The traditional treatment of severely impacted fractures of the distal radius involves bridging external fixation and maintaining reduction by applying continuous traction. The recent technique using fixed-angle screws within volar plates is reported restore the radial length and the articular profile whilst avoiding joint distraction. It is also believed to produce better and quicker clinical results. To test these claims, we carried out a randomised controlled comparison of the efficiency of external fixation (EF) 'versus' open reduction and internal fixation (ORIF) in treating severely impa

Background: Surface replacement arthroplasty (SRA) through a volar approach for the proximal interphalangeal (PIP) joint can preserve the integrity of the extensor tendon, which allows early range of motion (ROM) exercise postoperatively. However, a few reports have shown that the PIP ROM tends to decline with longer follow-up. The goal of this study is to assess the results of at least 5 years of follow-up of SRA through a volar approach and also to investigate the cause of deterioration of ROM with time after SRA through this approach. Methods: Eleven fingers with degenerative osteoarthritis

BACKGROUND: There are many plating systems available for treating distal radius fractures, and deciding which to use can be difficult. This prospective cohort study compared outcomes of two commonly used fixation systems: fragment-specific fixation and a fixed-angle volar locking plate system.
METHODS: Consecutive distal radius fractures were prospectively evaluated in a fragment-specific fixation cohort and a volar locking plate system cohort. Radiographic, functional, and patient-rated outcomes were collected immediately postoperatively and at 6 and 12 months postoperatively. Complications

BACKGROUND: Complications related to extensor tendons have begun to increase with the use of volar plates in the treatment of distal radius fractures. In this study, we aimed to compare four-plane radiography and ultrasonography in the evaluation of dorsal cortex screw penetration following volar plate fixation.
METHODS: We recruited 47 patients (33 males, 14 females, mean age 37.4 years; range 18-58 years). To evaluate dorsal screw penetration in all patients, we performed radiographs at 45° pronation, 45° supination and obtained dorsal tangential graphs at maximum palmar flexion, and a wris

The sagittal bands are a component of the extensor hood. They serve an important role in stabilizing the extensor tendon by forming a "check-rein" to radial-ulnar translation of the tendon over the metacarpal head, and extending the metacarpophalangeal (MCP) joint by virtue of attaching the extensor tendon to the palmar plate. Injury to the sagittal band is thought to cause extensor instability and subluxation to the contralateral side by disruption of this "check-rein" function, although recent evidence from cadaver studies suggests that ulnar sagittal band tear may be spared of extensor inst

Background Fractures or fracture dislocations of the proximal interphalangeal joint often occur during sports or accidents. Dislocations of the PIP-joint are the most common ligamentary injuries of the hand. As this kind of injury is so frequent, hand surgeons and other physicians should be aware of the correct treatment. Objectives This paper summarises the most common injury patterns and the correct treatment of PIP-joint dislocations. Materials and Methods This paper reviews the current literature and describes the standardised treatment of PIP-joint dislocations. Results What is most impor

We report a group of 14 patients with fracture dislocations of the proximal interphalangeal joint with fracture fragments of adequate size to allow reduction of the proximal interphalangeal joint and internal mini screw fixation of the bone fragment attached to the palmar plate to the base of the middle phalanx. Three years after surgery, (range 25-52 months) the average total active range of motion of the proximal interphalangeal joint was 100 degrees (range 65-115 degrees) for the acute group (operation within 14 days of injury, n=7) and 86 degrees (range 60-110 degrees) for the chronic grou

The use of the digital flexor sheath to reconstruct damaged structures in the fingers is an intriguing but under-investigated subject. The sheath is anchored firmly to the phalanges and palmar plates and has well-vascularized outer and synovial inner layers. The middle layer is strong and fibrous and not all of it is required for its main biomechanical function of maintaining the moment arm of the flexor tendons. These characteristics have led to several descriptions of different reconstructive uses. In sheath reconstruction, flaps can be used to repair damaged A2 and A4 pulleys. As an anchor,

Fractures of the ulnar styloid commonly accompany fractures of the distal radius, but their effect on the final outcome and need of separate treatment are not precisely determined. The objective of this review was to present the issue based on current literature. To date, results of both experimental and clinical studies show potentially negative effect of ulnar styloid fracture due to possible avulsion of the attachment of triangular fibro-cartilage complex and instability of the distal radio-ulnar joint. These injuries are more common when the base of the ulnar styloid is broken and consider

BACKGROUND: For some specific comminuted unstable intra-articular fracture, the plaster cast can not maintain the alignment of the articular surface effectively. The aim of this study was to evaluate the clinical effects of distal radius fracture treated with open reduction and internal plate fixation retrospectively.
METHODS: From January 2002 to March 2010, 539 cases of distal radius fracture were treated with open reduction and internal fixation, including 184 males and 355 females aging 21 - 72 years (mean 57 years). Fractures were caused by falling to the ground in 459 cases, by traffic

BACKGROUND: The use of the volar plate to treat distal radius fractures is increasing but despite the theoretical advantages of a volar approach there have been reports of extensor tendon ruptures due to prominent screw tips protruding past the dorsal cortex. The valley in the intermediate column between Lister tubercle and the sigmoid notch of the distal radius makes it difficult to rely on fluoroscopy to judge screw length. Our aim was to quantify the dimensions of this valley and to demonstrate the danger of relying on intraoperative image intensification fluoroscopy to determine lengths of

We present a case of volar rotatory subluxation of index finger proximal interphalangeal joint (PIPJ) following a skiing accident. The injury was initially perceived to be a central slip rupture and treatment was directed as such. After the initial delay in the diagnosis, the patient underwent surgery during which his radial collateral ligament was found to be avulsed from the proximal origin, the radial lateral band palmarly subluxed and was interposed in the joint space. This structure was also adhered to the uninjured volar plate.PIPJ volar rotatory subluxation could be readily missed in th

OBJECTIVE: To describe the surgical treatment of complex dorsal metacarpophalangeal dislocations, emphasising the volar approach.
METHODS: Seven cases of isolated, closed, complex dorsal metacarpophalangeal dislocation, treated surgically using the volar approach, were retrospectively evaluated. The median follow-up period was 91 months.
RESULTS: Five of the injuries involved children. The thumb was involved in four cases and the index finger in three. The volar plate was found to impede reduction in all cases. The operated joint was immobilised in a functional brace for a median of 3 weeks.

PURPOSE: Trapeziometacarpal (TMC) joint arthrodesis is an established surgical option to manage TMC joint arthritis. However, nonunion remains a complication after arthrodesis. In this study, we aimed to evaluate the outcomes of using a locking plate and headless compression screw with autogenous bone grafting.
METHODS: We identified 21 thumbs that underwent TMC arthrodesis using this procedure. Radiographs and computed tomography scans were obtained to define bone union. Furthermore, we asked patients whether they had returned to their original work at every follow-up visit. To assess object
