from decompression sickness (DCS) and other fatalities is lacking. This study aimed to perform a comprehensive review of DRIs in the year-period between 2010-2020 in a non-hyperbaric tertiary hospital in the Spanish Mediterranean coast, in addition to identifying patient risk factors for severe middle ear barotrauma.
METHODS: The study was conducted via a retrospective review of medical records during a 10-year period (2010-2020) at the University and Polytechnic Hospital La Fe (UPHLF) of Va

During underwater vehicle escape training with compressed air, a fit 26-year-old soldier suffered pulmonary barotrauma with cerebral arterial gas embolism after surfacing from a depth of 0.75-1.2 metres of freshwater or less. She presented with an altered level of consciousness. Rapid neurological examination noted slurred speech, a sensory deficit and right hemiparesis. Eleven hours after the accident, hyperbaric oxygen treatment was initiated using US Navy Treatment Table 6. The soldier almost completely recovered after repeated hyperbaric oxygen treatment. Given the very shallow depth this

BACKGROUND: Inside attendants working in hyperbaric chambers are exposed to risks related to the hyperbaric environment, handling and care. The aim of this study is to review the literature focusing on the impact of this activity on health.
MATERIALS AND METHODS: This is a literature review using the Medline database.
RESULTS: Eight articles studied decompression illness (DCI). The incidence of DCI ranged from 0 to 37 per 100,000 sessions in hyperbaric chambers (SHC). The incidence of injuries ranged from 0 to 412 per 100,000 SHC. The most prevalent cause of accidental death was fire: 77 dea

Hyperbaric oxygen therapy is widely accepted as life-saving treatment for decompression illness. Yet its use in acute carbon monoxide poisoning has remained controversial because of inconsistent findings in clinical trials. Hyperbaric oxygen therapy has an adjunctive role in managing gas gangrene, necrotising soft-tissue infection, and crush injury, as supported by case series. Several cases have been reported in the literature detailing the use of hyperbaric oxygen therapy in patients with severe anaemia in whom blood transfusion is not possible. Today, use of hyperbaric oxygen therapy in Hon

The foramen ovale is a slit-like anatomical structure located in the interatrial wall of the fetal heart that enables right-to-left shunting during fetal development. Although this hole generally closes completely shortly after birth due to shifting pressures in the atrial chambers, it remains open, or 'patent', in about 25% of cases representing a potential substrate for right-to-left shunting during adult life. A patent foramen ovale (PFO) is usually haemodynamically insignificant, even when large, but is the most common cause of right-to-left shunt. Large-diameter PFOs may act as a pathway

Medical issues in sport diving include illnesses that are caused by diving, and medical disorders that compromise safety. Cerebral air embolism and decompression sickness of the brain and spinal cord can result from diving. Sport divers may manifest a spectrum of symptoms from air embolism, which can range from unconsciousness to minimal symptoms, which include fatigue, personality change, poor concentration, irritability, and changes in vision. The physician must search for these minor symptoms in divers who are suspected of pulmonary barotrauma. Medical disorders of concern in diving include

We tested 1002 persons, 572 divers (including 38 who had diving accidents) and 430 nondivers, for defects of the blue-sensitive system during the years 1988 and 1989. Recent research has shown functional disorders of color vision in divers, linked to previously described alterations of the retinal capillary system and pigment epithelium. The desaturated Lanthony-15-HUE test was used as a screening method. No defect of the blue-sensitive system was found, either in divers or in nondivers. Our results are similar to those of other researchers. One exception was that we found no correlation betwe

The purpose of the study was to determine if a near-maximum exposure to air at increased atmospheric pressure causes gross fetal malformations, decreased birth weight, or death when administered to pregnant sheep during peak development of the embryo. Twenty-eight timed-pregnant sheep were alternately assigned to a series of 25- or 30-minute exposures at either 4.6 atmospheres absolute or surface pressure between days 12 and 40 of gestation. About day 130 of pregnancy, 11 experimental and eight control fetuses were recovered, weighed, measured, fixed, and examined for defects. No major structu

BACKGROUND: Glossopharyngeal neuralgia (GPN) is an uncommon craniofacial pain syndrome. Although rare, it is occasionally associated with cardiac syncope, as vago-glossopharyngeal neuralgia (VGPN).
CASE DESCRIPTION: We present the case of a 73-year-old man with VGPN misdiagnosed as trigeminal neuralgia. The patient was diagnosed with sick sinus syndrome, and a pacemaker was introduced. However, syncope still recurred. Magnetic resonance imaging revealed a branch of the right posterior inferior cerebellar artery contacting the root exit zone of the right glossopharyngeal and vagus nerves. We d

Ulnar nerve compression at the elbow (UNE) frequently affects people of working age. Surgically treated patients may not immediately return to work (RTW) postoperatively. In 2008, the Swedish Social Insurance Agency reformed the national insurance policy. We aimed to examine RTW postoperatively for UNE, variations among surgical methods, and potential risk factors for prolonged RTW (sick leave > 6 weeks). Surgically treated cases of UNE (n = 635) from two time periods (2004-2008 and 2009-2014) and two healthcare regions (Southern and South-eastern) were studied retrospectively regarding age, s

INTRODUCTION: Biomarker responses to intensive decompression indicate systemic proinflammatory responses and possible neurological stress. To further investigate responses, 12 additional brain and lung biomarkers were assayed.METHODS: A total of 15 healthy men (20 to 50 yr) undertook consecutive same-day ascents to 25,000 ft (7620 m), following denitrogenation, breathing 100% oxygen. Venous blood was sampled at baseline (T0), after the second ascent (T8), and next morning (T24). Soluble protein markers of brain and lung insult were analyzed by enzyme-linked immunosorbent assay with plasma micr

BACKGROUND AND OBJECTIVES: Video-based learning has improved educational outcomes in medicine and nursing. However, in the field of commercial diving, despite the broad range of knowledge required, the effectiveness of license renewal and continuing education systems has not been sufficiently examined. This study evaluated the short-term effects of a video-based educational intervention on OSH knowledge and learning motivation among commercial divers and examined its usefulness as a CE tool.
METHODS: A web-based quasi-experimental single-group pre-post intervention study was conducted between

INTRODUCTION: Dysbaric osteonecrosis (DON) is a type of aseptic bone necrosis of long bones such as the humerus, femur, and tibia. It is observed in workers who perform in high-pressure environments.
METHODS: There were 58 volunteer divers included in this study who had performed at least 500 dives, were working as a dive master or instructor, had never performed industrial and commercial dives, and did not have a diagnosis of osteonecrosis. Radiological evaluation was performed according to the guidelines suggested by The British Research Council Decompression Sickness Panel. A total of eigh

INTRODUCTION: Doppler ultrasound measurements have been recorded since the 1970s across the world and provide a valuable data resource for learning, analysis, and potential training of deep learning algorithms to recognise and grade venous gas emboli (VGE) allowing assessment of decompression sickness (DCS) risk.
METHODS: We collected a 'big database' of Doppler recordings and associated metadata. Audio tapes with recorded Doppler data were converted to digital files, then cut into individual recordings and matched with their metadata, including subject and pressure profile information. The a

INTRODUCTION: This study's aim was to determine the number of scuba tank fills done in Victoria, Australia from 1 July 2024 to 30 June 2025 to provide an estimate of the number of scuba dives conducted during that period and, from that, estimates of the fatality and decompression illness rates.
METHODS: Suppliers of compressed gas for scuba diving in Victoria were identified through internet searches, industry liaison and the Australasian Diving Safety Foundation records. Those identified were emailed an invitation to participate in the tank fill survey and provided with dedicated spreadsheet

Breath-hold diving exposes humans to repeated episodes of profound hypoxia and hypercapnia, eliciting physiological adaptations that enable prolonged underwater performance. This article summarises current knowledge on chronic adaptations in elite breath-hold athletes and traditional diving populations, including the Bajau sea nomads of Southeast Asia and the Korean Haenyeo divers. Evidence indicates that repeated apnoea induces adaptations across multiple physiological systems. Haematological changes include increased spleen size and enhanced splenic contraction, augmenting circulating haemog
