Phenome-wide comorbidity network analysis reveals clinical risk patterns in enthesopathy and enthesitis.

BACKGROUND: Enthesopathy and enthesitis, including rotator cuff disease and other tendon disorders, represent a heterogeneous group of musculoskeletal conditions with complex etiologies. Understanding how systemic health profiles influence their onset remains a critical challenge in musculoskeletal medicine. METHODS: We conducted a large-scale, phenome-wide comorbidity analysis using longitudinal electronic health records (EHR) from 432,757 UK Biobank participants. Incident cases of peripheral enthesopathies were compared to controls across 434 baseline disease phenotypes. A directed ego netw

CCL4L2 participates in tendinopathy progression by promoting macrophage inflammatory responses: a single-cell analysis.

BACKGROUND: Tendinopathy is very common in clinical practice, which is highly prevalent in athletes, sports enthusiasts and other people involved in high-load weight-bearing activities. Common types of tendinopathy include rotator cuff injury, Achilles tendinitis, tennis elbow and so on. Macrophages (Macs) are key immune cells in the pathogenesis of tendinopathy. In this study, CCL4L2+ M1-related signaling pathways were screened by combining single-cell RNA sequencing (scRNA-seq) to explore their significance in tendinopathy treatment. METHODS: Immune cell populations were screened by Uniform

Si-ITGA6-Loaded Liposomes Inhibit Capsule Fibrosis via the FAK/PI3K/Akt Signaling Pathway in Adhesive Capsulitis of Shoulder.

BACKGROUND: Adhesive capsulitis of shoulder (ACS) is a common shoulder disease with pain, joint stiffness, and capsule fibrosis. The pathogenesis of ACS remains unclear, and long-term efficacy of current treatment for ACS is not satisfactory. Given the absence of targeted therapy, proteomic analysis is conducted on the capsules of ACS patients, in an effort to identify the potential target for ACS treatment. The liposome-based siRNA delivery system was utilized for in vivo experiments due to its non-immunogenicity, stability, and low off-target effects compared to other siRNA delivery methods.

Revisiting open capsuloplasty for the treatment of anterior shoulder instability: 35-year follow-up of the Du Toit procedure.

The Du Toit open capsuloplasty for the treatment of anterior shoulder instability is based on the concept of restoring joint stability by recreating the integrity of the anterior glenoid labrum and inferior gleno-humeral ligament using staples. The long-term validity of this procedure for the treatment of anterior shoulder instability was retrospectively assessed by a clinical or telephone interview and radiographic evaluation in 58 patients with an average 35-year follow-up between 1948 and 1974. The range of motion was evaluated by comparing the treated side with the contralateral one; subje

Influenza Vaccination Is Not Associated with Increased Number of Visits for Shoulder Pain.

BACKGROUND: Shoulder injury from vaccination was approved for automatic compensation from the Vaccine Injury Compensation Program (VICP)-a federal government program started in 1988 to shield the manufacturers of childhood vaccines from liability. The approval was made on the basis of case reports rather than experimental evidence. This, combined with the addition of influenza vaccination to the VICP in 2005 (which broadened coverage to include adults) and other social factors, was associated with a rapid rise in the number of claims of shoulder injury from vaccination over the last decade, wh

Multifactorial approach training for anterior shoulder instability after a Latarjet procedure: protocol for a randomised controlled trial.

INTRODUCTION: The Latarjet procedure is the mainstay treatment in high-demand patients with substantial glenoid bone loss or after failed capsulolabral repairs. Patients typically return to sport (RTS) within 6 months postoperatively, requiring intensive rehabilitation. Current rehabilitation protocols focus on mobility, strength and stability. Yet, psychological factors, such as fear of reinjury, are the main reason not to RTS. Therefore, this study aims to determine whether integrating psychological interventions into postoperative rehabilitation improves patient-reported shoulder function c

Inferior subluxation of the humeral head after plate surgery for proximal humerus fracture is not always benign.

BACKGROUND: Inferior subluxation of the humeral head following shoulder trauma, osteosynthesis, or rotator cuff repair has been considered as a benign or temporary phenomenon due to muscle fatigue or capsular injury. However, the clinical impact is still unclear as to their occurrence after plating for proximal humerus fractures. This study aims to investigate their patterns and identify associated risk factors as well as clinical outcomes. METHODS: The research included patients who received locking plate surgery following an acute proximal humerus fracture with a minimum follow-up of 12 mon

Fibro-adhesive Bursitis: A Novel Sonographic Finding in Adhesive Capsulitis Patients and a Proposal of Management.

INTRODUCTION: Adhesive capsulitis, also known as "frozen shoulder," is a debilitating shoulder condition increasingly linked to fibroadhesive bursitis, particularly after COVID-19 and related vaccinations. There is no definitive gold standard for its treatment, the primary therapeutic objectives of which are the reduction of pain and the restoration of shoulder range of motion. The aim of our study was to analyze treatment outcomes based on quantitative measures of shoulder function and symptom relief. METHOD: Conducted between January 2022 and April 2023, the research involved 45 patients in

Adhesive capsulitis after COVID-19 vaccine injection: a peculiar case treated with combined bursa distention and glenohumeral capsular hydrodilatation.

Frozen shoulder is a common and self-limiting condition affecting the soft tissues of the shoulders, characterized by severe pain, impaired range of motion (ROM) and limitation of daily activities. Its prevalence is 5% and it occurs most commonly in the fifth and sixth decades of life; women are more affected [DePalma in Clin Orthop Relat Res 466:552-560, 2008]. It can be idiopathic or associated with other conditions such as metabolic disorders, diabetes, thyroid diseases, prolonged immobilization, trauma [DePalma in Clin Orthop Relat Res 466:552-560, 2008], or complications after vaccine adm

Ultrasound-Guided Injection of a Tendon-Compatible Hyaluronic Acid Preparation for the Management of Partial Thickness Rotator Cuff Tear: A Case Report.

Partial-thickness rotator cuff (RC) tear constitutes the most common cause of shoulder pain and disability. Its management is challenging, and a conservative approach is suggested as a first-line treatment. Nonetheless, minimally invasive approaches have been described in clinical trials, such as ultrasound (US)-guided tendon-compatible hyaluronic acid (HA) injection preparation in the rupture site. HA is believed to fill the intradermal space and thus support the regeneration process by its integration in the damaged extracellular matrix. A reduced healing period required for a tendon tear wh

Comparison of Cellular Responses to TGF-β1 and BMP-2 Between Healthy and Torn Tendons.

BACKGROUND: Tendons heal by fibrotic repair, increasing the likelihood of reinjury. Animal tendon injury and overuse models have identified transforming growth factor beta (TGF-β) and bone morphogenetic proteins (BMPs) as growth factors actively involved in the development of fibrosis, by mediating extracellular matrix synthesis and cell differentiation. PURPOSE: To understand how TGF-β and BMPs contribute to fibrotic processes using tendon-derived cells isolated from healthy and diseased human tendons. STUDY DESIGN: Controlled laboratory study. METHODS: Tendon-derived cells were isolated f
