Profound Vagal Tone and Bradycardia Mimicking Asystole: A Resuscitation Case Report.

A 48-year-old man presented with dizziness. When he arrived at the emergency department, he collapsed and became pulseless. Prior to his collapse, he was asymptomatic and now even participated in multiple marathon and ultra-running events per year. However, he previously experienced a vasospastic inferior STEMI eight years prior from cocaine use. As a result, he had an ischaemic cardiomyopathy with LVEF of 45%. He never took any further illicit substances after the STEMI; instead, he changed his lifestyle completely and commenced extreme endurance sports. After one hour of alternations between

THE MANAGEMENT OF ILIOTIBIAL BAND SYNDROME WITH A MULTIFACETED APPROACH: A DOUBLE CASE REPORT.

BACKGROUND AND PURPOSE: Iliotibial Band Syndrome (ITBS) has commonly been thought of as an overuse injury in runners. The exact etiology of ITBS is not well understood and there is no consensus on how to properly manage it. The purpose of this case series is to present a comprehensive model that utilizes a review of the current literature and the concept of regional interdependence as a foundation for the treatment of ITBS in runners. CASE DESCRIPTIONS: The first subject was a 36-year old female, referred from an orthopedic physician with the diagnosis of left iliotibial band friction syndrom

Psychological skills and mindfulness for distance runners: a systematic review of performance, psychological outcomes, and implications for coaching practice.

BACKGROUND: Contemporary athlete support increasingly recognizes that performance preparation extends beyond physical training to the regulation of attention, effort, emotions, and behavior. Psychological skills training (PST) and mindfulness-based approaches offer different strategies for developing these capacities, yet their value for distance runners and their relevance to applied coaching remain uncertain. This systematic review examined runner-specific evidence for these interventions, distinguishing direct running performance from psychological and other performance-related outcomes. M

Control of Protein and Energy Metabolism in the Pituitary Gland in Response to Three-Week Running Training in Adult Male Mice.

It is assumed that crosstalk of central and peripheral tissues plays a role in the adaptive response to physical activity and exercise. Here, we wanted to study the effects of training and genetic predisposition in a marathon mouse model on mRNA expression in the pituitary gland. Therefore, we used a mouse model developed by phenotype selection for superior running performance (DUhTP) and non-inbred control mice (DUC). Both mouse lines underwent treadmill training for three weeks or were kept in a sedentary condition. In all groups, total RNA was isolated from the pituitary gland and sequenced

Exercise-induced changes in hemostasis markers in marathon runners: effects of enzyme supplementation and determinants.

BACKGROUND: Increases in hemostatic markers, indicators of thrombotic risk, have been reported following strenuous exercise; however, determinants of these responses remain poorly understood. This study aimed to determine whether supplementation with hydrolytic enzymes and flavonoids (Wobenzym® [WOB]) modulates exercise-induced hemostatic responses in marathon runners and to identify factors associated with these changes. METHODS: This predefined subanalysis of the Enzy-MagIC trial was conducted within a prospective, randomized, double-blind, placebo-controlled trial. Participants were random

Sex, training variables, history of chronic disease, and chronic injury are risk factors associated with a history of exercise-associated muscle cramping in 10,973 ultramarathon race entrants: a safer XXXVIII study.

BACKGROUND: The prevalence of a history of exercise-associated muscle cramping (hEAMC) among ultramarathon runners is high. While the Comrades is one of the most popular mass community-based participation ultramarathons (90 km) globally, research on the epidemiology, clinical characteristics, and risk factors of entrants' lifetime hEAMC are scarce. This research aimed to describe the epidemiology, clinical characteristics, and risk factors of hEAMC among Comrades Marathon entrants. METHODS: This was a retrospective, cross-sectional study in which 10973 race entrants of the 2022 Comrades Marat

Effect of Single High-Dose Vitamin D3 Supplementation on Post-Ultra Mountain Running Heart Damage and Iron Metabolism Changes: A Double-Blind Randomized Controlled Trial.

Exercise-induced inflammation can influence iron metabolism. Conversely, the effects of vitamin D3, which possesses anti-inflammatory properties, on ultramarathon-induced heart damage and changes in iron metabolism have not been investigated. Thirty-five healthy long-distance semi-amateur runners were divided into two groups: one group received 150,000 IU of vitamin D3 24 h prior to a race (n = 16), while the other group received a placebo (n = 19). Serum iron, hepcidin (HPC), ferritin (FER), erythroferrone (ERFE), erythropoietin (EPO), neopterin (NPT), and cardiac troponin T (cTnT) levels wer

Consecutive Bilateral Iliac Stress Fracture in an Adult Male Runner.

Iliac stress fractures (ISF) are uncommon in sports, particularly among runners, and are attributed to repetitive loading and other predisposing factors. ISF poses diagnostic challenges due to nonspecific symptoms and the limited sensitivity of conventional imaging procedures. The reported case is about a 51-year-old male marathon runner with consecutive bilateral ISF. Initial symptoms included mechanical pain in the right buttock, leading to a diagnosis confirmed via pelvic MRI. Conservative management with eight weeks rest from sport activity was indicated with symptom resolution and return

Lower extremity alignment and risk of overuse injuries in runners.

A group of 304 runners enrolling in a marathon training program had alignment measurements performed and completed a questionnaire on training practices and injuries over the previous 12 months. The alignment measures consisted of arch index (AI), heel valgus (HV), knee tubercle-sulcus angle (TSA), knee varus (KV), and leg-length difference (LLD). Results indicated few consistent statistical associations between these alignment measures and risk of injuries, either bivariately or multivariately: left AI with hamstring injuries; right AI with shin injuries; right HV with back injuries; left TSA

Effects of genotype and exercise on hepatic gene expression in phenotype-selected marathon mice.

Experimental evolution through selection experiments is a vital tool for exploring the special features of polygenic traits. Here, we provide liver transcriptome data from a paternally selected marathon mouse model (DUhTP), characterized by exceptional running performance, and unselected controls (DUC), both descended from the same polygenic background. To study training responses, both lines were either challenged by three weeks of high-speed treadmill training or remained sedentary. For each condition, eight animals were used for liver tissue isolation and next-generation sequencing. Raw seq

Restricted pulmonary diffusion capacity after exercise is not an ARDS-like injury.

Pulmonary diffusion capacity (DLCO) is reduced 2 h after various types of exercise, such as rowing, treadmill running, arm cranking and marathon running. The decrease in DLCO may involve alterations in the alveolar-capillary membrane as well as depletion of the central blood volume. We hypothesized that the reduction in DLCO might also be influenced by oxygen free radicals, acute phase proteins and endotoxin, which are also involved in the adult respiratory distress syndrome (ARDS). Ten competitive male oarsmen performed a 6 min 'all-out' ergometer row. Single breath DLCO was determined before
