Local anaesthetic block therapy of posttraumatic neuralgia of the infraorbital nerve.

The pathogenesis, as well as the results of treatment in 12 patients with neuralgia of the infraorbital nerve are discussed. Patients were treated by means of local anaesthetic nerve blocks, or when this was unsuccessful, by transcutaneous nerve stimulation and or tricyclic antidepressive drugs. In this way, the results were good in previously untreated patients, while they were moderate in patients in whom an attempt to treat with surgery had been carried out earlier. We recommend the use of local anaesthetic nerve blocks as treatment of the first choice.

Association Between Maxillary Segmentation and Perioperative Complications in Le Fort I Osteotomy.

The Le Fort I osteotomy is used to reposition the maxilla to correct numerous maxillofacial and occlusal deformities. The aim of this study was to delineate perioperative complication rates associated with Le Fort I osteotomy and determine whether the number of maxillary segments or bone grafting yielded increased complication rates. Patients undergoing Le Fort I osteotomy from 2012 to 2019 were identified from the multi-institution "National Surgical Quality Improvement Program" database using Current Procedure Terminology codes. The predictor variables of interest included maxillary segmenta

Automated diagnosis for extraction difficulty of maxillary and mandibular third molars and post-extraction complications using deep learning.

Optimal surgical methods require accurate prediction of extraction difficulty and complications. Although various automated methods related to third molar (M3) extraction have been proposed, none fully predict both extraction difficulty and post-extraction complications. This study proposes an automatic diagnosis method based on state-of-the-art semantic segmentation and classification models to predict the extraction difficulty of maxillary and mandibular M3s and possible complications (sinus perforation and inferior alveolar nerve (IAN) injury). A dataset of 4,903 orthopantomographys (OPGs),

Anatomical study of critical features on the posterior wall of the maxillary sinus: clinical implications.

OBJECTIVES/HYPOTHESIS: Endonasal approaches to the pterygopalatine fossa for epistaxis or as part of a transpterygoid approach have been popularized somewhat by the ability to control the terminal branches of the maxillary artery (MA). Surgical landmarks are useful to identify these arteries. Therefore, identification of surface features on the posterior wall of the antrum that reflect the position of deeper structures within the pterygopalatine fossa would help predict anatomical position and orientation. STUDY DESIGN: Describe the incidence of identifiable markings on the posterior wall of

Is palatal injection mandatory prior to extraction of permanent maxillary tooth: a preliminary study.

INTRODUCTION: Even after the invention of the modern injection techniques, palatal injection still remains a painful experience for patients, and this pain is attributed to the presence of rich nerve complement and displacement of palatal mucosa during anesthesia. OBJECTIVE: The aim of the present study was to demonstrate if lidocaine HCl could provide palatal anesthesia if given buccally during maxillary tooth removal without the need for a palatal injection. MATERIALS AND METHODS: The study group consisted of 75 patients, and 25 were controls. All the patients in the study group had unilat

Abnormal facial development in the mouse mutant first arch.

The first arch mutation in mice, far, causes specific severe craniofacial defects, different from those caused by other craniofacial mutations. The purpose of the present study was to define the facial defects in far homozygotes, to identify heterozygote expression, if any, and to investigate the embryogenesis of the defect. A genetic study and a developmental study were done. The genetic study showed that only far homozygotes, observed on day 18 of gestation, have cleft palate, pointed snout, and deficiency and disorganized pattern of maxillary vibrissae. Approximately 75% have open eye(s). I

Use of bone compressors and atraumatic bone elevators in crestal sinus lift.

Atrophy of the alveolar processes of the jaws, as a result of edentulism, often cannot allow a prosthetic rehabilitation, also if it only consists of a total removable prosthesis - for the inadequate base support - and also if it is backed by implants, because the small residual thickness bone compromises the primary implant stability, a fundamental requirement for a correct osseointegration. In addition to atrophy itself, the presence of noble structures (maxillary sinus, inferior alveolar nerve) make an implant-prosthetic rehabilitation contraindicated. To obviate this problem, there are man

Infra-orbital nerve schwannoma: Report and review.

Extra-cranial schwannomas although common in head and neck region are very rarely seen originating from the infra-orbital nerve. We report a case of schwannoma arising from infra-orbital nerve in a 40-year-old male patient. The case presented as an isolated, asymptomatic, slow growing sub-cutaneous nodular swelling over left side of mid-face. On ultrasonography, a localized lesion within the sub-cutaneous tissue of cheek was observed, without involvement of orbital, maxillary sinus or underlying bone. Aspiration biopsy of the lesion showed spindle shaped cells predominantly arranged in Antoni

Facial canal decompression leads to recovery of combined facial nerve paresis and trigeminal sensory neuropathy: case report.

BACKGROUND: Trigeminal sensory neuropathy is often associated with facial idiopathic nerve paralysis (Bell's palsy). Although a cranial nerve viral polyneuropathy has been proposed as the usual cause, in many instances the etiology remains unclear. This case report of recovery of both trigeminal and facial neuropathy after surgical decompression of the facial nerve suggests an anatomic link. METHODS: A case of a 39-year-old woman presenting with recurrent unilateral facial paralysis is summarized. Her fifth episode, which did not spontaneously recover, was associated with retroorbital and max

Infraorbital Nerve Decompression for Chronic Post Traumatic Neuralgia: A Novel Approach and Review of the Safety and Efficacy of Piezoelectric Surgery on Soft Tissues and Nerves.

Chronic post traumatic infraorbital nerve (ION) hyperesthesia is a rare complication of orbital floor fractures. Surgical decompression of the ION has been reported to relieve chronic post traumatic infraorbital neuralgia. This case report describes a novel approach for ION decompression in a patient who suffered from chronic infraorbital neuralgia associated with a healed displaced orbital floor fracture. We used an intraoperative 3D image-guided navigation system to accurately localize the fracture site and employed piezoelectric surgery for nerve decompression. We further review the literat

The Anterolateral Triangle: Implications for a Transnasal Prelacrimal Approach to the Floor of the Middle Cranial Fossa.

BACKGROUND: The anterolateral triangle enclosed by the foramen rotundum and foramen ovale constitutes part of the floor of the middle cranial fossa (MCF). OBJECTIVE: To assess the feasibility of a transnasal prelacrimal approach for accessing the floor of MCF via an anterolateral triangle corridor and to determine the extent of maximal exposure while safeguarding neurovascular structures. METHODS: A transnasal prelacrimal approach was performed in 5 cadaveric specimens (10 sides). Following the identification of foramen rotundum and foramen ovale, the bony ridge between 2 was drilled to expo

SnapShot: Orofacial Sensation.

Ophthalmic, maxillary, and mandibular branches of the trigeminal nerve provide sensory innervation to orofacial tissues. Trigeminal sensory neurons respond to a diverse array of sensory stimuli to generate distinct sensations, including thermosensation, mechanosensation, itching, and pain. These sensory neurons also detect the distinct sharpness or pungency of many foods and beverages. This SnapShot highlights the transduction ion channels critical to orofacial sensation.

Perineural extension of facial melanoma.

A 64-year-old man presented with a pigmented cutaneous lesion on the right side of his face along with right facial numbness. Histological examination revealed malignant melanoma. Magnetic resonance imaging (MRI) revealed perineural extension along the entire course of the maxillary division of the right trigeminal nerve. This is a rare but important manifestation of the spread of head and neck malignancy.

[Microsurgical anatomy of the transoval percutaneous route to the trigeminal cave and the trigeminal ganglion].

The percutaneous Hartel transoval route goes through an inverted pyramid, with an inferior summit and a superior base that includes three compartments. The danger of the inferior compartment is the parotid duct. The middle compartment contains many branches of the mandibular nerve. The superior compartment is crossed by the internal maxillary artery and its branches, as well as the auditory tube. The base of the pyramid presents not only the foramen ovale, but also the foramen lacerum, where the trocar may injure the internal carotid, and the foramen jugulare, where the trocar may meet the int

Quantitative characterization of optic nerve atrophy in patients with multiple sclerosis.

BACKGROUND: Optic neuritis (ON) is one of the most common presentations of multiple sclerosis (MS). Magnetic resonance imaging (MRI) of the optic nerves is challenging because of retrobulbar motion, orbital fat and susceptibility artifacts from maxillary sinuses; therefore, axonal loss is investigated with the surrogate measure of a single heuristically defined point along the nerve as opposed to volumetric investigation. OBJECTIVE: The objective of this paper is to derive optic nerve volumetrics along the entire nerve length in patients with MS and healthy controls in vivo using high-resolut
