Biomechanical characteristics of osteochondral defects of the humeral capitellum.

BACKGROUND: The repetitive, excessive compression forces in the radiocapitellar joint caused by elbow valgus stresses during throwing motions can result in osteochondritis dissecans (OCD) of the humeral capitellum in adolescent athletes. PURPOSE: To assess the effect of elbow valgus torque on contact pressure in the radiocapitellar joint and that of central and lateral capitellar osteochondral defects on radiocapitellar joint contact pressure, elbow valgus laxity, and ulnar collateral ligament (UCL) strain. STUDY DESIGN: Controlled laboratory study. METHODS: In 8 matched pairs of fresh-froz

Elbow Fractures.

The elbow joint consists of the humeroulnar, humeroradial, and proximal radioulnar joints. Elbow stability is maintained by a combination of static and dynamic constraints. Elbow fractures are challenging to treat because the articular surfaces must be restored perfectly and associated soft tissue injuries must be recognized and appropriately managed. Most elbow fractures are best treated operatively with restoration of normal bony anatomy and rigid internal fixation and repair and/or reconstruction of the collateral ligaments. Advanced imaging, improved understanding of the complex anatomy of

Complications and range of motion of patients with an elbow dislocation treated with a hinged external fixator: a retrospective cohort study.

PURPOSE: Elbow dislocations are at risk for persistent instability and stiffness of the joint. Treatment with a hinged external fixation provides elbow joint stability, and allows early mobilization to prevent stiffness. Mounting a hinged elbow fixator correctly, however, is technically challenging. The low incidence rate of elbow dislocations with persistent instability suggests that centralization would result in higher surgeon exposure and consequently in less complications. This study aimed to investigate the results of treatment of elbow dislocations with a hinged elbow fixator on the rat

Sliding external fixator "a product design and cadaveric experiment".

BACKGROUND: External fixation spanning a joint like the elbow, while maintaining joint mobility, is a well-established practice, and it could be done with a variety of external fixation systems. In current systems, correct identification of the elbow center of rotation under X-ray guidance with lateral views is mandatory. If the center of rotation of the fixator is not aligned with that of the elbow joint, the assembly will not work. This new design idea aims to propagate the principle of sliding external fixation applied on the extensor side of a joint, with the limbs of the fixator being abl

The relationship between changes in elbow valgus laxity and forearm flexor strength during repetitive pitching.

This study examined the changes in the medial elbow joint space width and forearm flexor strength due to repetitive pitching and analysed the relationships among these factors. Thirty-one collegiate baseball players pitched seven sets of 15 pitches each. The medial elbow joint space width, grip strength, and forearm flexor strengths were measured before pitching and after each set. These parameters were compared at baseline and after pitching for each set. Additionally, the relationships between changes in the medial elbow joint space and forearm flexor strength were examined for each set. The

Relation between glenohumeral internal rotation deficit and valgus laxity of the elbow in high school baseball pitchers.

BACKGROUND: There are few literatures describing the association between glenohumeral internal rotation deficits (GIRD) and valgus laxity of the elbow on the throwing side of high school pitchers with and without a history of elbow symptom. METHODS: The passive range of motion of glenohumeral internal and external rotation on bilateral side shoulders was measured in 75 high-school baseball pitchers during the preseason. Using ultrasonography, the width of the ulnohumeral joint of the elbow was measured at 30° of flexion, both at rest and with valgus stress in the pitching side. Comparisons of

The role of the flexor pronator muscles as dynamic stabilizers against elbow valgus stress in patients with medial ulnar collateral ligament insufficiency.

BACKGROUND: The flexor pronator muscles (FPMs) have been reported to act as dynamic stabilizers against valgus forces in overhead-throwing athletes. Several studies have demonstrated the anatomic, biomechanical, and clinical effects of the FPMs. However, no studies have investigated the in vivo kinematics of the FPMs against the valgus forces on the elbow. This study aimed to clarify the clinical contribution of the FPMs as dynamic stabilizers in medial ulnar collateral ligament (MUCL) insufficiency. METHODS: Eighteen baseball players with MUCL injury participated in this study. The elbow was

Thrombosis of the brachial artery - a rare and devastating complication after a simple closed posterolateral elbow dislocation.

Dislocation of the elbow joint is the second most common dislocation after the shoulder joint. Although this pathology is relatively common, concomitant vascular injuries are rare. We present a case of a patient who sustained a fall on his arm with an outstretched elbow that resulted in a closed simple posterolateral elbow dislocation and delayed thrombosis of the brachial artery followed by two revascularisation surgeries. The physician must always maintain a high index of suspicion for a concomitant vascular injury before and after closed reduction of the elbow joint and have in mind that co
