Setting up a decision support system with decision tables.

The aim of our study was to develop a decision support system using a conventional method which can be used as a shell for different applications. So it was necessary to find a method which allows separation of decision principles and decision algorithms. In addition, documentation of the patient records should be simplified. This could be attained by using the decision table technique and the programming language MUMPS. The general system developed was applied to the therapy decision for patients with liver metastases. The application system was clinically evaluated in a randomized group of p

Use of narrative-based language intervention with children who have specific language impairment.

Ten 7-8-year-old children with specific language impairment participated in a 6-week program of narrative-based language intervention (NBLI) in an effort to evaluate NBLI's feasibility. Each intervention session targeted story content as well as story and sentence form using story retell and generation tasks. Eight children achieved the clinically significant improvement criterion from pre- and posttest comparisons of at least 1.45 points on a narrative quality (NQ) rating (p<.013). Throughout the NBLI program, the children were informally observed to show increased self-confidence in their na

Beyond Keywords: A Qualitative Study of Context-Dependent Stigmatizing Language in Clinical Documentation from a Large, Multi-Payer Home Healthcare Agency.

BACKGROUND: Stigmatizing language in clinical documentation is increasingly recognized as a contributor to healthcare disparities, with evidence that such language disproportionately affects marginalized populations. OBJECTIVE: This study aimed to explore home healthcare (HHC) clinicians' perceptions of stigmatizing language in documentation and to identify strategies for reducing bias. METHODS: We conducted semi-structured interviews with seven nurses between may and October 2024 from a large nonprofit HHC agency in New York City, which serves patients across diverse payer types, such as Me

Language lateralization in temporal lobe epilepsy: a comparison between fMRI and the Wada Test.

PURPOSE: Recent studies have claimed that language functional magnetic resonance imaging (fMRI) can identify language lateralization in patients with temporal lobe epilepsy (TLE) and that fMRI-based findings are highly concordant with the conventional assessment procedure of speech dominance, the intracarotid amobarbital test (IAT). METHODS: To establish the power of language fMRI to detect language lateralization during presurgical assessment, we compared the findings of a semantic decision paradigm with the results of a standard IAT in 68 patients with chronic intractable right and left tem

What (If Anything) is Wrong with High-Frequency Trading?

Hearing is crucial for the development of language skills. Many infants and young children with undiagnosed hearing impairments, particularly those with mild to moderate severity and no apparent risk factors, miss out on much of the critical early phase of language and speech development. This cross-sectional, hospital-based observational study was conducted at GSVM Medical College and LLRH Hospital on children aged 6 months to 5 years over a 12-month period. The study aimed to correlate the severity of hearing impairment with speech-language delay and to identify risk factors linked to sensor

Determinants influencing variability in outcomes of prelingual cochlear implantation.

OBJECTIVE: To identify factors affecting cochlear implantation (CI) outcomes in pre-lingual deaf children, focusing on language development. METHOD: A cross-sectional study of 106 pre-lingual deaf children who received unilateral CI was conducted to analyse factors influencing their language and speech outcomes. RESULTS: The average age of participating children was 83 ± 31.6 months; 53.8% were male, and 36.8% attended regular school programs. Most parents had higher education (64.2% of fathers, 55.7% of mothers), and 58.5% belonged to a low socioeconomic level. Hearing loss diagnosis was de

Greater Pattern Similarity between Mother Tongue and Second Language in the Right ATL Facilitates Understanding of Written Language.

Previous research has mapped out the brain regions that respond to semantic stimuli presented visually and auditorily, but there is debate about whether semantic representation is modality-specific (only written or only spoken) or modality-invariant (both written and spoken). The mechanism of semantic representation underlying native (L1) and second language (L2) comprehension in different modalities as well as how this mechanism is influenced by L2 proficiency, remains unclear. We used functional magnetic resonance imaging (fMRI) data from the OpenNEURO database to calculate neural pattern si

Dual neural foundations of coding cognition: Language comprehension scaffolds initial learning before mathematical systems engage.

To resolve the contested cognitive foundations of programming, we used functional near-infrared spectroscopy (fNIRS) to compare brain activity in 29 coding-naïve adults while they performed three carefully contrasted tasks, each selected to probe a distinct cognitive function: graphical coding (ScratchJr) engaged linguistic parsing and logical-spatial operations; language comprehension targeted semantic and syntactic processing; and mathematical tasks (mental rotation and arithmetic) tapped visuospatial reasoning and numerical cognition. Univariate analyses revealed that coding robustly engage

Genome sequencing for rightward hemispheric language dominance.

Most people have left-hemisphere dominance for various aspects of language processing, but only roughly 1% of the adult population has atypically reversed, rightward hemispheric language dominance (RHLD). The genetic-developmental program that underlies leftward language laterality is unknown, as are the causes of atypical variation. We performed an exploratory whole-genome-sequencing study, with the hypothesis that strongly penetrant, rare genetic mutations might sometimes be involved in RHLD. This was by analogy with situs inversus of the visceral organs (left-right mirror reversal of the he

Impact of Inpatient Patient-Provider Language Concordance on Exclusive Breastfeeding Rates Postpartum.

OBJECTIVE: Studies outside of obstetrics suggest that patient-provider language concordance may impact the efficacy of educational interventions and overall patient satisfaction. Many pregnant patients who present to the hospital for delivery with initial plans to exclusively breastfeed ultimately leave the hospital supplementing with formula. We aim to examine the impact of language concordance between patients and their primary bedside nurse during the delivery hospitalization period on the relationship between intended and actual feeding practices for term newborns of primiparous patients a

Auditory sequence learning with degraded input: children with cochlear implants ('nature effect') compared to children from low and high socio-economic backgrounds ('nurture effect').

Implicit sequence learning (SL) is crucial for language acquisition and has been studied in children with organic language deficits (e.g., specific language impairment). However, language delays are also seen in children with non-organic deficits, such as those with hearing loss or from low socioeconomic status (SES). While some children with cochlear implants (CI) develop strong language skills, variability in performance suggests that degraded auditory input (nature) may affect SL. Low SES children typically experience language delays due to environmental deprivation (nurture). The purpose o

Impedance screening. An integral part of hearing screening.

Rationale is presented for including tympanometry as an integral part of all hearing screening programs. Impedance screening will yield more accurate screening results in the identification of middle ear disease than traditional hearing testing or otoscopic examination. Medical complications often result from unidentified otitis media which also contributes to speech, language, and educational delays in children. Although additional research information is needed on the significance of early identification of children with otitis media, it is argued that tympanometry will foster such research

[Rehabilitation of deaf children with a cochlear implant].

Before the development of cochlear implants, the acquisition of oral and written language and consequently good school levels were barely expected from profoundly deaf children. Since then it has been largely demonstrated that with cochlear implants these objectives are realistic provided these children are implanted early, before the age of 4 years old and receive appropriated speech therapy. We present here the rehabilitation program proposed at our center to the very young users of cochlear implants. This program results from analysis and observations developed during 12 years of speech the

Aphasia treatment delivered by telephone.

Telephone delivery of speech and language treatment programs may be an effective supplement to the more traditional face-to-face delivery. This report describes the response of a nonfluent aphasic patient who, at 11 years postonset, was treated over the telephone using a hierarchically structured syntax treatment program. Examination of pre- and posttreatment performance on the Northwestern Syntax Screening Test and the Boston Diagnostic Aphasia Examination Cookie Theft Description showed improvement on both dependent variables. The results support the use of telephone therapy with patients wh

Melodic Intonation Therapy: present controversies and future opportunities.

This article describes the state of the art of Melodic Intonation Therapy (MIT), a structured aphasia therapy program using the melodic aspects of language (intonation, rhythm, and stress) to improve language production. MIT was developed in the 1970s and is still used worldwide. Nevertheless, we argue that many questions crucial for the clinical application of MIT are still unanswered. First, a review of MIT effect studies is presented showing that evidence from well-designed group studies is still lacking. It is also unclear which aspects of MIT contribute most to its therapeutic effect and
