Bilateral symptomatic hypertrophic ligamentum teres of the hip in a young athlete.

We report the case of a 14-year-old female gymnast who presents with bilateral hip pain and mechanical symptoms with extremes of range of motion. The patient failed conservative management and had worsening symptoms, including pain at rest. Diagnostic imaging failed to provide any definitive diagnosis and the patient underwent staged bilateral hip arthroscopies with debridement of inflamed, hypertrophic ligamentum teres. The patient did well postoperatively and has returned to athletic activities.

[Avascular necrosis of the capitate in athletes].

The possibility of osteonecrosis of the carpal bones should always be considered when athletes present with pain of unknown origin in the hand and wrist, in particular, if they are participating in sports such as gymnastics or weight-lifting that involve extreme loading of the wrist with axial compression and microtrauma. This sort of extreme loading of the wrist combined with a constitutionally "weak" blood supply to the individual carpal bones may lead to the formation of osteo-necrotic zones. A treatment method that can produce excellent results, depending on the pathomorphology, is availab

[Functional disorders of the rectum and pelvic floor. Ambulatory/conservative therapy].

Early diagnosed functional disorders of the ano-rectum or pelvic floor can be treated in the office by conservative treatment or out-patient surgery. The normal anatomy of the anal canal must be restituted by reducing enlarged haemorrhoids, removing a prolapse of the anterior rectal wall with rubber-band ligation, excision of chronic fissures and prolapsing tumours. The basic therapy then consists of normalisation of bowel habits and stool consistency. An anal stenosis must be dilated. Training of the sphincter, gymnastics of the pelvic floor, electrostimulation and biofeedback are the therapy

Distal Adductor Magnus Avulsion Fracture Treated with Excision After Failure of Nonoperative Management: A Case Report.

CASE: In this case report, we discuss a skeletally immature gymnast who presented with an isolated distal femoral adductor tubercle avulsion fracture that went on to develop a painful nonunion. After the failure of extensive nonoperative intervention, she had resolution of her symptoms with bony excision and soft-tissue repair. CONCLUSION: It is important for clinicians to be aware of this atypical injury pattern and that surgical intervention may be necessary because conservative treatment may not always resolve symptoms.

Spine Kinematics Behavior During the Handstand Posture: A Biplanar Radiographic Analysis.

Background/Objectives: The handstand is an exercise performed in many sports, either for its own sake or as part of physical training. Unlike the upright bipedal standing posture, little is known about the sagittal alignment and balance of the spine during a handstand, which may hinder coaching and reduce the benefits of this exercise if not performed correctly. The purpose of this study was to quantify the sagittal alignment and balance of the spine during a handstand using radiographic images to characterize the strategies employed by the spino-pelvic complex during this posture. Methods: Ni

Sport training and the growth and pubertal maturation of young athletes.

Growth and pubertal maturation of youth athletes are overwhelmingly within the broad range of normal variability. These include many successful male athletes who mature earlier than average, and many successful female athletes, especially in the "aesthetic" sports (artistic gymnastics, ice-skating, ballet) and distance running, who mature later than average. Artistic gymnasts generally receive the most attention, but their growth and maturation characteristics fall largely within the physiologic range and are indistinguishable from short, late maturing girls. It is difficult to ascribe causali

Bilateral olecranon epiphyseal fracture non-union in a competitive athlete.

Olecranon epiphyseal stress fractures and epiphyseal non-unions have been described in throwing athletes, weight lifters and gymnasts. We present a case in which bilateral olecranon epiphyseal fractures were diagnosed in a competitive NCAA Division One wrestler who presented with chronic elbow pain. Given the rigors and physical demands of collegiate wrestling, we present a novel technique for open reduction internal fixation, grafting and supplementation with BMP for accelerated healing and return to competition.

[The hyperventilation syndrome: the treatment problems].

The principal methods of the treatment of the hyperventilation syndrome (HVS) are reviewed. The following methods of the action on the patient are distinguished: psychotherapy, pharmacotherapy, remedial gymnastics, correction of respiratory disorders according to the principle of the biological feed-back mechanism, recurrent respiration or carbon dioxide inhalations. Emphasis is laid on the importance of an individual approach to the institution of the treatment measures. The relationship between the HVS and respiratory diseases is discussed. The authors advance an opinion that every patient m

Surfer's myelopathy: A review of etiology, pathogenesis, evaluation, and management.

Context: Surfer's myelopathy (SM) is an acute syndrome identified by nontraumatic paraparesis or paraplegia. Though traditionally tied to first-time surfers, the condition encompasses any activity involving hyperextension of the back such as gymnastics, yoga, and Pilates. Methods: MEDLINE® and Google Scholar literature searches were gathered to identify relevant case reports for determining the etiology, pathogenesis, evaluation, and management of SM.Results: While the rare nature of SM limits its full understanding, studies have pinpointed that hyperextension in the back leads to vasculature i

Incidence of elbow dislocations in the United States population.

BACKGROUND: There is minimal published information regarding the epidemiology of simple elbow dislocations. The purpose of this study was to report the estimated incidence of elbow dislocations in the United States, with use of the National Electronic Injury Surveillance System (NEISS) database. METHODS: The NEISS database includes 102 hospitals representing a random sampling of all patients presenting to U.S. emergency departments. The database was queried for elbow dislocation events. NEISS data for 2002 through 2006 were used for raw data and weighted injury counts. Incidence rates with 95

Trends in wrist fractures in children and adolescents, 1997-2009.

PURPOSE: Distal radius and carpal fractures in children and adolescents represent approximately 25% of all pediatric fractures. Incidence rates and causes of these fractures change over time owing to changes in activities and risk factors. The purpose of this study was to examine recent population-based trends in incidence and causes of wrist fractures in children and adolescents. METHODS: We obtained data from the Dutch Injury Surveillance System of emergency department visits of 15 geographically distributed hospitals, and from the National Hospital Discharge Registry. This included a repre

The effect of a specific strength-development exercise on bone mineral density in perimenopausal and postmenopausal women.

The objective of this study was to assess the effect of physical activity on bone density at the distal radius in healthy perimenopausal (23) and postmenopausal (60) women. The 83 women, aged 40-62 years, were randomized into two groups. The women participated in an exercise program of 40 minutes of jogging and 20 minutes of gymnastics three times a week for 1 year (group 1, n = 46) or 6 months (group 2, n = 37), respectively. Subjects in group 2 served as controls during the first 6 months. They were introduced to exercise classes after the first 6 months of the study and wore wrist weights (

The management of ulnocarpal abutment and degenerative triangular fibrocartilage complex tears in the competitive athlete.

Ulnar-sided wrist pain is a frequent cause for loss of practice time and competitive play for athletes. Ulnocarpal abutment, a common source of ulnar-sided pain, typically burdens athletes who participate in gymnastics, racket sports, and baseball. Although many athletes respond to nonoperative management, surgical intervention should be considered when symptoms persist. Surgical options include arthroscopic debridement, arthroscopic wafer, open wafer, or ulnar-shortening osteotomy. Treatment should be tailored to the athletes' level of function, expectations, and goals. The timing of interven

Epiphyseal Fracture in Sports.

In brief In this evaluation of 203 acute epiphyseal fractures in the long bones of 183 patients aged 3 to 18, 79 fractures (39%) were related to the following ten sports activities: football, hockey, basketball, downhill skiing, baseball/softball, wrestling, soccer, gymnastics, tennis, and volleyball. An additional 21 fractures occurred in weight lifting, roller skating, ice skating, and skateboarding. Approximately 50% of the fractures in this urban population were sports- related. The symptoms of epiphyseal fractures are the same as symptoms of fractures or ligament injuries in general. Phys

[Influence of considerable athletic training on the foot condition of young athletes at a boarding school with a cross-section of sports].

The present day sport inevitably causes enormous physical loading on the locomotor system of the sportsman, and in particular on his foot. To obtain best results in any sport is closely connected with participation of children in systematic purposive training, therefore a necessity arises to study the influence of different time-limited loadings on the locomotor system--foot of young sportsmen. The work represents the data on changes of different parameters of the foot under time-limited loading for 18-30 h per week of the children attending a boarding-school of sporting type, which is a new t

Wrist injuries: a comparison between high- and low-impact sports.

Wrist injuries can be categorized as those caused by high- or low-impact sports. High-impact sports include auto racing, motorcross or bicycle racing, in-line skating, gymnastics, football, soccer, ice skating, snowboarding, and alpine skiing. Low-impact sports include tennis, track and field, and golf. High-impact injuries of the wrist range from displaced fractures and dislocations to ligamentous and acute tendinous tears. Low-impact sports typically result in nondisplaced or occult fractures, contusions, stress reaction, ligamentous sprain, tendinopathy, tenosynovitis, or tendon subluxation
