A lateral pancreaticojejunostomy (LPJ), also known as the Puestow procedure, is a complex procedure performed for chronic pancreatitis when the pancreatic duct is dilated and unable to drain properly. Traditionally, these procedures are performed with open surgery. A minimally invasive approach to the LPJ using rigid handheld nonarticulating instruments is tedious and rarely performed. PURPOSE OF REVIEW: The use of robotic surgery in reconstructive urology has steadily increased since the year 2000. Articles in the MEDLINE database (from January 2000 to May 2011) identified using the Boolean search: robotic or robot-assisted and reconstructive surgery and urology. In addition, the reference lists of previously published articles on robotic-assisted reconstructive surgery were checked for additional studies. This review will summarize the current advances in robotic-assisted reconstructive urology.

Until now 40 robotic splenectomies were performed in our department, the first case being done on February 25, 2008. Our data show that robotic splenectomy with the DaVinci surgical system is technically feasible and safe, with good results and without complications. The main advantages are a better tridimensional view and an increased versatility of the surgical instruments. The DaVinci system allows an accurate dissection around the splenic hilum and preservation of the splenic remnant vessels in partial splenectomy. PURPOSE: Ureterocalicostomy is a potential option in patients with ureteropelvic junction obstruction and significant lower pole calicectasis. It is often reserved for patients with a failed pyeloplasty and a minimal pelvis, or patients with an exaggerated intrarenal pelvis. Dismembered pyeloplasty is the gold standard treatment for adult ureteropelvic junction obstruction with published success rates consistently over 90%. We report the first case of robot-assisted thoracoscopic resection of a mediastinal parathyroid adenoma in the aorto-pulmonary window. Intervention planning was based on preoperative CT-MIBI image fusion, a new imaging modality that enabled reliable and precise localization of the parathyroid.
