Scuba divers' pulmonary oedema: recurrences and fatalities.

Scuba divers' pulmonary oedema (SDPE) is an increasingly recognised disorder in divers. We report three fatal cases of SDPE, demonstrating its potential serious nature even in the absence of underlying cardiac disease demonstrable clinically or at autopsy. This, together with the frequency of recurrences, has implications on assessing fitness for subsequent diving, snorkelling and swimming. The differential diagnosis of this disorder is also considered, as is its possible inducement by salt water aspiration and its relationship to drowning.

Effect of hypercapnia on spleen-related haemoglobin increase during apnea.

BACKGROUND: Splenic contraction associated with apnea causes increased haemoglobin concentration and haematocrit (Hct), an effect that may promote prolonged breath-holding. Hypoxia has been shown to augment this effect, but hypercapnic influences have not been investigated previously. METHODS: Eight non-divers performed three series of apneas on separate days after inspiration of oxygen with different carbon dioxide (CO₂) levels. Each series consisted of three apneas 2 minutes apart: one with pre-breathing of 5% CO₂ in oxygen (O₂, 'Hypercapnia'); one with pre-breathing of 100% O₂ ('Normocapni

Suitability of the partially implantable active middle-ear amplifier Vibrant Soundbridge® to hyperbaric exposure.

INTRODUCTION: Active middle ear amplifiers represent a modern possibility to treat sensorineural, conductive and combined hearing loss. They can be in use in divers and patients who need hyperbaric oxygen therapy. Therefore, active middle-ear amplifiers have to be tested to determine whether or not they are prone to implosion or function loss in hyperbaric conditions. MATERIAL AND METHODS: We asked three of the companies registered by the German health authorities as manufacturers of active middle ear amplifiers to test their devices in hyperbaric conditions. Med-El agreed to support the stud

The use of drugs by UK recreational divers: illicit drugs.

AIMS: Anecdotal observations suggest the use of illicit drugs takes place amongst recreational divers but, to date, there has been little open debate within the diving community concerning possible prevalence. This study investigated the prevalence and type of illicit drugs used by recreational divers in the United Kingdom (UK). METHODS: Anonymous questionnaires were circulated via UK dive clubs, dive schools, dive shows and conferences. Questions incorporated diver and diving demographics and general health, which included anxiety, depression and panic attacks, alcohol use, smoking and illic

Predicting performance in competitive apnea diving. Part III: deep diving.

The first of these reviews described the physiological factors defining the limits of static apnea, while the second examined performance in apneic distance swimming. This paper reviews the factors determining performance in depth disciplines, where hydrostatic pressure is added to the stressors associated with apnea duration and physical work. Apneic duration is essential for performance in all disciplines, and is prolonged by any means that increases gas storage or tolerance to asphyxia or reduces metabolic rate. For underwater distance swimming, the main challenge is to restrict metabolism

S100B and its relation to intravascular bubbles following decompression.

INTRODUCTION: When neurological damage occurs in divers, it is considered to be caused by gas bubbles. Entrapment of these bubbles may lead to cellular injury and cerebral oedema. S100B is a protein biomarker that is released in CNS injuries and the concentration is related to the amount of brain damage. METHODS: A total of 27 rats were randomly assigned to one of three groups. Group I served as controls (n = 9). Group II (n = 7) underwent a simulated dive to 400 kPa and Group III to 700 kPa (n = 11). In groups II and III, venous gas bubble scores were evaluated by ultrasound during the first

Severe hydrogen sulphide poisoning treated with 4-dimethylaminophenol and hyperbaric oxygen.

UNLABELLED: INTRODUCTION Hydrogen sulphide (H2S) is a highly toxic gas which originates mainly during breakdown of organic matter under anaerobic conditions. After inhalation, H2S binds to mitochondrial respiratory enzymes preventing oxidative phosphorylation, thereby causing reversible inhibition of aerobic metabolism and cellular anoxia. The use of hyperbaric oxygen therapy (HBOT) for H2S poisoning remains controversial, but has a similar underlying rationale to that in carbon monoxide poisoning. METHODS: A retrospective review of patients with severe H2S intoxication who presented during 2

Analyser position for end-tidal carbon dioxide monitoring in a rebreather circuit.

INTRODUCTION: A diving rebreather currently nearing release incorporates an infra-red CO2 analyser at the end of the exhale hose and uses the expired gas CO2 measurement made at this position to detect hypercapnia. This configuration may allow exhaled anatomic and mouthpiece dead space gas to mix with alveolar gas in the exhale hose thus falsely lowering the CO2 measurement, especially at low tidal volumes. METHODS: A test circuit was constructed using a typical rebreather mouthpiece and exhale hose connected into an anaesthetic machine breathing loop. True end-tidal PCO2 was measured in gas

Predicting performance in competitive apnea diving, part II: dynamic apnoea.

Part I of this series of articles identified the main physiological factors defining the limits of static apnea, while this paper reviews the factors involved when physical work is added in the dynamic distance disciplines, performed in shallow water in a swimming pool. Little scientific work has been done concerning the prerequisites and limitations of swimming with or without fins whilst breath holding to extreme limits. Apneic duration influences all competitive apnea disciplines, and can be prolonged by any means that increase gas storage or tolerance to asphyxia, or reduce metabolic rate,

Hyperbaric oxygen in the treatment of asphyxia in two newborn infants.

Hypoxic-ischaemic encephalopathy (HIE) is a common cause of brain damage in the neonatal period. Approximately 10% of births involve some degree of asphyxia, and 1% of these are severe. Current treatment has been limited to supportive measures and the recent use of hypothermia. Beneficial effects of hyperbaric oxygen treatment (HBOT) in neonatal asphyxia have been reported in the Chinese literature. We report the use of HBOT to treat two term neonates with moderate HIE according to Sarnat's classification. Clinical improvement occurred following HBOT. A 50% decrease in the total creatine phosp

Malignant otitis externa: experience with hyperbaric oxygen therapy.

With the low incidence of malignant otitis externa (MOE) and its variable manifestations, it is not possible to perform relevant prospective controlled studies in this condition. Therefore, the best evidence on which to base therapeutic decisions is likely to remain retrospective reviews of case series. Even though our knowledge in regard to MOE is not greatly increased by the retrospective case series by Saxby et al in this issue, nevertheless it has value.1 The larger number of cases here confirms that multimodal therapy, with the inclusion of hyperbaric oxygen therapy (HBOT), enables a redu

Problems of a preclinical treatment algorithm for diving accidents: analysis of the Swiss hyperbaric situation.

Following recent changes in the medical infrastructure in Switzerland, the number of hyperbaric facilities fell from ten to two, and the existing algorithms for telemedical assistance of divers required review. A retrospective study of decompression illness (DCI) from 2004 to 2008 showed that many divers were asymptomatic at the start of hyperbaric treatment. Because of this and recent publications, we may need to modify in the future the decision to recompress a diver. The descriptive nomenclature for DCI was used to determine the probability that symptoms represent DCI. Secondly we evaluated

Scuba divers' pulmonary oedema. A review.

The literature on scuba divers' pulmonary oedema (SDPE) is reviewed, especially in its relationship to other immersioninduced pulmonary oedemas. It is concluded that although the three forms induced by swimming, freediving and scuba diving have some features in common, there are significant differences in their demographics, causation and clinical management. The swimming-induced cases tend to be young and fit, but exposed to excessive exertion. The freedivers experience extreme breath-holding and barotraumatic influences. The scuba divers are an older group and may have preexisting or occult

Hyperbaric oxygen therapy in the treatment of skin ulcers due to calcific uraemic arteriolopathy: experience from an Australian hyperbaric unit.

INTRODUCTION: Calcific uraemic arteriolopathy (CUA), also known as 'calciphylaxis', is a syndrome of ischaemic necrotic ulcers occurring in uraemic patients with end-stage renal failure. It is a debilitating condition with a high morbidity and mortality. Hyperbaric oxygen (HBO) has been used to treat such wounds for many years but evidence of its efficacy is limited. AIM: We aimed to study the efficacy of HBO on the healing of problem ulcers secondary to CUA. METHOD: A retrospective case review of all patients with chronic skin ulcers secondary to CUA treated at the Alfred Hospital Hyperbari

Diving-related pulmonary oedema as an unusual presentation of alcoholic cardiomyopathy.

Diving-related pulmonary oedema (DRPO) is an uncommon and incompletely understood phenomenon. Pulmonary oedema has been rarely documented in shallow water. It is also associated with cold water and exertion and has been seen in swimmers as well as divers with no underlying heart disease. We describe an otherwise well 69-year-old lady who developed diving-related pulmonary oedema on her second and third dives in a shallow, heated pool. Follow-up echocardiogram revealed moderate global left ventricular dysfunction with an ejection fraction of 37%. There was a history of alcohol consumption of ha

Hyperbaric oxygen therapy in the treatment of late-onset hemorrhagic cystitis after allogeneic hematopoietic stem cell transplantation.

INTRODUCTION: Hemorrhagic cystitis (HC) is a common complication after allogeneic hematopoietic stem cell transplantation (HSCT), characterized by inflammation and bleeding of the bladder. Hyperbaric oxygen therapy (HBOT) has been shown to be effective in the treatment of radiation-induced HC. However, the optimal treatment for HC after allogeneic HSCT has not yet been established. Furthermore, limited research has been conducted on the use of HBOT in this setting. This study aimed to evaluate the effectiveness and safety of HBOT in patients with late-onset HC after allogeneic HSCT. METHODS:

Livedoid vasculopathy: A multidisciplinary clinical approach to diagnosis and management.

Livedoid vasculopathy (LV) is a rare, chronic, and occlusive disease of the veins supplying the upper parts of the skin. The pathogenesis of the disease is not precisely understood, and its attacks are often unpredictable but tend to worsen during the summer. LV affects women more often. This increased risk for LV in women might be related to sex-specific physiological conditions, such as pregnancy, or a higher incidence of LV-associated conditions, such as connective tissue diseases, hypercoagulable states, and venous stasis in women. The typical clinical appearance of LV consists of three ma
