Human middle-ear muscles rarely contract in anticipation of acoustic impulses: Implications for hearing risk assessments.

The current study addressed the existence of an anticipatory middle-ear muscle contraction (MEMC) as a protective mechanism found in recent damage-risk criteria for impulse noise exposure. Specifically, the experiments reported here tested instances when an exposed individual was aware of and could anticipate the arrival of an acoustic impulse. In order to detect MEMCs in human subjects, a laser-Doppler vibrometer (LDV) was used to measure tympanic membrane (TM) motion in response to a probe tone. Here we directly measured the time course and relative magnitude changes of TM velocity in respon

Clinical study of a digital vs an analogue hearing aid.

Digital signal processing in hearing instruments has brought new perspectives to the compensation of hearing impairment and may result in alleviation of the adverse effects of hearing problems. This study compares a commercially available digital signal processing hearing aid (HA) (Senso) with a modern analogue HA with programmable fitting (Logo). The HAs tested are identical in appearance and, in spite of a different mode of operation, the study design ensured blinding of the test subjects. Outcome parameters were: improvements in speech recognition score in noise (deltaSRSN) with the HAs; ov

Characterization of middle ear ossicular ligaments using photon counting detector CT.

BACKGROUND AND PURPOSE: The middle ear suspensory ossicular ligaments have been previously described on traditional energy integrating detector CT scanners. With recent developments in photon counting detector CT, these ligaments can be better visualized due to greater spatial and contrast resolution. MATERIALS AND METHODS: 50 patients (100 temporalbones) without hearing loss or middle ear cavity pathology were retrospectively reviewed to characterize the middle ear suspensory ossicular ligaments including the anterior, superior, lateral mallear, and posterior incudal ligaments, and stapes an

Effects of cochlear synaptopathy on middle-ear muscle reflexes in unanesthetized mice.

Cochlear synaptopathy, i.e. the loss of auditory-nerve connections with cochlear hair cells, is seen in aging, noise damage, and other types of acquired sensorineural hearing loss. Because the subset of auditory-nerve fibers with high thresholds and low spontaneous rates (SRs) is disproportionately affected, audiometric thresholds are relatively insensitive to this primary neural degeneration. Although suprathreshold amplitudes of wave I of the auditory brainstem response (ABR) are attenuated in synaptopathic mice, there is not yet a robust diagnostic in humans. The middle-ear muscle reflex (M

Effect of contralateral acoustic stimulation on active cochlear micromechanical properties in human subjects: dependence on stimulus variables.

1. Outer hair cells (OHCs) have active micromechanical properties that are thought to be the origin of evoked otoacoustic emissions (EOAEs). In the present study, click-evoked otoacoustic emissions were recorded in humans with or without various contralateral acoustic stimulations. A previous study, concentrating on contralateral stimulation with broadband noise, had shown a decrease of the EOAE amplitude in humans. Results support a role for the efferent system in cochlear mechanics; indeed, medial efferent neurons of the olivocochlear bundle terminate on the OHCs. To obtain a better understa

[Effect of noise on changes in the acoustic reflex].

Acoustic, stapedial reflex represents a response of the m. stapedius to a sonic excitation of supra speech intensity. It is the constitutive part of impendancmetric investigations, it is performed on the same apparatus after tympanometry, and it is the inseparable part in representation of impendancmetric findings. Until now, the most frequently monitored parameters of acoustic reflex of clinical importance are: threshold, amplitude, output and input angle of the reflex curve. The aim of this work was to performed detailed analysis of mentioned parameters in workers exposed to extensive action

Automated Adaptive Wideband Acoustic Reflex Threshold Estimation in Normal-hearing Adults.

OBJECTIVES: Acoustic stapedius reflex threshold (ART) tests are included in a standard clinical acoustic immittance test battery as an objective cross-check with behavioral results and to help identify site of lesion. In traditional clinical test batteries, middle-ear admittance of a 226 Hz probe is estimated using ear-canal measurements in the presence of a reflex-activating stimulus. In the wideband (WB) acoustic immittance ART test used in this study, the pure-tone probe is replaced by a WB probe stimulus and changes in absorbed power are estimated using ear-canal measurements in the presen

The evaluation of the sense of hearing in patients with carotid artery stenosis within the extracranial segments.

Stenosis of arteries that supplies blood to the brain is one of the main causes of ischemic stroke which is the third most common cause of deaths in Europe. Atherosclerosis of carotid and vertebral arteries is responsible for 20% of the ischemic stroke cases. Stenosis may be either asymptomatic or manifested with typical neurological symptoms including motor and sensory disturbances as well as disturbances in vision and speech. However, discrete non-specific symptoms of ischemia, including headaches and vertigo, tinnitus and hearing loss, are also quite common. These symptoms may be indicative

Identifying Otosclerosis with Aural Acoustical Tests of Absorbance, Group Delay, Acoustic Reflex Threshold, and Otoacoustic Emissions.

BACKGROUND: Otosclerosis is a progressive middle-ear disease that affects conductive transmission through the middle ear. Ear-canal acoustic tests may be useful in the diagnosis of conductive disorders. This study addressed the degree to which results from a battery of ear-canal tests, which include wideband reflectance, acoustic stapedius muscle reflex threshold (ASRT), and transient evoked otoacoustic emissions (TEOAEs), were effective in quantifying a risk of otosclerosis and in evaluating middle-ear function in ears after surgical intervention for otosclerosis. PURPOSE: To evaluate the ab
