Advancing drug development with "Fit-for-Purpose" modeling informed approaches.

Model-informed Drug Development (MIDD) is an essential framework for advancing drug development and supporting regulatory decision-making. The current review presents a strategic blueprint to closely align MIDD tools with key questions of interests (QOI), content of use (COU), and model impact across stages of development -from early discovery to post-market lifecycle management. To demonstrate how the strategy works, we have also provided examples of how the MIDD tools can be applied to enhance the target identification, assist with lead compound optimization, improve preclinical prediction a

A standardizable human-based psoriasis skin model for drug development.

Psoriasis is a chronic inflammatory autoimmune disease with a prevalence of 2-3% in the western population. For the development of urgently needed new therapeutic options, standardizable and human-relevant disease models are essential. The transcription factor signal transducer and activator of transcription 3 (STAT3) plays a key role in the pro-inflammatory response of psoriatic keratinocytes. In this study, we established STAT3 overexpressing human keratinocytes and combined the cells with our epidermis and immune cell supplemented full-thickness skin model to generate a novel in vitro psori

Development of novel pegylated imidazobenzodiazepines as inhaled asthma drug candidates.

In this work, we identified a novel asthma drug candidate that upon nebulized administration reduced airway hyperresponsiveness in A/J mice comparable to the beta agonist albuterol. We identified this compound among synthesized pegylated imidazodiazepines with substituent variations in two positions. These compounds combine good affinity towards the gamma aminobutyric acid (A) receptors (GABAARs) expressed in the lungs and a polyethylene glycol (PEG) chain for excellent aqueous solubility to allow nebulization. As expected, the aqueous solubility of these derivatives increased with the PEG cha

Ursolic Acid Conjugates: A New Frontier in Anticancer Drug Development.

New Ursolic Acid (UA) conjugates were synthesized using optimized synthetic protocols through the molecular hybridization approach at C-3 and C-28. This resulted in the targeted molecules being produced in good yields. Some of the synthesized conjugates showed significantly relevant bioactivity against mammalian cells and in animal models of cancers. Selected UA conjugates were tested against bladder and breast cancer cell lines. The conjugates showed moderate to significantly enhanced antiproliferative activities against Triple Negative Breast Cancer (TNBC; MDA-MB 231), which is an aggressive

Drug-Induced sleep endoscopy in children: NAVOTEL scoring system development.

OBJECTIVES: Pediatric drug-induced sleep endoscopy (DISE) lacks a universal and easy-to-use scoring system. The velum, oropharynx, tongue, epiglottis (VOTE) scoring system is widely used but needs to be completed in pediatrics. The main objective of this study was to investigate the distribution of obstructive sites in DISE and to propose an appropriate pediatric scoring system. The secondary objective was to evaluate the changes in surgical management induced by the proposed scoring system. METHODS: A single-center prospective 5-year study was conducted from March 2016 to December 2021, incl

Exploiting gene dependency to inform drug development for multiple myeloma.

Despite recent advances in therapy, multiple myeloma essentially remains an incurable malignancy. Targeting tumour-specific essential genes, which constitute a druggable dependency, potentially offers a strategy for developing new therapeutic agents to treat MM and overcome drug resistance. To explore this possibility, we analysed DepMap project data identifying 23 MM essential genes and examined the relationship between their expression and patient outcome in three independent series totalling 1503 cases. The expression of TCF3 and FLVCR1 were both significantly associated with progression-fr

Health Equity in Pediatric Drug Development: Translating Aspiration into Operation.

The concept of health equity-the attainment of the highest possible level of health for all members of society-requires equitable access to all aspects of healthcare, including pediatric drug development. However, many communities are under-represented in pediatric drug development programs. Barriers to participation include geographic, economic, racial/ethnic bias, legal, cultural, linguistic, and other factors. While there is no "one size fits all" approach to addressing these barriers, community engagement and collaboration is recognized by the Centers for Disease Control, the World Health

Pyrazolopyridine: An efficient pharmacophore in recent drug design and development.

Among the various heterocyclic molecules employed for drug design and discovery, pyrazolopyridine is one of the promising pharmacophores. Pyrazolopyridine is a result of fusion of pyrazole and pyridine rings. The potent pharmacology of pyrazolopyridine may be the synergistic effect of pyrazole and pyridine moieties in a single framework. It has been used in drug design of a wide range of diseases such as anticancer, antimicrobial, anti-inflammatory, and neuroprotection. Cancer has become a common disease among elderly people now a days that might be because of genetic inheritance to some exten

Antileishmanial Drug Discovery and Development: Time to Reset the Model?

Leishmaniasis is a vector-borne parasitic disease caused by Leishmania species. The disease affects humans and animals, particularly dogs, provoking cutaneous, mucocutaneous, or visceral processes depending on the Leishmania sp. and the host immune response. No vaccine for humans is available, and the control relies mainly on chemotherapy. However, currently used drugs are old, some are toxic, and the safer presentations are largely unaffordable by the most severely affected human populations. Moreover, its efficacy has shortcomings, and it has been challenged by the growing reports of resista
