BVES downregulation in non-syndromic tetralogy of fallot is associated with ventricular outflow tract stenosis.

BVES is a transmembrane protein, our previous work demonstrated that single nucleotide mutations of BVES in tetralogy of fallot (TOF) patients cause a downregulation of BVES transcription. However, the relationship between BVES and the pathogenesis of TOF has not been determined. Here we reported our research results about the relationship between BVES and the right ventricular outflow tract (RVOT) stenosis. BVES expression was significantly downregulated in most TOF samples compared with controls. The expression of the second heart field (SHF) regulatory network genes, including NKX2.5, GATA4

Revisitation of Double-Inlet Left Ventricle or Tricuspid Atresia With Transposed Great Arteries.

BACKGROUND: Patients with double-inlet left ventricle or tricuspid atresia with transposition of the great arteries are predisposed to systemic outflow tract obstruction necessitating systemic outflow relief operations (SORO). METHODS: Between January 2000 and February 2018, 30 patients with double-inlet left ventricle (n = 20) or tricuspid atresia (n = 10) with transposition of the great arteries underwent single-ventricle palliation. Arch obstruction was observed in 14 patients. Initial palliative procedures comprised pulmonary artery banding with (n = 4) or without (n = 14) arch repair, bi

Bartonella henselae endocarditis of percutaneously implanted pulmonary valve: a case report.

Percutaneous pulmonary valve implantation (PPVI) has revolutionized the management of right ventricular outflow tract dysfunction after repaired congenital heart disease. The technology is considered to be safe, with a relatively low complication rate. Infection is one of the described complications of PPVI, and to date five cases of culture-positive infective endocarditis of percutaneously implanted pulmonary valve have been reported worldwide. Herein is reported the first ever case of culture-negative endocarditis of a percutaneously implanted pulmonary valve, caused by Bartonella henselae,

Hypertrophic cardiomyopathy in childhood.

Hypertrophic cardiomyopathy has important differences in children compared with adults, particularly with regard to the range of causes and the outcomes in infants. Survival is highly dependent on etiology, particularly in the youngest patients, and pursuit of the specific cause is therefore necessary. The clinical utility of defining the genotype in children with familial hypertrophic cardiomyopathy exceeds that at other ages and has a highly favorable cost/benefit ratio. Although most of the available information concerning treatment and prevention of sudden death is derived in adults, manag

Long-term follow-up study on obstructive hypertrophic cardiomyopathy patients treated with disopyramide: evidences of a notable trend in symptom control within a real-world clinical setting.

BACKGROUND: In obstructive hypertrophic cardiomyopathy (HOCM), disopyramide is used in patients who remain symptomatic despite β-blockers or verapamil. However, effectiveness of disopyramide therapy has not been clearly established due to inconsistent definition of responders and the insufficient length of follow-ups reported in literature. To address these shortcomings, we have conducted a retrospective analysis from detailed databases with long follow-up, from two HCM Referral Centers. METHODS: 62 symptomatic HOCM patients (43% women, age 52 ± 14 years) with left ventricular (LV) outflow tr

Effects of Renin-Angiotensin-Aldosterone System Inhibition on Left Ventricular Hypertrophy, Diastolic Function, and Functional Status in Patients With Hypertrophic Cardiomyopathy: A Systematic Review.

The renin-angiotensin-aldosterone system (RAAS) plays a vital role in cardiovascular homeostasis by regulating blood pressure, salt, and water balance. The kidneys produce renin which converts angiotensinogen to angiotensin-1 (AT-I) and angiotensin-converting enzyme (ACE) to angiotensin-II (AT-II). AT-II binds to receptors in the adrenal cortex to release aldosterone. AT-II and aldosterone promote water and salt retention, vascular tone, and myocardial contractility. These physiological changes raise blood pressure and circulation. Reduced renal perfusion pressure sensed by baroreceptors and t

Timing of invasive septal reduction therapies and outcome of patients with obstructive hypertrophic cardiomyopathy.

BACKGROUND: Whether early vs. delayed referral to septal reduction therapies (SRT, alcohol septal ablation or surgical myectomy) bears prognostic relevance in hypertrophic obstructive cardiomyopathy (HOCM) is unresolved. We analyzed the impact of SRT timing on the outcome of HOCM patients. METHODS: We followed 126 patients for 5 ± 4 years after SRT (mean age 53 ± 15 years; 55 post-ASA and 71 post-SM). Based on time-to-treatment (TTT; from HOCM diagnosis to SRT), patients were divided into three groups: "<3" years, N = 50; "3-5" years, N = 25; ">5" years, N = 51. RESULTS: Patients with TTT >

Long non-coding RNAs as molecular hubs integrating inflammatory and osteogenic pathways in calcific aortic valve disease.

SIGNIFICANCE: Cardiovascular disease remains the leading cause of morbidity and mortality worldwide. Among valvular pathologies, CAVD is the most prevalent and poses a growing burden on the aging population. Once considered a passive degenerative process, aortic stenosis (AS) is now understood to be an actively regulated disease characterized by progressive leaflet fibrosis, calcification, and inflammation, ultimately leading to left ventricular outflow obstruction and heart failure. Current treatment options are limited to surgical or transcatheter valve replacement, as no pharmacological the

Tetralogy of Fallot and abnormal coronary artery: use of a prosthetic conduit is outdated.

OBJECTIVES: Repair of tetralogy of Fallot (ToF) can be challenging in the presence of an abnormal coronary artery (CA) in 5-12% of cases. The aim of this study was to report our experience with ToF repair without the systematic use of a right ventricle-to-pulmonary artery (RV-PA) conduit. METHODS: We conducted a monocentric retrospective study from 2000 to 2016, including 943 patients with ToF who underwent biventricular repair, of whom 8% (n = 76) presented with an abnormal CA. Mean follow-up time was 50 months (1 month-18 years). RESULTS: The most frequent CA anomaly was the left descendin

Evolution of aortic valve replacement in children: a single center experience.

BACKGROUND: The availability of an ideal prosthesis for aortic valve replacement (AVR) in children remains controversial due to an early degeneration of xenografts and the potential risks related to anticoagulation with mechanical prostheses. This has led many surgeons to the Ross procedure. This study outlines the evolution of our 30-year experience with AVR in children. METHODS: One hundred and forty-seven children, aged 10 days to 18 years (mean 11.9+/-5.7 years), underwent AVR between 1974 and June 2005. Preoperative diagnosis included aortic insufficiency (n=39), aortic stenosis (n=14),

Performance of aortic and pulmonary homografts in the right ventricular outflow tract in children.

Cryopreserved homografts have been used at the German Heart Institute Berlin since October 1986. Until 31st May 1994, cryopreserved aortic (AA) or pulmonary (PA) homografts were implanted in the pulmonary position and followed up in 104 patients. The mean age at operation was 5.5 years. The patients were treated with 3-5 mg/kg aspirin daily after the operation for three months. They all had routine echocardiographic evaluation of their homografts, 47 patients were studied by heart catheterization. Excellent functional results of the homograft valves were found in 92 patients (88.5%). Early deg

Right ventricular outflow tract transannular patch placement without cardiopulmonary bypass.

In children with pulmonary atresia not amenable to initial complete correction, antegrade pulmonary blood flow can be established with surgical right ventricular outflow tract (RVOT) patch enlargement. An 11-year experience with RVOT transannular patch (TAP) augmentation without the use of cardiopulmonary bypass (off-pump) is reported. From March 1993 to October 2004, off-pump surgical RVOT enlargement with a TAP was attempted in all patients in whom a concurrent procedure that required bypass was not required. The procedure was performed with cardiopulmonary bypass standby. Twenty-two consecu

Transseptal Transcatheter Mitral Valve Replacement for Mitral Annular Calcification: ENCIRCLE MAC Registry 1-Year Outcomes.

BACKGROUND: Patients with symptomatic mitral valve dysfunction (MVD) and mitral annular calcification (MAC) represent one of the most challenging populations in structural heart disease. Advanced age, multiple comorbidities, extensive annular calcification, and unfavorable anatomy often preclude surgical mitral valve replacement or transcatheter repair, leaving many patients with limited treatment options. Whether fully percutaneous transseptal transcatheter mitral valve replacement (TMVR) can provide a meaningful treatment pathway for patients with MAC-related MVD remains uncertain. OBJECTIV
