Thalamic metabolic alterations with cognitive dysfunction in idiopathic trigeminal neuralgia: a multivoxel spectroscopy study.

INTRODUCTION: Although abnormalities in metabolite compositions in the thalamus are well described in patients with idiopathic trigeminal neuralgia (ITN), differences in distinct thalamic subregions have not been measured with proton magnetic resonance spectroscopy ((1)H-MRS), and whether there are correlations between thalamic metabolites and cognitive function still remain unknown. METHODS: Multivoxel MRS was recorded to investigate the metabolic alterations in the thalamic subregions of patients with ITN. The regions of interest were localized in the anterior thalamus (A-Th), intralaminar

Thalamic astrocytomas: surgical anatomy and results of a pilot series using maximum microsurgical removal.

Deep-seated astrocytomas within the basal ganglia and the thalamus are considered unfavourable for microsurgical removal since the circumferential neighbourhood of critical structures limits radical resection. On closer assessment, the thalamus has a unique configuration within the basal ganglia. Its tetrahedric shape has 3 free surfaces and only the ventrolateral border is in contact with vital and critical functional structures, e.g. the subthalamic nuclei and the internal capsule. The purpose of the present study was to investigate the feasibility of maximum microsurgical removal in a serie

Subcortical mechanisms in language: lexical-semantic mechanisms and the thalamus.

Four previously published cases of dominant thalamic lesion in which the author has participated are reviewed to gain a better understanding of thalamic participation in lexical-semantic functions. Naming deficits in two cases support Nadeau and Crosson's (1997) hypothesis of a selective engagement mechanism involving the frontal lobes, inferior thalamic peduncle, nucleus reticularis, and other thalamic nuclei, possibly the centromedian nucleus. This mechanism selectively engages those cortical areas required to perform a cognitive task, while maintaining other areas in a state of relative dis

Structural MRI alterations in children with form-deprivation amblyopia: a propensity score-matched case-control study.

AIMS: Form-deprivation amblyopia (FDAM) is a severe neurodevelopmental disorder from early visual deprivation. This study uses T1-weighted MRI to explore its neuroanatomical correlates. METHODS: 18 Chinese children with bilateral FDAM and 36 propensity score-matched healthy Chinese controls (1-to-2 matched for age, sex and mean cortical thickness) underwent 3.0T T1-weighted MRI (1×1×1 mm³ resolution). Cortical and subcortical structures were parcellated using FreeSurfer V.7.3.2 with multiatlas (Desikan-Killiany-Tourville, Destrieux, subset Brodmann and Aseg), alongside specialised modules for

Thalamic connectivity mirrors spatial maps of network dysfunction in nonlesional focal epilepsy.

OBJECTIVE: Focal epilepsy is increasingly conceptualized as a network disorder, yet the extent to which network dysfunction reflects a shared phenotype remains unknown. Spatially conserved patterns of network dysfunction may implicate a centralized mechanism underlying widespread impairment. Here, we investigate whether network connectivity disruptions are spatially similar across temporal lobe and extra-temporal lobe epilepsy cohorts and whether shared dysfunction aligns with thalamic connectivity profiles. METHODS: We retrospectively analyzed resting-state magnetoencephalographic imaging fr

Neuroimaging signatures of mesial temporal lobe epilepsy: A coordinate-based meta-analysis of structural and resting-state functional imaging literature.

Mesial temporal lobe epilepsy (MTLE) seizures are known to alter neural architecture, yet imaging studies report conflicting findings of their effects on the brain. This study aimed to identify consistent regions exhibiting structural or functional changes in MTLE and compare the regional distributions of pathology detected by different neuroimaging modalities. To that end, thirty-six coordinate-based meta-analyses were performed by applying Alteration Likelihood Estimation to voxel-based morphometry (VBM) and voxel-based physiology (VBP) studies. The meta-analyses revealed convergent MTLE pat

The Impact of Childhood Maltreatment on Central Pain Processing in Individuals With Psychosis.

INTRODUCTION: Exposure to childhood maltreatment can contribute to multiple behavioral and clinical manifestations, including the development of psychotic illnesses and pain-related abnormalities. Aberrant pain perception in individuals with psychosis may be associated with the worsening psychiatric symptoms, including an increase in mood episodes and a higher risk for suicidality. Despite the multiple connections between psychosis, pain, and childhood maltreatment, the combined investigation of these three domains remains limited. METHODS: In this study, patients with schizophrenia (SZ, n =

Normative Modeling of Thalamic Nuclear Volumes and Characterization of Lateralized Volume Alterations in Alzheimer's Disease Versus Schizophrenia.

BACKGROUND: Thalamic nuclei facilitate a wide range of complex behaviors, emotions, and cognition and have been implicated in neuropsychiatric disorders including Alzheimer's disease (AD) and schizophrenia (SCZ). The aim of this work was to establish novel normative models of thalamic nuclear volumes and their laterality indices and investigate their changes in SCZ and AD. METHODS: Volumes of bilateral whole thalami and 10 thalamic nuclei were generated from T1 magnetic resonance imaging data using a state-of-the-art novel segmentation method in healthy control participants (n = 2374) and par

Differential Patterns of Visual Sensory Alteration Underlying Face Emotion Recognition Impairment and Motion Perception Deficits in Schizophrenia and Autism Spectrum Disorder.

BACKGROUND: Impaired face emotion recognition (FER) and abnormal motion processing are core features in schizophrenia (SZ) and autism spectrum disorder (ASD) that have been linked to atypical activity within the visual cortex. Despite overlaps, only a few studies have directly explored convergent versus divergent neural mechanisms of altered visual processing in ASD and SZ. We employed a multimodal imaging approach to evaluate FER and motion perception in relation to functioning of subcortical and cortical visual regions. METHODS: Subjects were 20 high-functioning adults with ASD, 19 patients

A Novel Missense GLA Mutation (p.G35V) Detected in Hemodialysis Screening Leads to Severe Systemic Manifestations of Fabry Disease in Men and Women.

BACKGROUND/AIMS: Fabry disease (FD), an X-linked lysosomal storage disorder, leads to accumulation of globotriaosylceramide. Screening in dialysis patients may identify genetic variants of unknown clinical significance. We aimed to characterize the pathogenicity of a novel GLA gene mutation identified during hemodialysis screening and the histologic findings of early Fabry nephropathy. METHODS: One out of 108 male hemodialysis patients screened for FD presented low α-galactosidase A activity. A novel missense mutation (p.G35V) in the GLA gene was detected. Family screening identified 11 addit

Disentangling disorders of consciousness: Insights from diffusion tensor imaging and machine learning.

Previous studies have suggested that disorders of consciousness (DOC) after severe brain injury may result from disconnections of the thalamo-cortical system. However, thalamo-cortical connectivity differences between vegetative state (VS), minimally conscious state minus (MCS-, i.e., low-level behavior such as visual pursuit), and minimally conscious state plus (MCS+, i.e., high-level behavior such as language processing) remain unclear. Probabilistic tractography in a sample of 25 DOC patients was employed to assess whether structural connectivity in various thalamo-cortical circuits could d

Prevalence of Fabry disease in young patients with cryptogenic ischemic stroke.

BACKGROUND: A German study diagnosed 4% of young cryptogenic ischemic stroke patients with Fabry disease, an X-linked lysosomal storage disease caused by mutations in the alpha-galactosidase A (α-GAL-A) gene resulting in an accumulation of glycosphingolipids. A lower prevalence was found in other geographic regions. AIM: To determine the prevalence of Fabry disease in a Canadian population of young cryptogenic ischemic stroke patients. MATERIALS AND METHODS: Patients with cryptogenic ischemic stroke at age 16-55 were retrospectively identified in our institutional stroke database and underwe

Diagnostic utility of different MRI and MR angiography measures in Fabry disease.

BACKGROUND: Neurologic hallmarks of Fabry disease (FD) include small fiber neuropathy as well as cerebral micro- and macroangiopathy with premature stroke. Cranial MRI shows progressive white matter lesions (WML) at an early age, increased signal intensity in the pulvinar, and tortuosity and dilatation of the larger vessels. To unravel the most promising imaging tool for the detection of CNS involvement in FD we compared the diagnostic utility of the different MR imaging findings. METHODS: Twenty-five clinically affected patients with FD (age 36.5 +/- 11.0) and 20 age-matched controls were in

Functional MRI of verbal self-monitoring in schizophrenia: performance and illness-specific effects.

Previous small-sample studies have shown altered frontotemporal activity in schizophrenia patients with auditory hallucinations and impaired monitoring of self-generated speech. We examined a large cohort of patients with schizophrenia (n = 63) and a representative group of healthy controls (n = 20) to disentangle performance, illness, and symptom-related effects in functional magnetic resonance imaging-detected brain abnormalities during monitoring of self- and externally generated speech in schizophrenia. Our results revealed activation of the thalamus (medial geniculate nucleus, MGN) and fr

[Lateral thalamic infarction. 22 cases].

Isolated lateral thalamic infarcts (LThl) are rare. They often produce lacunar syndromes, and their main cause is thought to be an hypertensive arteriolopathy. To verify these data, we reviewed 639 cerebral infarcts demonstrated by CT and/or MRI and included in a hospital stroke registry over a 4-year period. We identified 22 cases (3.5 p. 100) of isolated LThl (right LThl: 15; left LThl: 7). Nineteen had MRI study. There were 13 men and 9 women of mean age 65 years. None had the complete Dejerine-Roussy syndrome: sensory disturbances 21 cases, hemiparesis 7, hemiataxia 5, involuntary movement

Dissociable neural networks for processing fearful bodily expressions at different spatial frequencies.

The human brain processes visual input across various spatial frequency (SF) ranges to extract emotional cues. Prior studies have extensively explored SF processing in facial expressions, yielding partly conflicting results. However, bodily expressions, which provide complementary emotional and survival-relevant cues, remain unexplored. We investigated the neural mechanisms underlying the processing of low (LSF), high (HSF), and broad spatial frequency (BSF) components in fearful versus neutral bodily postures. Using functional Magnetic Resonance Imaging, we examined brain activity in 20 parti

Multimodal Brain Monitoring-Guided Anesthesia Management Improves Functional Connectivity, Enhances Recovery and Attenuates Postoperative Pain in Elderly Surgical Patients.

PURPOSE: Perioperative neurocognitive disorder (PND) is common in elderly surgical patients and severely affects postoperative recovery. However, effective prevention is still lacking. Potential perioperative cerebral stressors (including inappropriate sedative/analgesic depth and imbalanced cerebral oxygen supply/demand) may be important contributing factors. We developed an anesthesia management protocol based on multimodal brain monitoring to achieve standardized, individualized, and real-time regulation of sedative/analgesic depth and cerebral oxygen saturation and investigated whether it

Periaqueductal gray connectivity in spinal cord injury-induced neuropathic pain.

BACKGROUND AND PURPOSE: Neuropathic pain (NP) is a debilitating condition following spinal cord injury (SCI). The role of periaqueductal gray (PAG) in NP development following SCI remains underexplored. Using resting-state functional MRI (rsfMRI), our study aimed to demonstrate the alterations in functional connectivity (FC) of PAG in NP following SCI. METHODS: Ten SCI patients (SCI + NP, n = 7, and SCI - NP, n = 3), alongside 10 healthy controls (HCs), were enrolled. rsfMRI was conducted followed by seed-to-voxel analysis using PAG as the seed region and then group-based analysis comprising
