Prevalence of Pre-Existing Hearing Loss Among Patients With Drug-Resistant Tuberculosis in South Africa.

Purpose Hearing loss, resulting from aminoglycoside ototoxicity, is common among patients with drug-resistant tuberculosis (DR-TB). Those with pre-existing hearing loss are at particular risk of clinically important hearing loss with aminoglycoside-containing treatment than those with normal hearing at baseline. This study aimed to identify factors associated with pre-existing hearing loss among patients being treated for DR-TB in South Africa. Method Cross-sectional analysis nested within a cluster-randomized trial data across 10 South African TB hospitals. Patients ≥ 13 years old received cl

Synergistic effects of sulopenem in combination with cefuroxime or durlobactam against Mycobacterium abscessus.

UNLABELLED: Mycobacterium abscessus (Mab) affects patients with immunosuppression or underlying structural lung diseases such as cystic fibrosis (CF). Additionally, Mab poses clinical challenges due to its resistance to multiple antibiotics. Herein, we investigated the synergistic effect of dual β-lactams [sulopenem and cefuroxime (CXM)] or the combination of sulopenem and CXM with β-lactamase inhibitors [BLIs-avibactam (AVI) or durlobactam (DUR)]. The sulopenem-CXM combination yielded low minimum inhibitory concentration (MIC) values for 54 clinical Mab isolates and ATCC19977 (MIC50 and MIC90

Epidemiological trajectories and predictive modeling of latent tuberculosis prevalence in South Asia: an in-depth global burden of disease analysis (1990-2021) with advanced forecasting to 2050.

BACKGROUND: Latent tuberculosis infection (LTBI) remains a critical reservoir for the global tuberculosis (TB) epidemic, particularly in South Asia, where high population density and socioeconomic disparities amplify its burden. However, LTBI epidemiological trajectories and long-term forecasts have received limited attention. This study characterizes LTBI prevalence trends in South Asia from 1990 to 2021 and forecasts prevalence to 2050 to inform targeted interventions. METHODS: We analysed age-standardised LTBI prevalence rates (ASPR per 100 000 population) from the Global Burden of Disease

Current Understanding of Interactions between Nanoparticles and ABC Transporters in Cancer Cells.

BACKGROUND: ATP-binding cassette (ABC) transporters-mediated multidrug resistance (MDR) remains the major obstacle for effective cancer therapy. Nanoparticles (NPs)-based delivery systems are promising to overcome MDR, but only a few of them have been accepted for clinical treatment, which should be due to their insufficient transportation and potential toxicity. In this respect, more and more attentions are being attracted on the interactions between NPs and ABC transporters, which hold a key role in the treatment of MDR cancer and the toxicity of NPs. However, there are no systematic reviews

Latent tuberculosis infection in children: a call for revised treatment guidelines.

BACKGROUND: Guidelines for latent tuberculosis infection do not consider drug-resistance patterns when recommending treatment for immigrant children. OBJECTIVES: The purpose of this research was to decide at what rate of isoniazid resistance a different regimen other than isoniazid for 9 months should be considered. METHODS: We constructed a decision tree by using published data. We studied 3 regimens considered to be effective for susceptible organisms: (1) isoniazid for 9 months, (2) rifampin for 6 months, and (3) isoniazid for 9 months plus rifampin for 6 months. In addition, we evaluated

Fluoroquinolones and isoniazid-resistant tuberculosis: implications for the 2018 WHO guidance.

INTRODUCTION: 2018 World Health Organization (WHO) guidelines for the treatment of isoniazid (H)-resistant (Hr) tuberculosis recommend a four-drug regimen: rifampicin (R), ethambutol (E), pyrazinamide (Z) and levofloxacin (Lfx), with or without H ([H]RZE-Lfx). This is used once Hr is known, such that patients complete 6 months of Lfx (≥6[H]RZE-6Lfx). This cohort study assessed the impact of fluoroquinolones (Fq) on treatment effectiveness, accounting for Hr mutations and degree of phenotypic resistance. METHODS: This was a retrospective cohort study of 626 Hr tuberculosis patients notified in

Cost per patient of treatment for rifampicin-resistant tuberculosis in a community-based programme in Khayelitsha, South Africa.

OBJECTIVES: The high cost of rifampicin-resistant tuberculosis (RR-TB) treatment hinders treatment access. South Africa has a high RR-TB burden, and national policy outlines decentralisation to improve access and reduce costs. We analysed health system costs associated with RR-TB treatment by drug resistance profile and treatment outcome in a decentralised programme. METHODS: Retrospective, routinely collected patient-level data were combined with unit cost data to determine costs for each patient in a cohort treated between January 2009 and December 2011. Drug costs were based on recommended

What is the true tuberculosis mortality burden? Differences in estimates by the World Health Organization and the Global Burden of Disease study.

BACKGROUND: The World Health Organization (WHO) and the Global Burden of Disease (GBD) study at the Institute for Health Metrics and Evaluation (IHME) periodically provide global estimates of tuberculosis (TB) mortality. We compared the 2015 WHO and GBD TB mortality estimates and explored which factors might drive the differences. METHODS: We extracted the number of estimated TB-attributable deaths, disaggregated by age, HIV status, sex and country from publicly available WHO and GBD datasets for the year 2015. We 'standardized' differences between sources by adjusting each country's differen

Biopsychosocial determinants of tuberculosis care and treatment outcomes: a scoping review and bibliometric analysis.

BACKGROUND: Tuberculosis (TB) remains the leading infectious-disease cause of death globally, with an estimated 10.7 million incident cases and 1.23 million deaths in 2024. Its persistence is driven not by microbial factors alone, but by a complex interplay of biological, psychological, and social determinants influencing exposure, vulnerability, and access to care. Despite growing recognition of these biopsychosocial dimensions, the available evidence remains fragmented across disciplines, and no previous review has comprehensively mapped their scope while simultaneously characterizing the st

Towards universal social protection for people affected by tuberculosis in the Western Pacific Region: a social protection baseline assessment and policy entry points.

BACKGROUND: Achieving universal social protection (SP) coverage for people affected by tuberculosis (TB) is increasingly recognised as an essential component of its response, as well as other diseases of poverty. Realising this goal requires to clearly understand the SP needs of people affected by TB and to identify means to maximise their access to existing or new SP benefits in an efficient, effective, and sustainable manner. MAIN BODY: To address these questions, between 2022 and 2023, the WHO Western Pacific Regional office conducted the first SP baseline assessment for people affected by

Stigmatization and discrimination of female tuberculosis patients in Kyrgyzstan - a phenomenological study.

INTRODUCTION: The Republic of Kyrgyzstan is among the 30 countries with the highest burden of multidrug-resistant Tuberculosis worldwide. One of the reasons is widespread stigmatization and discrimination. As previous research has shown, particularly women experience stigma while its impact on their life and (mental) health is even greater than for men. This is the first phenomenological study to explore women's lived experiences of TB-related stigmatization in Kyrgyzstan. This study aims to raise awareness about the gender-specific impact of stigmatization and discrimination. METHODOLOGY: De

Perspectives on pregnant women's educational needs to prevent TB complications during pregnancy and the neonatal period. A qualitative study.

BACKGROUND: Tuberculosis (TB) during pregnancy could confer a high risk for maternal and infant morbidity. Literature indicates that the global burden of active TB disease among pregnant women is not well researched. Statistics for South Africa from WHO give an estimated incidence of 360, 000 cases of TB in 2019; 14,000 people became ill with multidrug-resistant TB in 2019, with a rate of 615 per 100,000 population, implying that the cohorts included pregnant women with and without a diagnosis of TB infection. Therefore, the study aims to increase the understanding of the educational needs req

Treatment optimization in patients co-infected with HIV and Mycobacterium tuberculosis infections: focus on drug-drug interactions with rifamycins.

Tuberculosis (TB) and HIV continue to be two of the major causes of morbidity and mortality in the world, and together are responsible for the death of millions of people every year. There is overwhelming evidence to recommend that patients with TB and HIV co-infection should receive concomitant therapy of both conditions regardless of the CD4 cell count level. The principles for treatment of active TB disease in HIV-infected patients are the same as in HIV-uninfected patients. However, concomitant treatment of both conditions is complex, mainly due to significant drug-drug interactions betwee

Clinical characteristics of tuberculosis in children in the north of Taiwan.

Tuberculosis (TB) in children is an endemic and sometimes life-threatening disease in Taiwan. This study analyzed the clinical characteristics of a total of 112 children with TB managed in a referral children's hospital in the north of Taiwan between 1998 and 2002. The diagnosis of TB was made by either a positive result of culture, acid-fast stain, histopathology, or polymerase chain reaction (PCR). Tuberculin skin test was positive (indurations > or =10 mm) in 73% of 63 patients tested. The male-to-female ratio was 0.93 (54/58), and the mean age was 10.7 years. Thirty one percent of children
