BACKGROUND: The recommendation of Anderson and colleagues to bone graft even relatively minor amounts of comminution (a third of the bone diameter) in the treatment of diaphyseal forearm fractures with plate-and-screw fixation has been questioned. This study examines factors related to nonunion in adult patients with comminuted diaphyseal fractures of both the radius and the ulna to determine the relative influence of bone grafting.
METHODS: Over a 15-year period at two hospitals, 41 patients with diaphyseal fractures of both forearm bones satisfied the following criteria: comminution (Orthop

Data from the Child Research Council (Denver, CO) were analyzed to model longitudinal growth changes in the humerus, radius, femur, and tibia in 31 boys and 36 girls between 3 and 10 years of age. Multilevel modeling of growth changes allowed efficient estimates of bone size and bone growth variation to be obtained as well as comparisons of growth patterns within and between limbs. The long bones displayed decelerating growth through time, with greater velocities for the larger lower limb (vs. smaller upper limb) bones and the larger proximal (vs. smaller distal) elements within limbs. Sports-related tibial diaphyseal fractures occur mainly in contact sports and in downhill skiing. Due to the demand for rapid healing and the need for an early recovery of the mostly young patients, surgery is almost imperative. The clinical outcome and sports ability of 38 patients with sports-related tibial diaphyseal fractures were investigated 29 months after the initial trauma. Soccer and wintersport accounted for the majority of injuries. Operative treatment has been mainly unreamed nailing. After an average of 10.3 months 32 patients (84 %) returned to sports. Almost one-third, however

PURPOSE OF THE STUDY: Diaphyseal aneurysmal bone cyst is very rare. We report a case in a 20-year-old female.
PATIENT AND METHOD: The clinical signs developed rapidly, within one month. The lesion was first curetted and grafted with cortico-cancellous bone. Recurrence was observed 6 months later. Complete excision and reconstruction with a free fibular vascularized grafted enabled healing.
DISCUSSION: The pathogenesis of aneurysmal bone cysts is debated. We hypothesize that it is due to the opposed direction of periosteal and medullary blood circulation. This could explain the relative rarit

From January 1982 through December 1988, 150 patients with 153 Winquist Class III and IV comminuted diaphyseal femur fractures due to high energy blunt trauma were treated with immediate plate fixation. A total of 260 major general surgical systems were injured in 150 patients. Forty-nine patients did not have adequate preoperative spine radiographs due to positioning or time factors. Nineteen patients had spine fractures; nine were diagnosed post-femoral fixation. The average injury severity score (ISS) was 22.7. Three patients died (2%). Our institution predicted mortality with this ISS for

130 septic diaphyseal pseudoarthroses (SDP) were treated over a period of 10 years, 58 in the context of a war situation and 72 in civilian practice. The two etiologies differed in that the size of the initial associated cutaneous lesion was much greater in battle injuries. A staged treatment procedure was applied in 110 cases: treatment of the infection, followed by osteogenic grafts and depending on the case, stabilisation of the assembly or replacement of internal by external fixation materials. More recently, the use of muscular or musculocutaneous pedicle flaps has enabled, in a single pr

Our clinical experience with different constructions of interposition and arthrodesis nails, used in 3 and 12 patients respectively, is presented. It has shown that cemented intramedullary nailing for the stabilization of long bones and the knee joint using an approach from the site of the defect is a useful additional means in reconstructive bone and joint surgery.

OBJECTIVE: To investigate the effect of high versus low trauma energy on the risk of medical and surgical complications after intramedullary nailing of femoral diaphyseal fractures, and to determine the overall rates of these complications.
DESIGN: Retrospective cohort study.
SETTING: Level 1 trauma center.
PATIENTS: The study included 279 patients with 289 fractures who underwent primary surgery with a locked reamed rigid intramedullary nail for diaphyseal femoral fractures between May 2007 and May 2022 at a Level 1 trauma center in Finland, with follow-up assessments organized within the

BACKGROUND: Diaphyseal-engaging stems are conventionally used in conversion total hip arthroplasty (cTHA) after prior intramedullary nailing (IMN) to bypass screw holes and previously instrumented meta-diaphyses. However, it remains unclear if cementless metaphyseal or standard cemented stems can also provide satisfactory fixation in this scenario. This study compared the early outcomes of cTHA using three femoral stem designs: metaphyseal cementless, diaphyseal cementless, and standard cemented.
METHODS: A retrospective review of 80 patients who underwent cTHA after prior IMN at a single aca

Background and Objectives: Pediatric forearm fractures are among the most common childhood injuries. COVID-19-related societal restrictions, including school closures and suspension of sports activities, altered children's daily routines and may have influenced injury patterns. This study aimed to evaluate whether periods of stricter COVID-19 restrictions were associated with changes in the incidence of pediatric distal and diaphyseal forearm fractures after accounting for seasonal variation and long-term temporal trends. Materials and Methods: This retrospective observational time-series stud

BACKGROUND: This study investigated the rate and degree of deformity in femoral diaphyseal nonunion using preoperative bilateral CT-scans.
METHODS: A retrospective cohort study was conducted at a single Level 1 trauma center. Patients aged > 18 years who underwent operative treatment for femoral diaphyseal nonunion between 2015 and 2025 and had preoperative bilateral femur CT-scans were included. Axial, sagittal, coronal, and length differences between the femoral nonunion and the contralateral healthy femur were measured. Torsional deformity ≥ 15° and length discrepancy ≥ 20 mm were consider

Mega-endoprostheses enable wide management options in the treatment of primary and periprosthetic fracture of the lower extremities. In this study, we report on the use of custom-made subtotal diaphyseal endoprosthetic replacement in treatment of interprosthetic femoral fracture. This procedure is off-label, but in this particular case, it was the safest and most stress-stable treatment option. The implant was delivered within three weeks. The surgical procedure for subtotal replacement of the femoral diaphysis was performed without any intra- or postoperative complication. The duration for th

We describe a trifocal femur injury with intracapsular femoral neck fracture, diaphyseal fracture with bone loss, and distal complete articular (AO/OTA C type) fracture, an injury rarely described in the literature. Surgical management utilized a not-yet-reported implant combination: screw-side plate device for the intracapsular femoral neck, retrograde nail for the diaphysis, and lag screws plus mini fragment buttress plating for the distal fracture. The patient had uneventful fracture union with no changes in alignment. Given the rarity and complexity of this injury, there is little consensu

BACKGROUND: Forearm shaft fractures are common injuries, often caused by falling from a fully-upright position or falling off a bike. They can be treated nonoperatively or surgically with intramedullary nailing or plates. The method of choice for treating pediatric forearm shaft fractures is the application of elastic stable intramedullary nailing (ESIN)|. The aim of the study was to evaluate ESIN in pediatric patients with forearm shaft fractures based on radiological images, and determine the etiology and complication rate associated with the injury.
METHODS: The study included 201 patients

INTRODUCTION: In treating nonunion, the removal of a broken nail is often difficult. We report a technique for removing a broken Kuntscher intramedullary nail. In this technique, a chisel is inserted into the lumen of the intramedullary nail, and the nail is rotated.
CASE PRESENTATION: A 57-year-old man presented to our hospital with femoral diaphyseal nonunion lasting 17 years after injury. The patient suffered a right femoral diaphyseal fracture at 40 years old and was treated with a Kuntscher intramedullary nail. The patient underwent two additional surgeries at another hospital for the no

PURPOSE: This study aimed to evaluate the effect of distal blocking screws on the stability and healing of ulnar diaphyseal fractures treated with intramedullary (IM) nails. The primary research question was whether the addition of distal blocking screws enhanced fracture stabilization and promoted faster healing than the standard IM nailing techniques.
METHODS: This retrospective study reviewed medical records of 30 patients with ulnar diaphyseal fractures treated from February 2018 to September 2023. The patients were divided into two groups: those treated using IM nails alone (n = 17) and

Diaphyseal clavicle fractures occur most frequently in adolescents (defined as ages 10 to 19 years by the World Health Organization). Multiple prospective comparative studies exist in the adult literature, whereas studies focusing on adolescents are limited. Given the notable differences in healing potential between pediatric, adolescent, and adult diaphyseal clavicle fractures, treatment algorithms tailored specifically to children, adolescents, and adults are required. In the past two decades, there has been a dramatic rise in surgical fixation of adolescent diaphyseal clavicle fractures, la
