Microvascular changes in the white mater in dementia.

Our studies of the brain microvascular system have focused on some aspects not commonly studied by other research groups because we use some techniques not often used by others. Our observations tend to add new details to the pathological picture rather than contradict the mainstream findings. We use large, thick celloidin sections which provide a three dimensional view of vascular networks, and alkaline phosphatase (AP) staining which allows one to differentiate between afferent and efferent vessels. We found millions of lipid microemboli in the brains of patients after cardiac surgery, and c

Hemodynamic disturbances in vertebral artery dominance: a potential pathogenic mechanism in acute isolated pontine infarction.

BACKGROUND: Isolated pontine infarction (IPI) is closely linked to the hemodynamic properties of the vertebral artery. However, the underlying pathogenic mechanisms remain insufficiently defined. This study was designed to explore the contribution of vertebral artery hemodynamic disturbances to the development of IPI and identify the optimal hemodynamic cut-off values for predicting IPI risk in patients with vertebral artery dominance (VAD). MATERIALS AND METHODS: A total of 381 individuals were enrolled. These included 205 patients with acute IPI and 176 control individuals without cerebrovas

Prediction of adverse pregnancy outcome in monochorionic diamniotic twin pregnancy complicated by selective fetal growth restriction.

OBJECTIVE: To identify key factors associated with adverse perinatal outcome in monochorionic diamniotic twin pregnancy complicated by selective fetal growth restriction (sFGR). METHODS: This was a retrospective cohort study of all monochorionic diamniotic twin pregnancies diagnosed with sFGR at ≥ 16 weeks' gestation, in a single tertiary referral center between March 2000 and May 2015. The presence of coexisting twin-twin transfusion syndrome (TTTS) was noted. Fetal biometry and Doppler indices, including those of the umbilical artery (UA) and ductus venosus (DV), were recorded at the time o

Mid-pregnancy uterine artery Dopplers and child neurodevelopment.

OBJECTIVE: Resistance to blood flow in the maternal uterine artery (UtA) has been associated with both preeclampsia (PE) and small-for-gestational age (SGA). Our objective was to assess the association of increased maternal UtA resistance with child neurodevelopmental and behavioral outcomes in low-risk pregnancies. STUDY DESIGN: Serial ultrasound exams were prospectively performed in 2334 racial and ethnically diverse, healthy patients with low-risk singleton pregnancies (July 2009 to Jan 2013) as part of the NICHD-Fetal Growth Studies. Maternal UtA Dopplers were performed between 18w0d and

Perinatal risk assessment in pregnancies complicated by early-onset fetal growth restriction: development and internal validation of a prediction model for composite adverse perinatal outcome.

OBJECTIVE: Early-onset fetal growth restriction (FGR) frequently requires iatrogenic preterm delivery to prevent stillbirth or the sequelae of hypoxia. A prediction model for adverse perinatal outcome could aid clinical decision-making and parental counseling. However, an adequate externally validated model, including predictors that are readily available for this purpose, is currently lacking. The aim of the present study was to develop a prediction model for composite adverse perinatal outcome (CAPO) to be used at the time of hospital admission for fetal monitoring in early-onset FGR. [New examination methods for macular disorders--application of diagnosis and treatment].

To establish a diagnosis or evaluate the efficacy of treatment for macular disorders, we need methods to evaluate the anatomical and functional changes of these disorders. In this article, we describe several studies that we have conducted for 2 years. In section 1, we report our new methods for making a diagnosis and evaluating visual function in macular disorders. In section 2, we describe our trials of these examination methods in treatment. Here is the summary of our results. In section 1, to examine the structures of the macular area, we used a retinal thickness analyzer (RTA), a confocal

Associations between arterial stiffening and brain structure, perfusion, and cognition in the Whitehall II Imaging Sub-study: A retrospective cohort study.

BACKGROUND: Aortic stiffness is closely linked with cardiovascular diseases (CVDs), but recent studies suggest that it is also a risk factor for cognitive decline and dementia. However, the brain changes underlying this risk are unclear. We examined whether aortic stiffening during a 4-year follow-up in mid-to-late life was associated with brain structure and cognition in the Whitehall II Imaging Sub-study. METHODS AND FINDINGS: The Whitehall II Imaging cohort is a randomly selected subset of the ongoing Whitehall II Study, for which participants have received clinical follow-ups for 30 years

Placental biomarker and fetoplacental Doppler abnormalities are strongly associated with placental pathology in pregnancies with small-for-gestational-age fetus: prospective study.

OBJECTIVE: Placental dysfunction can result in small-for-gestational age (SGA) or fetal growth restriction (FGR). The aim of this prospective cohort study was to assess the association of the cerebroplacental ratio (CPR) and other more conventional fetoplacental Doppler indices, circulating placental growth factor (PlGF) levels and soluble fms-like tyrosine kinase-1 (sFlt-1)/PlGF ratio, with specific placental abnormalities in a large cohort of pregnancies with an SGA/FGR fetus. METHODS: This was a prospective cohort study of singleton pregnancies with a SGA/FGR fetus conducted at the Centre

Recipient umbilical artery elongation (redundancy) in twin-twin transfusion syndrome.

BACKGROUND: Chronic hypertension in adults causes arterial lengthening in major arteries, but the effects of early fetal hypertension on the twin-twin transfusion syndrome recipient's vascular architecture remains unknown. OBJECTIVE: We hypothesize that arterial cord redundancy is related to recipient hypertension and subsequent heart failure. Our objectives were to: (1) establish a 3-dimensional color Doppler ultrasound method of measuring umbilical arterial length relative to its corresponding venous segment in the umbilical cord using artery vein angle; (2) compare recipient artery vein an

Intraoperative transit time flow measurements during off-pump coronary artery bypass surgery: The impact of coronary stenosis on competitive flow.

BackgroundCombining preoperative angiography findings with intraoperative transit time flow measurements (TTFM) may improve patency of coronary artery bypass grafts. Nevertheless, graft flow might be impaired by native coronary flow based on the severity of stenoses, with inferior long-term outcomes. This study investigates the impact of left anterior descending artery (LAD) stenosis on competitive flow measured in left internal mammary artery (LIMA) grafts during off-pump coronary artery bypass grafting.MethodsFifty patients were included in this prospective single-center cohort study. Current approach to the diagnosis and treatment of femoral-popliteal arterial disease. A systematic review.

Peripheral arterial disease (PAD) is a common manifestation of atherosclerosis affecting 5 million adults in the United States, with an age-adjusted prevalence of 4% to 15% and increasing up to 30% with age and the presence of cardiovascular risk factors. In this article we focus on lower extremity PAD and specifically on the superficial femoral and proximal popliteal artery (SFPA), which are the most common anatomic locations of lower extremity atherosclerosis. We summarize current evidence and perform a systematic review on the diagnostic evaluation as well as the medical, endovascular and s

Discontinuation of intravenous oxytocin in the active phase of induced labour.

BACKGROUND: In most Western countries, obstetricians and midwives induce labour in about 25% of pregnant women. Oxytocin is an effective drug for this purpose, but associated with serious adverse effects of which uterine tachysystole, fetal distress and the need for immediate delivery are the most common. Various administration regimens such as reduced or pulsatile dosing have been suggested to minimise these. Discontinuation in the active phase of labour, i.e. when contractions are well-established and the cervix is dilated at least 5 cm is another method which may reduce adverse effects. OB
