Long-term effect of presurgical nasoalveolar molding on growth of maxillary arch in unilateral cleft lip and palate: randomized controlled trial.

The objective of this study was to investigate the long-term effect of presurgical nasoalveolar molding (PNAM) on growth of the maxillary arch through early childhood until 6 years of age in complete unilateral cleft lip and palate (UCLP) patients presenting for PNAM at different ages. Complete UCLP patients who were treated at our centre were divided into two groups. The study group underwent PNAM and was further subdivided into three subgroups (PNAM initiated within 1 month, between 1 and 6 months, and between 6 and 12 months of age in subgroup I, II, and III, respectively). The control grou

Changes in nose symmetry in unilateral cleft lip and palate treated by differing pre-surgical assistance: An objective assessment of primary repair.

BACKGROUND: Residual deformity of the nose, not lip, continues to be the greater challenge in UCCLP rehabilitation. Platform distortions often re-emerge following primary reconstruction revealing the stereotypical cleft-nose. Nasal alveolar molding reduces nose asymmetry. However, this study applies directional mechanics to the underlying platform distortions and soft tissue nose, introducing a novel device addressing the distorted septo-premaxillary junction. METHODS: Retrospective assessment of 47 UCCLP patients by 2-dimensional photographic analysis with 24 subjects treated by dento-maxill

Trends in prosthodontic treatment of cleft palate patients at one institution: a twenty-one year review.

This 21-year retrospective study evaluated the trends observed in the prosthodontic treatment of 269 patients. The patients were divided into three groups. The speech aid group consisted of 203 patients and showed a significant decrease in numbers over time. The maxillary obturator group consisted of 24 patients and did not show a significant change. The infant feeding prosthesis group consisted of 42 patients and showed a significant increase in numbers. Possible reasons for the trends observed are discussed.

Cleft size at the time of palate repair in complete unilateral cleft lip and palate as an indicator of maxillary growth.

Cleft size at the time of palate repair might affect the difficulty of surgical repair and, thus, indirectly postoperative maxillary growth. This retrospective study aimed to determine whether a correlation existed between the cleft size at the time of palate repair and the growth of the maxilla. Maxillary dental casts of 39 infants with non-syndromic complete unilateral cleft lip and palate, taken at the time of palate repair, were used to measure cleft size. Cleft size was defined as the percentage of the total palatal area. The later growth of the maxilla was determined using lateral and po

ATTITUDE OF CLEFT CARE SPECIALISTS IN AFRICA TOWARDS PRESURGICAL ORTHOPAEDICS.

OBJECTIVE: To determine the attitude of cleft care specialists working in Africa towards pre-surgical orthopaedics. DESIGN: A cross sectional study. SETTING: Pan-African conference on cleft lip and palate in Nigeria. SUBJECTS: Sixty cleft care specialists working in Africa. RESULTS: The general attitude towards pre-surgicalorthopaedics was positive. Majority of the specialists employ pre-surgical orthopaedics before surgical repair. Fifty four (91%) of the specialists use plaster strapping for cases of bilateral cleft, five (8.3%) use nasoalveolar molding and one (2%) use feeding plate. Ha

Esthetic and functional rehabilitation of a bilateral cleft palate patient with fixed prosthodontic therapy.

UNLABELLED: Patients with bilateral cleft lip and palate present with multiple challenges to the prosthodontist. Some of them include mobility of the premaxillary segment, multiple missing and malposed anterior teeth, unfavorable soft tissues, and a tense maxillary lip. This clinical report describes the fixed prosthodontic management of a bilateral cleft lip and palate patient with a surgically corrected lip and a mobile premaxillary segment. The patient presented with an 11-unit metal-ceramic fixed partial denture made of a base metal alloy that was made 25 years ago. He had multiple porcela

Conversion of an interim into a definitive speech aid: a predictable approach.

In patients with partial or total soft palatectomy, an interim prosthesis generally is inserted as soon as possible after surgery. Over the average postoperative healing period of 3 months, numerous clinical appointments are needed to shape the prosthesis. When the definitive prosthesis is completed, reshaping the pharyngeal portion of the speech aid unnecessarily involves duplication of work by the clinician. A laboratory procedure is described that can save clinic time, increase cost-effectiveness, and shift work normally performed by the maxillofacial prosthodontist to the maxillofacial lab

Assessment of oral health-related quality of life outcomes: A preliminary study to compare conventional and digital impressions in obturator fabrication for patients with maxillectomy defects.

OBJECTIVE: The objective of this study was to compare oral health-related quality of life (OHRQoL) as measured by the University of Washington Quality of Life Scale Version 4 (UW-QOL v4) among patients with maxillectomy defects resulting from COVID-19-associated mucormycosis (CAM). The comparison focused on patients whose maxillary obturators were fabricated using conventional or digital impression techniques. METHODS: Twenty patients with maxillary defects requiring oral rehabilitation with obturators were alternately assigned to two groups. Conventional impressions using stone models were t

Palatal Mucormycosis in An Infant.

The maxilla rarely undergoes necrosis due to its rich vascularity. Maxillary necrosis can occur due to bacterial infections, viral infections, or fungal infections. Mucormycosis is an opportunistic fulminant fungal infection that mainly infects immunocompromised patients. The fungus invades the arteries, leading to thrombosis that subsequently causes necrosis of hard and soft tissues. The occurrence of mucormycosis is not considered rare in the jaws of adults, but involvement of the maxilla in infants is not usually seen. The purpose of this report is to discuss the diagnosis and management of

[Implant-borne prosthesis for an edentulous maxilla with a large maxillectomy defect--a step-wise therapy concept].

Due to the great variety of maxillectomy defects standardized restorative treatment protocols are rarely documented. For the practitioner it is from highest importance to consider the specific defect morphology and, even more important, the individual needs of affected patients. The presented case shows the planning and realization of the restoration of a maxilla with an extended resection defect following tumor surgery. Because the demanding patient remained edentulous after extraction of the residual teeth for periodontal reasons, an implant retained obturator prosthesis was inserted. Follow

[Implant-supported prostheses for the edentulous patient after partial maxillectomy--clinical considerations in the choice of retention elements--2 case reports].

The prosthetic rehabilitation of edentulous patients following hemimaxillectomy proves to be functionally unsatisfactory in many cases because of a heavily changed oral morphology and oral communication with the nasal and maxillary sinus cavities, which makes it difficult to achieve a sufficient retention and stability of the restoration using a conventional obturator denture. Using implants as anchorage increases comfort considerably for patients wearing such restorations. Due to the wide variety of the individual anatomy of the defect area and the residual parts of intact maxilla clinicians

Philtrum formation in cleft lip surgery.

This is a modification of Millard's rotation-advancement principle in closing unilateral lip clefts, published in 1971. In selected cases, if there is enough soft tissue at the base of the columella to fill the defect between the columella base and the rotated short part of the philtrum, a slightly curved scar will be formed resembling the normal philtral column. From 1967 till the end of 1981, out of a total number of 1305 unilateral cleft lip repairs, 189 were operated in this manner using the described modification of Millard's technique with good results. For most of the other cases and es

Surgery of the cleft lip and nose--the GOStA approach.

The technique for primary correction of the cleft lip and nose, as practised by The North Thames Cleft Lip and Palate Centre at Great Ormond Street Hospital/St Andrew's Centre (GOStA) has evolved over 28 years of personal experience. It is an amalgamation of techniques developed by many others with some personal modifications. Patients are routinely audited at 5, 10, 15 and 20 years and outcomes evaluated and compared with other centres where possible. Secondary surgery of lip and nose is based on the same principles of anatomical reconstruction used in the primary surgery.

Dentinogenic Ghost Cell Tumor in an Elderly Female: A Rare Case Report.

Dentinogenic ghost cell tumor (DGCT) is an uncommon odontogenic neoplasm that accounts for less than 0.5% of all tumors with odontogenic origin. Its rarity, along with identical radiological and clinical features as other odontogenic lesions, often makes the diagnosis challenging. The current case report discusses the clinical, radiological, and histopathological features of DGCT in a 71-year-old female patient presenting with facial swelling and pain in the maxillary anterior region. Clinical examination revealed a hard, tender, localized swelling from the infraorbital to the zygomatic region

Endodontic Retreatment of Maxillary Second Molar with Four Roots.

The present communication describes the endodontic retreatment of a rare four-rooted maxillary second molar. A 25-year-old patient was referred to our dental practice requesting an apicoectomy because of continuous and permanent pain reaction six months after the first endodontic treatment. The sent radiograph demonstrated three filled root canals (one mesial, two distal) and four radiographically superimposing roots (two mesial, two distal). Due to the diagnosed chronic apical periodontitis and based on the visible untreated root canal, we decided to endodontically re-treat the quadrangular t

Infected florid osseous dysplasia: clinical and imaging follow-up.

Florid osseous dysplasia (FOD) is a rare fibro-osseous lesion of the jaw usually identified incidentally on radiograph. It rarely presents with clinical symptoms. A case of FOD presenting with features similar to osteomyelitis is discussed here. The diagnosis is based on radiographic findings; biopsy or surgical intervention should be avoided as it may predispose to infection.
