[Perineal tears and episiotomy: Surgical procedure - CNGOF perineal prevention and protection in obstetrics guidelines].

OBJECTIVES: To recommend the episiotomy procedure, repair of perineal or vaginal tears and episiotomy. METHODS: Published Literature was retrieved using PubMed and Cochrane Library computer databases up to May 2018 and recommendations issued from international societies. RESULTS: A midline episiotomy increases the risk of OASIS compared with a mediolateral procedure (LE2). OASIS rates are similar for mediolateral and lateral episiotomies (LE1). A scar angle of at least 45° (measured in relation to the midline after suturing) is associated with a lower risk of OASIS (LE3). To obtain this fina

Return to high-level throwing after combination infraspinatus repair, SLAP repair, and release of glenohumeral internal rotation deficit.

BACKGROUND: The overhead-throwing athlete is a unique patient, requiring an elite, precise functional ability. Superior labral tears are quite common, and the percentage of athletes who return to play after superior labrum anterior-posterior (SLAP) repair has been variable. A tear of the infraspinatus caused by either internal impingement or tension overload may compromise this return. HYPOTHESIS: The rate of return to a level of play similar to or greater than the preinjury level after repair of combined SLAP and infraspinatus injuries will be lower than in previous reports of SLAP repair al

Arthroscopic Repair of a 360° Labrum and Full-Thickness Rotator Cuff Tear After Shoulder Dislocation.

BACKGROUND: Rotator cuff tears after anterior shoulder instability are more common in patients >40 years of age and rare in younger patients where shoulder instability is most common. Although infrequent, little data exist on the evaluation of combined labral and rotator cuff tears in athletes. INDICATIONS: Combined rotator cuff and labral tears in the young patient population have a high risk of recurrent instability and require unique intraoperative and postoperative considerations. This patient is a Division 1 collegiate wrestler who sustained a 360° labral and full-thickness rotator cuff

Biodegradable stents for caustic esophageal strictures: a new therapeutic approach.

The treatment of caustic esophageal strictures is a challenging topic. Although traditional therapies have limited efficacy, most of these patients eventually require surgery. Biodegradable (BD) stents are newly designed stents for benign conditions. This is a retrospective case series of seven patients with caustic esophageal stricture. BD esophageal stents were inserted for palliation of dysphagia. The position of the stent was checked at 1, 4, 8, 12 16, 20, and 24 weeks and at the end of follow-up period. The follow-up period was 60 ± 23 (36-102) weeks. Complete dissolution of the stent occ

Urethral-fixation technique improves early urinary continence recovery in patients who undergo retropubic radical prostatectomy.

OBJECTIVES: To describe step-by-step an original urethrovesical anastomosis technique (urethral fixation) in patients undergoing retropubic radical prostatectomy (RRP), to compare their early urinary continence recovery with those in a control group receiving a standard anastomosis technique and to identify the predictors of early urinary continence recovery. PATIENTS AND METHODS: We compared 70 patients who underwent RRP with the urethral-fixation technique with a contemporary control group of 51 patients who received RRP with a standard urethrovesical anastomosis. In the urethral-fixation g

Arthroscopically assisted repair of foveal triangular fibrocartilage complex tear using modified "double loop suture" - a retrospective cohort study.

BACKGROUND: The triangular fibrocartilage complex (TFCC) plays an important role in distal radioulnar joint (DRUJ) stabilization, and is frequently torn. In particular, when conservative treatment fails, surgical treatment is needed. METHODS: In this retrospective study, fourteen individuals with TFCC foveal tears and DRUJ instability were admitted to the department and treated with arthroscopic-assisted modified "double loop suture" transosseous repair between January 2021 and 2023. During surgery, an osseous tunnel was established, and two nickel-based alloy loops and a 2-0 polydioxanone II

Posterior Cruciate Ligament Reconstruction With Quadriceps Tendon Autograft and Concomitant Meniscal Ramp Repair.

BACKGROUND: Quadriceps autograft, though well established for anterior cruciate ligament reconstruction, is underutilized in posterior cruciate ligament (PCL) reconstruction largely due to slow adoption. All-inside meniscal ramp repair and quadriceps tendon autograft PCL reconstruction have been described in isolation, but not concomitantly in a video journal. INDICATIONS: PCL reconstruction is indicated in grade 3 isolated tears with instability that have not improved with nonoperative management and in instances with associated injuries such as meniscal ramp tears, as observed in this 18-ye

The Use of Endoclips in Thoracoscopic Correction of Esophageal Atresia: Advantages or Complications?

Background: Thoracoscopic correction of esophageal atresia (EA) with tracheoesophageal fistula (TEF) has been increasingly widespread, but is still one of the most advanced pediatric surgical skills. This procedure has a challenging learning curve, and usually initially requires a longer operative time than the open approach; furthermore to perform this intervention, the surgeon must be very experienced in endocorporeal knotting. In our opinion, standardization of the technique and the application of "tricks" (including the use of titanium endoclips for TEF closure) to make surgical steps easi

Treatment of fractures of the condylar head with resorbable pins or titanium screws: an experimental study.

We aimed to compare in vivo the stability of fixation of condylar fractures in sheep using sonic bone welding and standard titanium screws. We assessed stability of the osteosynthesis and maintenance of the height of the mandibular ramus. Height decreased slightly in both groups compared with the opposite side. The volume of the condyle increased considerably in both groups mainly because callus had formed. The results showed no significant disadvantages for pin fixation compared with osteosynthesis using titanium screws.

Continuous Mass Versus Layered Fascial Closure in Contaminated Emergency Midline Laparotomies: A Prospective Observational Cohort Study.

BACKGROUND: Emergency midline laparotomy in a contaminated field carries a high risk of acute fascial dehiscence (burst abdomen) and of late incisional hernia. Recommendations for abdominal wall closure derive largely from elective surgery, and the optimal anatomical closure technique in the emergency contaminated setting remains undefined. This study compared continuous mass closure with continuous layered closure in this population. MATERIALS AND METHODS: We conducted a hospital-based prospective observational cohort study of 340 consecutive adults undergoing emergency midline laparotomy fo

Genital Sparing Robot-Assisted Radical Cystectomy with Intracorporeal Neobladder & Paravaginal Repair.

OBJECTIVE: Vaginal prolapse is a known complication after radical cystectomy, requiring additional procedures in 10% of the patients.1 This results from loss of level I and II vaginal support due to the removal of pelvic structures. In addition, a neobladder urinary diversion, with Valsalva voiding, predisposes to vaginal prolapse. A genital-sparing approach with paravaginal repair can help prevent such complications. METHODS: The genital sparing technique preserves the uterus, fallopian tubes, ovaries, and vagina, while paravaginal repair involves suturing of the lateral vaginal wall to the
