Scuba diving and otology: a systematic review with recommendations on diagnosis, treatment and post-operative care.

Scuba diving is a popular recreational and professional activity with inherent risks. Complications related to barotrauma and decompression illness can pose significant morbidity to a diver's hearing and balance systems. The majority of dive-related injuries affect the head and neck, particularly the outer, middle and inner ear. Given the high incidence of otologic complications from diving, an evidence-based approach to the diagnosis and treatment of otic pathology is a necessity. We performed a systematic and comprehensive literature review including the pathophysiology, diagnosis, and treat

Cerebral arterial gas embolism in a professional diver with a persistent foramen ovale.

A 33-year-old, male professional scallop diver diving on the Outer Hebrides in Scotland rapidly developed symptoms of cerebral arterial gas embolism following a provocative dive with possibly a fast ascent. During transfer by helicopter to the mainland for treatment, his symptoms improved on surface oxygen. He was recompressed on a Royal Navy Treatment Table 62 (RN TT62) with complete resolution. Just over six weeks later, again diving on the Outer Hebrides and after adopting more conservative diving practices, he developed symptoms and signs of vestibular decompression sickness after a proble

Menstrual history in altitude chamber trainees.

Previous studies have determined a higher rate of altitude-induced decompression sickness (DCS) in women than in men. Women are reportedly at higher risk for developing DCS during menses. A study of menstrual history in women completing altitude chamber training without developing DCS has never been accomplished. The purpose of this study was to collect and analyze menstrual history in these women. Thirteen U.S. Air Force Aerospace Physiology Units participated in a USAF-approved survey for 1 year. After completing altitude chamber flights, data on age, day of menstrual cycle (DMC), birth cont

Entrapment of the Greater Occipital Nerve with Chronic Migraine and Severe Facial Pain: A Case Report.

Migraine is thought to be a primary neurovascular headache due to brain dysfunction and is known to involve peripheral and central sensitization. A female patient with chronic migraine symptoms for 30 years reported severe pain in the deep ear and face. This headache always showed the same pattern and temporal progression. The sudden onset of ache and throbbing pain in the right temporo-occipital area extended to the left temporo-occipital areas. She felt sick as if the head would burst, and nausea and vomiting occurred. During the last 3 years, the patient endured sharp pain in bilateral deep

An echo from the past: open access repository of over 10,000 annotated Doppler audio recordings of venous gas emboli.

INTRODUCTION: Doppler ultrasound measurements have been recorded since the 1970s across the world and provide a valuable data resource for learning, analysis, and potential training of deep learning algorithms to recognise and grade venous gas emboli (VGE) allowing assessment of decompression sickness (DCS) risk. METHODS: We collected a 'big database' of Doppler recordings and associated metadata. Audio tapes with recorded Doppler data were converted to digital files, then cut into individual recordings and matched with their metadata, including subject and pressure profile information. The a

Scuba tank fill survey in Victoria, Australia, 1 July 2024 to 30 June 2025.

INTRODUCTION: This study's aim was to determine the number of scuba tank fills done in Victoria, Australia from 1 July 2024 to 30 June 2025 to provide an estimate of the number of scuba dives conducted during that period and, from that, estimates of the fatality and decompression illness rates. METHODS: Suppliers of compressed gas for scuba diving in Victoria were identified through internet searches, industry liaison and the Australasian Diving Safety Foundation records. Those identified were emailed an invitation to participate in the tank fill survey and provided with dedicated spreadsheet

Adaptations to breath-hold diving: from traditional divers to elite athletes.

Breath-hold diving exposes humans to repeated episodes of profound hypoxia and hypercapnia, eliciting physiological adaptations that enable prolonged underwater performance. This article summarises current knowledge on chronic adaptations in elite breath-hold athletes and traditional diving populations, including the Bajau sea nomads of Southeast Asia and the Korean Haenyeo divers. Evidence indicates that repeated apnoea induces adaptations across multiple physiological systems. Haematological changes include increased spleen size and enhanced splenic contraction, augmenting circulating haemog

Acute ophthalmic artery occlusion in decompression illness with underlying anterior cerebral artery A1 segment hypoplasia.

A diver presented with total loss of vision in the left eye and right hemiparesis following a routine no-stop scuba dive to 20 metres' depth. A diagnosis of decompression illness (DCI) with acute ophthalmic artery air embolism and left carotid artery insult causing acute anterior circulatory ischaemia was made. He underwent seven hyperbaric treatments leading to a full recovery. Magnetic resonance angiography revealed an underlying left anterior cerebral artery A1 segment hypoplasia. Making a prompt diagnosis and early hyperbaric oxygen treatment are crucial to halt further tissue damage from

Anton's syndrome as a presentation of decompression illness.

We present a case of a patient with Anton's syndrome due to decompression illness (DCI) after recreational scuba diving. Visual anosognosia, or denial of loss of vision, which is associated with lack of awareness regarding visual loss in the setting of cortical blindness, is known as Anton's syndrome (also termed Anton-Babinski syndrome). Our patient presented with progressive neurological DCI treated with repeated recompression. The anosogosia resolved after 48 h. Subsequent echocardiography revealed a persistent (patent) foramen ovale.

[Scuba diving and the heart. Cardiac aspects of sport scuba diving].

Diving with self-contained underwater breathing apparatus (scuba) has become a popular recreational sports activity throughout the world. A high prevalence of cardiovascular disorders among the population makes it therefore likely that subjects suffering from cardiovascular problems may want to start scuba diving. Although scuba diving is not a competitive sport requiring athletic health conditions, a certain medical fitness is recommended because of the physical peculiarities of the underwater environment. Immersion alone will increase cardiac preload by central blood pooling with a rise in b

The indigenous fisherman divers of Thailand: diving practices.

Diving practices of a group of indigenous people living on Thailand's west coast were investigated. Village chiefs were first interviewed using a questionnaire. Three hundred and forty-two active divers were then interviewed by health care workers using a second questionnaire. Field observation was used to further develop information and confirm diving practices. Divers in 6 villages, whose basic means of making a living is from diving for marine products such as fish and shellfish, have diving patterns that put them at substantial risk of decompression illness. Breathing air from a primitive

Patent foramen ovale in underwater medicine.

The foramen ovale, between the right and left atria, exists in the foetal heart as a vital physiological communication. Haemodynamic closure occurs in the neonatal period with most people having permanent fusion of the foramen. In up to a third of adults the closure is functional only and a potential right to left atrial communication persists as a patent foramen ovale. Studies in patients with decompression illness after diving suggest a consistent increase in the prevalence of patent foramen ovale, as detected by transthoracic contrast echocardiography. The association is strongest for those

[Patients' condition one year following surgery for a lumbosacral radicular syndrome].

In 492 patients with a lumbosacral radicular syndrome caused by a lumbar intervertebral disc herniation or by a stenosis of the lumbar spinal canal, the situation 1 year after a root decompressing operation was compared with the situation before surgery. About 75% of the patients reported satisfactory improvement, while about 10% were not satisfied. The judgement of the physicians is somewhat more favourable: 80% and 5%, respectively. The best results are obtained in patients in whom the preoperative diagnosis of lumbar disc herniation is highly probable; the worst results are obtained in pati

Decompression illness in breath-hold divers: insights from an online survey.

INTRODUCTION: Breath-hold divers can surface with neurological symptoms consistent with nitrogen buildup in tissues or gas entry into the arterial circulation, collectively termed decompression illness (DCI). While DCI has historically been attributed to diving with compressed air, breath-hold divers have reported similar syndromes. The causes, diagnosis, and management of DCI in breath-hold divers is poorly understood. METHODS: We developed an online survey that queried breath-hold divers on the symptoms they experienced during decompression illness events and the medical management of each

Complications After Covid-19 Infection In Singapore Military Divers: A Retrospective Cohort Study.

Studies suggest that COVID-19 infections may have longer-term and more significant complications, even with mild or absent symptoms. This may predispose divers to pulmonary barotrauma, arterial gas embolisms, and reduced exercise tolerance, and impact physical and cognitive performance during diving. Military diving is physically, physiologically, and psychologically taxing on the individual. This study aims to assess the incidence of complications after COVID-19 infections in a cohort of active military divers and the incidence of diving-related injuries such as decompression sickness and bar

Mini Trampoline, a New and Promising Way of SCUBA Diving Preconditioning to Reduce Vascular Gas Emboli?

Background: Despite evolution in decompression algorithms, decompression illness is still an issue nowadays. Reducing vascular gas emboli (VGE) production or preserving endothelial function by other means such as diving preconditioning is of great interest. Several methods have been tried, either mechanical, cardiovascular, desaturation aimed or biochemical, with encouraging results. In this study, we tested mini trampoline (MT) as a preconditioning strategy. Methods: In total, eight (five females, three males; mean age 36 ± 16 years; body mass index 27.5 ± 7.1 kg/m2) healthy, non-smoking, div
