Distal Femoral Osteotomy and Medial Patellofemoral Ligament Reconstruction for Patellar Dislocation in Hereditary Multiple Exostoses: A Case Report.

CASE: A 44-year-old man with hereditary multiple exostoses (HME) presented with recurrent patellar dislocation of the left knee. Radiographs revealed valgus deformity and trochlear dysplasia. The patient underwent lateral opening-wedge distal femoral osteotomy with medial patellofemoral ligament reconstruction using FiberTape and knotless SwiveLock anchors. Postoperatively, patellar tracking normalized with no recurrence. CONCLUSIONS: This case highlights the need to address bony alignment and medial soft-tissue insufficiency concurrently in complex patellofemoral instability, particularly in

Total knee arthroplasty and medial patellofemoral ligament reconstruction for knee osteoarthritis with habitual patellar dislocation: a case report.

BACKGROUND: Knee osteoarthritis and habitual patellar dislocation are clinically common osteoarticular diseases. However, existing literature lacks reports on their co-occurrence and treatment approaches. Thus, we propose a surgical approach that integrates total knee arthroplasty with medial patellofemoral ligament reconstruction, addressing knee osteoarthritis alongside habitual patellar dislocation. CASE PRESENTATION: A 78-year-old female developed habitual lateral patellar dislocation of the left knee after an accidental fall over 20 years ago. One year prior, she experienced weight-beari

Simultaneous Osteochondral Fractures of the Medial Patella and Lateral Femoral Condyle Following Lateral Patellar Dislocation: A Case Report.

Patellar dislocation most commonly occurs laterally and is associated with underlying anatomical abnormalities such as trochlear dysplasia and patella alta. While osteochondral injuries typically involve the lateral femoral condyle, simultaneous osteochondral fractures of the medial patella and lateral femoral condyle are extremely rare and often under-diagnosed due to subtle imaging findings and spontaneous patellar reduction. The purpose of this report is to highlight a rare injury pattern following lateral patellar dislocation and to emphasize the importance of early diagnosis using advance

Femoral anteversion angle is more advantageous than TT-TG distance in evaluating patellar dislocation: A retrospective cohort study.

PURPOSE: The purpose of this study is to report the parameter characteristics of the femoral anteversion angle (FAA) and tibial tubercle-trochlear groove (TT-TG) distance in patients with patellar instability compared to healthy individuals and to evaluate their reliability in predicting patellar dislocation, providing potential indications for osteotomy. METHODS: A retrospective collection of consecutive patients with patellar instability constituted the study group, while individuals without patellofemoral disorder served as the control group. Measurement of the FAA and TT-TG distance were

Healing of a bone-exposed soft tissue defect after surgical correction of traumatic patellar dislocation: A case report.

RATIONALE: Reconstructive surgery is widely considered the primary treatment for soft tissue defects around the knee owing to its high flexibility. However, in our recent case study, we explored an alternative approach using decellularized collagen dressings, which proved highly effective in healing a soft tissue defect involving bone exposure following surgical correction of a traumatic patellar dislocation. PATIENT CONCERNS: A 65-year-old male with a traumatic patellar dislocation in the lower extremity failed to approximate the wound after surgical correction. The patient refused additiona

Patellar dislocation recurrence after pediatric MPFL reconstruction: Bone tunnels and soft tissues versus suture anchors and interference screw.

BACKGROUND: Several Medial Patellofemoral Ligament (MPFL) reconstruction techniques have been developed, and those with soft tissue fixation are often preferred in children because they allow the growth cartilage to be preserved. Nevertheless, the recurrence rate of patellar dislocation varies widely from one series to another, with no clear superiority of one technique in the pediatric setting. The objectives of this study were to compare the results of two tendon graft fixation techniques (tendon-tendon fixation and anchor-screw fixation) by analyzing: 1) the rate of patellar dislocation rec

A Novel Technique Utilizing a Vastus Lateralis Anchor in Treatment of Chronic Patellar Dislocation After Total Knee Arthroplasty.

Extensor mechanism complications involving the patella frequently result in total knee arthroplasty (TKA) failure. Postoperative patellar dislocation may be caused by soft tissue imbalance, improper sizing, and position of the prosthesis. This report describes a case of revision TKA requiring several rounds of soft tissue releases in an effort to treat the patient's chronic patellar dislocation. In the process, a novel surgical approach was developed for the surgical management of refractory chronic patellar dislocations. Postoperative follow-up at 1, 2, 6, 12, and 24 months indicated no compl

Editorial Commentary: Proximal Stabilization for Recurrent Patellar Dislocation--Respect the Anatomy and Achieve Excellent Outcomes Regardless of Specific Technique.

In a Level IV evidence systematic review of 13 studies comparing medial patellofemoral ligament reconstruction and medial soft-tissue realignment for recurrent lateral patellar instability without osseous pathology, both techniques showed excellent outcomes with low recurrent dislocation rates (<10% for both) without significant clinical, radiographic, or instability differences between the two groups at minimum 2-year follow-up. As in all well-designed and -conducted systematic reviews, the authors delicately balance study homogeneity (similar patients and surgical procedures-excluding distal
